Maternal and fetal outcomes in an Italian multicentric cohort of women with multiple sclerosis exposed to dimethyl fumarate during pregnancy.

Mult Scler

October 2024

Multiple Sclerosis Clinical and Research Unit, Department of Systems Medicine, Fondazione PTV Policlinico Tor Vergata, Tor Vergata University, Rome, Italy.

Article Synopsis
  • The study investigates the effects of discontinuing dimethyl fumarate (DMF) during early pregnancy in women with multiple sclerosis (MS), analyzing 137 pregnancies from Italian MS Centers.
  • Results show that disease activity typically decreases during pregnancy but increases postpartum; higher relapse rates before conception correlate with faster relapses after giving birth.
  • Importantly, DMF exposure during early pregnancy did not negatively affect fetal outcomes, suggesting it is safe for the pregnancy context.

Non-bacterial thrombotic endocarditis (NBTE) is a form of non-infective endocarditis characterized by the deposition of sterile fibrin and platelets on cardiac valves. Even though some studies have identified important pathophysiological features, many aspects remain poorly understood. Given its wide availability, transthoracic echocardiography is typically the initial diagnostic approach to the patient.

Article Synopsis
  • - The study aims to compare immediate versus staged percutaneous coronary intervention (PCI) strategies for patients with aneurysmatic right coronary artery (ARCA) during acute coronary syndrome (ACS), analyzing outcomes from 85 patients.
  • - Results show that both PCI strategies had similar rates of procedural success and long-term outcomes, but the staged approach led to a higher incidence of bleeding and longer hospital stays.
  • - The findings suggest that while both methods are viable, immediate PCI might be preferable due to lower complications and shorter hospital duration.

Deep learning approaches for the detection of scar presence from cine cardiac magnetic resonance adding derived parametric images.

Med Biol Eng Comput

August 2024

Department of Electronics, Information and Biomedical Engineering, Politecnico di Milano, P.zza L. da Vinci 32, 20133, Milan, Italy.

This work proposes a convolutional neural network (CNN) that utilizes different combinations of parametric images computed from cine cardiac magnetic resonance (CMR) images, to classify each slice for possible myocardial scar tissue presence. The CNN performance comparison in respect to expert interpretation of CMR with late gadolinium enhancement (LGE) images, used as ground truth (GT), was conducted on 206 patients (158 scar, 48 control) from Centro Cardiologico Monzino (Milan, Italy) at both slice- and patient-levels. Left ventricle dynamic features were extracted in non-enhanced cine images using parametric images based on both Fourier and monogenic signal analyses.

Int J Cardiol

October 2024

Department of Perioperative Cardiology and Cardiovascular Imaging, Centro Cardiologico Monzino IRCCS, Milan, Italy; Department of Biomedical, Surgical and Dental Sciences, University of Milan, Milan, Italy. Electronic address:

Article Synopsis
  • The study aimed to determine if feature-tracking strain measurements during stress cardiac MRI could help predict long-term risks of major adverse cardiac events (MACE) in patients with coronary artery disease (CAD) without needing contrast agents.
  • After analyzing 729 patients over about 6 years, the presence of late gadolinium enhancement (LGE) and stress global circumferential strain (GCS) were found to be independent indicators of MACE, occurring in 9.6% of the patients.
  • Ultimately, the research demonstrated that a contrast-free stress cardiac MRI with stress GCS could provide similar prognostic information as traditional myocardial perfusion assessments, highlighting its importance for patients with severe renal issues.

Introduction: The effectiveness of early interventions in young autistic children is well established, but there is great interindividual variability in treatment response. Predictors of response to naturalistic developmental behavioral interventions (NDBI), like the Early Start Denver Model (ESDM), are needed.

Methods: We conducted an exploratory study to prospectively seek predictors of response in 32 young children treated with ESDM after receiving an ASD diagnosis.

Article Synopsis
  • Diet plays a significant role as a risk factor for gestational diabetes mellitus (GDM), but studies on the link between women's diet and glucose tolerance during pregnancy are limited.
  • This study aimed to explore how previous dietary habits affect the results of glucose tolerance tests (GTT) in women with GDM, focusing on the potential inverse relationship between antioxidant micronutrient intake and abnormal GTT results.
  • Findings indicated that women with only one abnormal GTT value had higher fiber, vitamin D, and vitamin C intake compared to those with two or more abnormal values, suggesting a connection between these nutrients and better glucose tolerance.

Early adiposity rebound: predictors and outcomes.

Ital J Pediatr

May 2024

Department of Human Pathology of Adulthood and Childhood, University of Messina, Via Consolare Valeria, 98124, Messina, Italy.

Adiposity rebound (AR) refers to the second rise of the body mass index (BMI) curve that usually occurs between six and eight years of age. AR timing has a significant impact on patients' health: early AR (EAR), usually before the age of five, is considered to be the earliest indicator of obesity and its related health conditions later in life. Many studies have evaluated factors that can be predictors of EAR, and identified low birth weight and gestational weight gain as novel predictors of EAR, highlighting the role of the intrauterine environment in the kinetics of adiposity.
