Introduction: Mycetoma is a chronic granulomatous infection, common throughout tropical regions, and is considered a neglected disease that mostly affects impoverished populations. Mycetoma is divided into eumycetoma, caused by fungi, and actinomycetoma, caused by filamentous bacteria. Clinical presentation is distinctive, and making the diagnosis is usually not difficult; however, access to safe and effective treatments is a major challenge.

Chronic thromboembolic pulmonary hypertension (CTEPH) is a rare but severe condition characterized by persistent obstruction and vascular remodeling in the pulmonary arteries following an acute pulmonary embolism (APE). Although APE is a significant risk factor, up to 25% of CTEPH cases occur without a history of APE or deep vein thrombosis, complicating the understanding of its pathogenesis. Herein, we carried out a narrative review discussing the mechanisms involved in CTEPH development, including fibrotic thrombus formation, pulmonary vascular remodeling, and abnormal angiogenesis, leading to elevated pulmonary vascular resistance and right heart failure.

Introduction: Obesity constitutes a complex global health that carries several comorbidities that include cardiovascular disease, diabetes, and cancer. Current treatments, such as lifestyle modifications and bariatric surgery, are often difficult to implement or carry risks, creating a need for alternative approaches. Methylphenidate (MPH), a drug commonly used to treat Attention Deficit and Hyperactivity Disorder (ADHD), has shown potential in regulating dopamine levels to modulate appetite and feeding behaviors.

Rev Alerg Mex

September 2024

Servicio de Neumología y Cirugía de Tórax; Facultad de Estudios Superiores Iztacala, Universidad Nacional Autónoma de México (UNAM), Tlalnepantla, Estado de México.

Article Synopsis
  • Allergic bronchopulmonary aspergillosis is a rare immune response to the fungus Aspergillus fumigatus, leading to symptoms like lung inflammation and high eosinophil counts in the blood.
  • A 42-year-old male with diabetes exhibited severe respiratory issues and was found to have high eosinophils and elevated IgE for the fungus; he was treated with oxygen, prednisone, and itraconazole, resulting in improvement.
  • The condition can be deadly, especially in immunocompromised individuals; diabetic patients have a higher prevalence of fungal infections, with nearly 37% potentially affected compared to only 4.7% in non-diabetics.

Background: Assessment of muscle strength is of great interest for the study of age-associated pathologies, including frailty and sarcopenia. However, the cost of some of the equipment necessary for its evaluation could be prohibitive for its generalized use in some settings.

Objective: The purpose of the present study was to compare two handheld dynamometers to evaluate the measure of concordance between the measurements.

Background: Accumulating evidence has proposed phase angle (PhA) as a marker for assessing cellular integrity and nutritional status in ulcerative colitis (UC) patients; the aim of the study was to evaluate the efficacy of PhA in assessing nutritional status in patients with UC, investigating its potential as a biomarker of disease activity.

Methods: We conducted a cross-sectional study in patients with UC and healthy controls. We determined PhA by electrical bioimpedance and categorized participants through bioelectrical impedance analysis.

November 2024

Coordinación Clínica de Turno, Hospital General de Zona No. 48, Órgano de Operación Administrativa Desconcentrada Ciudad de México Sur, Ciudad de México. México.

Objective: To compare the costs of care and the opportunity of care for the management of grade I-II ankle sprain in two Family Medicine Units, one with rehabilitation service (FMU 13) and one without rehabilitation service (FMU 41).

Method: Observational analytical study, records with diagnosis of grade I-II ankle sprain attended at the FMU were included January-November 2021. Consultations were recorded in the emergency department, family medicine, rehabilitation, cabinet studies and time to grant the appointment in the rehabilitation service (opportunity of care), the Mann-Whitney U test was used to compare costs of care and timeliness of care.

Background: Metabolic dysfunction-associated steatotic liver disease (MASLD), previously known as non-alcoholic fatty liver disease (NAFLD), is a prevalent hepatic condition linked to metabolic alterations. It gradually causes liver damage and potentially progresses to cirrhosis. Despite its significance, research, especially in the pediatric population, is limited, leading to contradictory findings in diagnosis and treatment.

  • KS is the most common cancer among HIV-positive individuals in developing countries, significantly impacting their quality of life.
  • The study included 28 male HIV-positive patients diagnosed with KS, revealing that most had low CD4+ counts, high viral loads, and common skin lesions associated with the disease.
  • Antiretroviral therapy demonstrated a long-term protective effect against KS, but opportunistic infections, particularly tuberculosis, were prevalent, emphasizing the need for a comprehensive approach in managing both KS and related infections.

Gestational diabetes (GD) is a metabolic disorder characterized by glucose intolerance during pregnancy, significantly impacting maternal and fetal health. Its global prevalence is approximately 14%, with risk factors including obesity, family history of diabetes, advanced maternal age, and ethnicity, which are linked to cellular and molecular disruptions in glucose regulation and insulin resistance. GD is associated with short- and long-term complications for both the mother and the newborn.

Background: Kidney transplant is the best replacement therapy for kidney function; However, donors are insufficient, so extended criteria donors (ECD) are needed, which, in well-selected recipients, provide better survival than being on dialysis. Dual kidney transplantation (DRT) should be offered to elderly patients with lower immunological risk and with a lower body mass index. DCE are considered donors who died due to stroke, ≥ 60 years old or aged between 50 and 59 years, with diabetes or previous hypertension, clinical proteinuria, estimated glomerular filtration rate (eGFR) between 30 and 60 mL/min, or Terminal serum creatinine > 2.

Background: Recently, magnetic and inertial measurement units (MIMU) based systems have been applied in the spine mobility assessment; this evaluation is essential in the clinical practice for diagnosis and treatment evaluation. The available systems are limited in the number of sensors, and neither develops a methodology for the correct placement of the sensors, seeking the relevant mobility information of the spine.

Methods: This work presents a methodology for analyzing a system consisting of sixteen MIMUs to reduce the amount of information and obtain an optimal configuration that allows distinguishing between different body postures in a movement.

Background: Vasculogenic mimicry (VM) is an alternative intratumoral microcirculation system that depends on the capacity of tumor cells to reorganize and grow in three-dimensional (3D) channel architectures like the capillaries formed by endothelial cells. Both VM and angiogenesis may coordinately function to feed cancer cells, allowing tumor growth. Long noncoding RNAs (lncRNAs) regulate critical cellular functions in cancer cells, including cell proliferation, apoptosis, angiogenesis, invasion, and metastasis.
