Total extraperitoneal approach for laparoscopic inguinal hernia repair (L-TEP) allows for better dissection, lesser chance of bowel injury, and quicker operating time. However robotic groin hernia repair is currently performed only through transabdominal route as it allows for more mobility of the arms. This study is aimed at studying the feasibility and outcomes of robotic totally extraperitoneal (R-TEP).

Background: Most studies on minimally invasive pancreatoduodenectomy (MIPD) combine patients with pancreatic and periampullary cancers even though there is substantial heterogeneity between these tumors. Therefore, this study aimed to evaluate the role of MIPD compared to open pancreatoduodenectomy (OPD) in patients with non-pancreatic periampullary cancer (NPPC).

Methods: A systematic review of Pubmed, Embase, and Cochrane databases was performed by two independent reviewers to identify studies comparing MIPD and OPD for NPPC (ampullary, distal cholangio, and duodenal adenocarcinoma) (01/2015-12/2021).

Objective: Assessment of minimally invasive pancreatoduodenectomy (MIPD) in patients with pancreatic ductal adenocarcinoma (PDAC) is scarce and limited to non-randomized studies. This study aimed to compare oncological and surgical outcomes after MIPD compared to open pancreatoduodenectomy (OPD) for patients after resectable PDAC from published randomized controlled trials (RCTs).

Methods: A systematic review was performed to identify RCTs comparing MIPD and OPD including PDAC (Jan 2015-July 2021).

Framework for Training in Minimally Invasive Pancreatic Surgery: An International Delphi Consensus Study.

J Am Coll Surg

September 2022

Department of General Surgery, Instituto Ospedaliero Fondazione Poliambulanza, Brescia, Italy (Korrel, Lof, Abu Hilal).

Background: Previous reports suggest that structured training in minimally invasive pancreatic surgery (MIPS) can ensure a safe implementation into standard practice. Although some training programs have been constructed, worldwide consensus on fundamental items of these training programs is lacking. This study aimed to determine items for a structured MIPS training program using the Delphi consensus methodology.

Article Synopsis
  • A systematic review of randomized trials was conducted to compare laparoscopic pancreatoduodenectomy (LPD) and open pancreatoduodenectomy (OPD), focusing on major complications and high-risk patient subgroups.
  • The study collected data from 224 patients, finding no significant difference in major complications or mortality rates between LPD and OPD, despite LPD having longer surgery times but shorter hospital stays.
  • Ultimately, LPD did not show a reduction in major postoperative complications compared to OPD, highlighting the need for further investigation in high-risk groups.

Nabothian cysts (NCs) are benign cervical mucous retention cysts, which are usually considered insignificant. A 45 years para 2, presented with lower abdominal heaviness, abnormal uterine bleeding and severe dysmenorrhea for 1 year. On examination, only a part of the anterior cervical lip was visible, and a tense bulging cyst was replacing the posterior lip.

Coronavirus disease-2019 (COVID-19) pandemic has affected liver transplantation in many ways. There is risk of infection to the transplant recipients; and COVID-19 is associated with significant risk of mortality in patients on wait list. The Liver Transplant Society of India (LTSI) has prepared guidelines regarding selection of adult and pediatric patients for liver transplantation, transplant for acute liver failure, use of deceased donor organs, transplant techniques and minimally invasive donor hepatectomy, pre- and postsurgery testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-related coronavirus disease 2019 in donors and recipients, role of COVID-19 antibody testing, shifting of recipients from COVID-19 to non-COVID-19 areas after recovery, isolation policy of team members exposed to COVID-19 patients, drug therapy of proven or suspected COVID-19 infection early posttransplant, care of SARS-CoV-2 positive donors and recipients and a separate COVID-19 consent for surgery.

Article Synopsis
  • - Thoracolaparoscopic esophagectomy (TLE) shows better short-term outcomes for esophageal cancer compared to open esophagectomy, while the use of robot-assisted laparoscopic esophagectomy (RALE) is less established.
  • - A study analyzed patients undergoing TLE and RALE, focusing on lymph node yield, surgery duration, cost, and other complications, with 90 patients in TLE and 25 in RALE, matched to 22 each for comparison.
  • - Results indicated that while both techniques had similar lymph node yields and comparable post-operative outcomes, RALE had longer surgeries and higher costs than TLE, indicating it may not be the preferred choice despite similar oncological results.

Introduction: Data on laparoscopic treatment of operable gastric cancer from India is sparse. This study aims to document outcomes of laparoscopic D2 gastrectomy in an Indian population.

Materials And Methods: Data of patients who underwent laparoscopic D2 gastrectomy for operable advanced gastric cancer between February 2012 and January 2017 were collected from electronic hospital records supplemented by telephonic interviews and analyzed.

Posterior gastric vessel is commonly encountered (4-99%) during surgery of the stomach, spleen, and pancreas. Due to high variations in its origin, and course, it has not been unanimously described in literature. Its significance in bariatric surgery is largely neglected.

Background: Laparoscopy for ventral hernia repair is now an established technique with its proven benefits of less pain, early recovery, low-recurrence rate as compared to open repair. Several techniques have been described such as IPOM, MILOS, TES, EMILOS, SCOLA, e-TEP. e-TEP was originally conceptualized as an alternative approach to inguinal hernia in difficult cases (obese, previous scars) and for training surgery residents.

Background: Laparoscopic sleeve gastrectomy (LSG) is the most commonly performed metabolic surgery worldwide. There are few mid- to long-term studies for LSG, especially from the Indian subcontinent.

Objective: The primary outcome of the study was percent total weight loss (%TWL), and secondary outcomes included type 2 diabetes mellitus remission (T2DM) rates, comorbidity resolution rates, revisional surgeries, and complications related to LSG, 3 and 5 years after surgery.

Laparoscopic choledochoduodenostomy as a reliable rescue procedure for complicated bile duct stones.

Surg Endosc

April 2018

Department of Surgical Gastroenterology and HPB Surgery, GEM Hospital and Research Center, 45/A, Pankaja Mill Road, Ramanathapuram, Coimbatore, Tamil Nadu, 641045, India.

Background: Endoscopic retrograde cholangiopancreatography (ERCP) with stone extraction is generally accepted as first line management for common bile duct (CBD) stones. CBD exploration, either by open or laparoscopic approach nowadays, is usually reserved for ERCP failures, complicated stone locations, along with altered anatomical situations. The aim of this study was to highlight the increasing role of laparoscopic choledochoduodenostomy which is not only a reliable but also as a rescue procedure for those failed ERCP cases due to complicated bile duct stones.

Performing complex surgical procedures in patients with situs inversus totalis is a challenge because of the need to orient to the completely mirror transposed abdominal and thoracic viscera. We report our experience in performing a three phase minimally invasive (thoracoscopic and laparoscopic) esophagectomy for carcinoma of the mid esophagus in a patient with situs inversus totalis. We believe that this is the first reported case of this kind.
