Background And Objectives: The Pediatric Emergency Care Applied Research Network Fluid Therapies Under Investigation in Diabetic Ketoacidosis (DKA) (FLUID) Trial found that rapid fluid infusion does not increase the risk of cerebral injury. Concern persists, however, whether fluid rates should be adjusted for overweight or obese patients. We used the FLUID Trial database to evaluate associations between fluid infusion rate and outcomes in these patients.

Asian Conscientization: Reflections on the Experiences of Asian Faculty in Academic Medicine.

Teach Learn Med

December 2024

Pediatrics and Centre for Health Education Scholarship, University of British Columbia, Vancouver, British Columbia, Canada.

Issue: Asians have experienced a rise in racialized hate crimes due to the anti-Asian rhetoric that has accompanied the COVID-19 pandemic. However, there has been little acknowledgement of anti-Asian discrimination within the medical education community. While anti-Asian hate is not new or unfamiliar to us, four authors of Asian descent, it has given us an opportunity to reflect on how we have been complicit in and resistant to the larger racial narratives that circulate in our communities.

  • Three sisters had a rare health condition that caused their uterus to be underdeveloped and made it hard for them to have regular periods, but they still had pregnancies in their fallopian tubes.
  • Researchers studied a specific gene called OSR1 to understand its role in the development of the uterus in both humans and mice.
  • The study showed that the OSR1 gene is crucial for the proper development of the uterus and could explain some cases of infertility related to uterine problems.

Article Synopsis
  • The study aimed to evaluate the effects of tralokinumab, a treatment for chronic moderate-to-severe atopic dermatitis, compared to a placebo on various clinically important outcomes in patients not reaching the main goal of clear skin at week 16.
  • A total of 1328 patients from two phase III trials, ECZTRA 1 and 2, were analyzed, focusing on improvements in eczema severity and patient-reported symptoms like itch and quality of life.
  • Results showed that significantly more patients using tralokinumab attained meaningful improvements in eczema severity (EASI-50), itch ratings, and quality of life compared to those on placebo, indicating tralokinumab's potential benefits beyond just achieving clear skin.

Article Synopsis
  • - Stasis dermatitis (SD) is an inflammatory skin condition linked to chronic venous insufficiency (CVI) that causes symptoms like pain, swelling, and itching, severely impacting patients' daily lives and overall well-being.
  • - As a progressive disorder, SD can lead to complications such as ulcerations, further deteriorating patients' quality of life and increasing healthcare costs due to misdiagnosis and the need for unnecessary treatments.
  • - While compression therapy and elevating the legs are primary treatments, they can be uncomfortable and hard for patients to manage, leading to non-adherence and disease progression; additionally, there are no approved medications specifically for treating inflammation in SD apart from topical corticosteroids.

Anti-Inflammatory Effects of Peripheral Dopamine.

Int J Mol Sci

September 2023

Division of Kidney Diseases and Hypertension, Department of Medicine, The George Washington School of Medicine and Health Sciences, Washington, DC 20037, USA.

Dopamine is synthesized in the nervous system where it acts as a neurotransmitter. Dopamine is also synthesized in a number of peripheral organs as well as in several types of cells and has organ-specific functions and, as demonstrated more recently, is involved in the regulation of the immune response and inflammatory reaction. In particular, the renal dopaminergic system is very important in the regulation of sodium transport and blood pressure and is particularly sensitive to stimuli that cause oxidative stress and inflammation.

Congenital vertical talus (CVT) is the presence of rigid flatfoot deformity characterized by hindfoot valgus and equinus. This foot deformity is associated with midfoot dorsiflexion and forefoot abduction due to a fixed dorsal dislocation of the navicular relative to the head of the talus. It is often underdiagnosed in children due to its similarity to other disorders of the foot.

Increasing Knowledge in the Requesting of Clinical Investigations.

J Physician Assist Educ

December 2023

Brogan Guest, PA-C, is a reader, St George's, University of London, London, United Kingdom .

Purpose: Overtesting, ordering diagnostic investigations that do not help diagnose or manage a patient, is well-recognized as a problem across multiple healthcare settings in developed countries. One of the reasons often cited for overtesting is a lack of confidence or knowledge, so this article addresses our attempt to reduce overtesting through an investigation-specific course for a physician associate (PA) program based in the United Kingdom. We found no evidence of pedagogical literature that focused on clinical diagnostics teaching and assessment for PAs, so we aim to be the first to provide this research.

Endoscopic management via retrograde ureteroscopic laser ablation of upper tract urothelial carcinoma (UTUC) has become the preferred treatment modality for low-risk tumors. The most popular ablative lasers over the past 15-20 years have been the holmium:yttrium-aluminum-garnet (Ho:YAG) and neodymium (Nd:YAG) lasers, but recently the thulium (Th:YAG) laser has emerged as a potential alternative. This review compares the mechanism of action, physiological properties and effects, and oncologic outcomes of Ho:YAG/Nd:YAG lasers versus the Th:YAG laser for UTUC treatment.

Article Synopsis
  • A total laryngectomy often requires a tracheoesophageal prosthesis for voice rehabilitation, typically done using an endoscope to puncture the tracheal wall.
  • In a case involving an elderly male with significant esophageal stenosis, the use of an endoscope wasn't possible, leading to the use of intraoperative ultrasound for visualizing the procedure.
  • This approach highlights the effectiveness of ultrasound technology in guiding tracheoesophageal punctures when traditional methods cannot be employed.

Racial discrimination is a well-known risk factor of racial disparities in health. Although progress has been made in identifying multiple levels through which racism and racial discrimination influences health, less is known about social factors that may buffer racism's associations with health. We conducted a systematic review of the literature with a specific focus on social connectedness, racism, and health, retrieving studies conducted in the United States and published between January 1, 2012, and July 30, 2022, in peer-reviewed journals.

Integration of a multilanguage electronic discharge medication platform may help decrease medication errors. This quality improvement (QI) project's primary aim (process measure) was to increase utilization in the electronic health record (EHR) of the integrated MedActionPlanPro (MAP) for cardiovascular surgery and blood and marrow transplant patients at hospital discharge and for the first clinic follow-up visit to 80% by July 2021.
