In this editorial, we comment on the article by Lyu published in the recent issue of the (2023; 2219-2840). Hepatocellular carcinoma (HCC) is a frequently encountered and highly aggressive primary liver cancer, which remains the third-commonest cause of cancer-related death despite the current therapeutic modalities. There is urgency in developing novel therapeutic approaches, such as by manipulating extracellular vesicles, which constitute a highly heterogeneous nanoparticle population that contains various cargoes.

Background: Malnutrition in esophageal and pharyngeal cancer patients constitutes a common and serious concern, which significantly reduces patients' prognoses. Cancers of the esophagus and the pharynx can considerably impair feeding in patients, resulting in severe undernutrition. Hepatocellular carcinoma (HCC) constitutes a frequent highly malignant form of primary liver cancer and is the third cause of death attributable to malignancy. Case: We report the case of an 82-year-old woman with diabetes, arteriosclerosis, chronic heart failure, and hypertension treated with an anatomical locking plate and multiple cables for a spiral-wedged periprosthetic fracture of the distal femur which was complicated by direct occlusion of the femoral artery and crush of the sciatic nerve, resulting in leg necrosis and, ultimately, through-knee disarticulation despite early recognition and arterial repair.

Conclusions: Neurovascular injury is a potential complication of cerclage cables placement around the femoral shaft during complex fracture fixation. Devastating complications cannot always be corrected despite early vascular intervention.
