Article Synopsis
  • Nutritional anemia affects around 1.6 billion people globally, with limited data on its connection to celiac disease (CeD) in Western India.
  • In a study of 116 patients with nutritional anemia, 16.3% tested positive for IgA anti-tissue transglutaminase antibodies, and 9.3% had biopsy-confirmed CeD.
  • Chronic diarrhea and short stature were significant predictors of CeD, suggesting the need for routine screening of nutritional anemia patients for CeD, particularly those who don’t respond to iron treatment.

Article Synopsis
  • A 34-year-old woman diagnosed with severe psychosis also showed signs of Cushing syndrome and underwent an 18 F-FDG PET/CT scan, which identified a hypermetabolic lung lesion and increased brain metabolism in the basal ganglia.
  • The lung lesion was surgically removed and confirmed as an adrenocorticotropic hormone-producing lung carcinoid through histopathological analysis.
  • After treatment, a follow-up 18 F-FDG PET scan revealed normal brain metabolism and a complete reversal of the psychosis, highlighting the potential for psychosis to be an initial symptom of Cushing syndrome and the usefulness of PET imaging in assessing brain changes.

Article Synopsis
  • The study analyzed data from ten clinical trials to see how different medications taken alongside advanced therapies for ulcerative colitis (UC) affect treatment outcomes.
  • Among the 6044 patients studied, many were using additional medications (like corticosteroids or 5-aminosalicylates), but most did not significantly alter the efficacy or safety of the advanced therapies.
  • However, using immunomodulators was linked to a lower chance of achieving clinical remission, especially with treatments that are not TNF antagonists.

The temporal bone is a complex neurovascular structure. The procedure of cochlear implantation is performed with in few millimetres of area around which important structures like internal carotid artery, jugular bulb and facial nerve are present. Displacement of an electrode into the carotid canal although rare is considered as a major complication in cochlear implant surgery.

Torsemide in Edema Associated with Chronic Kidney Disease.

J Assoc Physicians India

September 2024

Country Head, Medical Affairs, Cipla Ltd., Mumbai, Maharashtra, India.

Torsemide is a loop diuretic used to manage edema associated with chronic kidney disease (CKD) and acute kidney injury (AKI). It acts by inhibiting sodium and chloride ions reabsorption in the ascending limb of the loop of Henle, thereby increasing urine output and reducing fluid accumulation. Compared to other diuretics, torsemide has an extended duration of action, higher bioavailability, and its elimination route is primarily through the hepatic route, making it effective in patients with CKD and AKI.

Objectives: The primary objective of this prospective cohort study was to assess the usefulness of a predefined multidisciplinary care pathway-based management on pregnancy outcome(s) in women with SLE who already had at least one adverse obstetric outcome(s).

Methods: Between March 2010 and March 2023, all consecutive, consenting women with SLE who already had at least one previous adverse obstetric outcome (preterm labour, pre-eclampsia, termination of pregnancy, miscarriage, intrauterine growth retardation (IUGR), preterm birth, low birth weight (LBW), intrauterine death (IUD) or stillbirth] were prospectively screened and counselled. The protocol comprised preconception and post-natal drug and disease status review, periodic ante-natal visits for the monitoring of pregnancy and drug and disease status review and post-natal drug and disease status review and contraception advice.

Study of the Anatomical Variations of Spinal Accessory Nerve Seen During Neck Dissection.

Indian J Otolaryngol Head Neck Surg

June 2024

Department of Otorhinolaryngology and Head and Neck Surgery, Maulana Azad Medical College, Delhi, India.

In our study, thirty one neck dissections in thirty patients were performed as a part of their treatment for head and neck cancers over a period of one year. In this study, we aimed to report anatomical variations of the spinal accessory nerve (SAN) encountered during neck dissection with respect to important reference points and structures in the neck and correlate them with the length of the neck and height of patient. We preserved SAN in all the neck dissections and studied its course and branching in relation to internal jugular vein (IJV), sternocleidomastoid (SCM) muscle, greater auricular point (GAP), mastoid process, clavicle, angle of mandible, length of the neck and height of the patient.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) diagnosis is the need of the hour, as cases are persistently increasing, and new variants are constantly emerging. The ever-changing nature of the virus leading to multiple variants, has brought an imminent need for early, accurate and rapid detection methods. Herein, we have reported the design and fabrication of Screen-Printed Electrodes (SPEs) with graphene oxide (GO) as working electrode and modified with specific antibodies for SARS-CoV-2 Receptor Binding Domain (RBD).

Article Synopsis
  • The study assessed the risk of serious infections in Crohn's disease patients treated with adalimumab, focusing on how treatment response affects this risk.
  • Among 1,515 patients, those who responded well to treatment (50.4%) had significantly fewer moderate to severe symptoms and lower steroid and opiate usage compared to nonresponders.
  • Findings revealed that responders were 34% less likely to experience serious infections, suggesting effective treatment leads to better disease control and reduced infection risk.

Septic pulmonary embolism (SPE) is a rare complication that happens when infected thrombi from the original site of infection break off and travel to the pulmonary blood vessels, causing an infarction or an abscess. Cases were reported on SPE, with tricuspid or pulmonary valve endocarditis being the most common primary site, especially in intravenous drug abusers. There are, however, very few reports of SPE brought on by septic cavernous sinus thrombosis (CST).

Frequency of Positive Cuff Leak Test Before Extubation in Robotic Surgeries Done in Steep Trendelenburg Position.

Indian J Surg Oncol

December 2022

Department of Anaesthesiology, Critical Care and Pain, Tata Memorial Hospital, Homi Bhabha National Institute, Mumbai, Pin 400012 India.

Anaesthesia for robotic surgeries done in steep trendelenburg position are associated with risks such as facial oedema, conjunctival chemosis, raised intraocular pressure, laryngeal oedema, and delayed awakening. We proposed the use of the cuff leak test in them to record the frequency of laryngeal oedema at the end of surgery and attempted to find its correlation with probable risk factors. We conducted a prospective observational study of 100 patients aiming primarily to assess the frequency of positive cuff leak test in robotic abdominal surgeries performed in trendelenburg position.
