It has been hypothesized that the biopsychosocial stress associated with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic, in combination with the immunological effects of SARS-CoV-2 and pancreatic β-cell dysfunction, may contribute to the onset of type 1 diabetes (T1D) in children. In this study, we documented the incidences of T1D in Yamanashi Prefecture, Japan, from 1986 to 2018, and expanded the analysis to include cases from 2019 to 2022 to evaluate the potential influence of coronavirus disease 2019 (COVID-19) on T1D incidence. The COVID-19 pandemic period was defined as 2020 to 2022.

Takotsubo syndrome induced by severe hyponatraemia in mineralocorticoid-responsive hyponatraemia of the elderly: a case report.

Eur Heart J Case Rep

October 2024

Division of Cardiology, Department of Internal Medicine, Showa University Fujigaoka Hospital, 1-30 Fujigaoka, Aoba-ku, Yokohama, Kanagawa 227-8501, Japan.

Article Synopsis
  • An 85-year-old woman's severe hyponatraemia (low sodium levels) led to transient loss of consciousness and subsequent cardiac dysfunction, ultimately diagnosed as Takotsubo syndrome, indicating a heart condition triggered by stress.
  • The patient was treated with hypertonic saline to gradually correct her low sodium levels, which required careful monitoring to avoid complications, and her condition improved over 14 days.
  • This case highlights the importance of promptly treating severe hyponatraemia in elderly patients, raises concerns about current treatment protocols for mineralocorticoid-responsive hyponatraemia (MRHE), and suggests a possible link between MRHE and acute cardiomyopathy.

Background And Objectives: Although intraprocedural rupture (IPR) is rare, it is a devastating complication of endovascular treatment (EVT) for intracranial aneurysms. Very few studies have been conducted on IPR, and the safety and efficacy of management techniques of IPR have not been investigated.

Methods: Patients who experienced IPR during EVT between 2013 and 2022 were enrolled from a multicenter observational registry.

Introduction: In Kawasaki disease (KD), accurate prediction of intravenous immunoglobulin (IVIG) resistance is crucial to reduce a risk for developing coronary artery lesions.

Objective: To establish a simple scoring model predicting IVIG resistance in KD patients based on the machine learning model.

Methods: A retrospective cohort study of 1002 KD patients diagnosed at 12 facilities for 10 years, in which 22.

This study presents a new idea for estimating the number of measurements required for determining the uncertainty in obtaining the desired water absorbed dose using the variation obtained from multicenter absorbed dose measurement data. The number of dose measurements depends on the performance of each linear accelerator (LINAC) tested and the dosimetry equipment and procedure used. However, optimising the number of data collected for the absorbed dose to water has been inadequately reported.

Acquired cystic lung disease in premature infants is a serious respiratory complication, and pulmonary interstitial emphysema (PIE) has been widely reported. We report a rare case of giant pulmonary bulla in an infant treated with bullectomy where chest computed tomography was useful in directing treatment.

Microglia modulate cardiorespiratory activities during chronic hypoxia. It has not been clarified whether microglia are involved in the cardiorespiratory responses to acute hypoxia. Here we investigated this issue by comparing cardiorespiratory responses to two levels of acute hypoxia (13% O for 4 min and 7% O for 5 min) in conscious unrestrained rats before and after systemic injection of minocycline (MINO), an inhibitor of microglia activation.

Neuroendocrine prostate cancer (NEPC) is rare, with short overall survival, and no established standard therapy. Therefore, the management of NEPC is often challenging. We report a case of a 63-year-old male diagnosed with NEPC and treated with multimodal examinations and therapies.

Background: Psychological stress has been reported to cause hyperthermia. Persistent excessive hyperthermia can, in turn, cause hypercytokinemia and organ damage. We report a case of postoperative severe hyperthermia leading to a systemic inflammatory response and multiple organ failure in a child with Down syndrome.

Background: Kawasaki disease is suspected to be triggered by previous infection. The prevention measures for coronavirus disease 2019 (COVID-19) have reportedly reduced transmission of certain infectious diseases. Under these circumstances, the prevention measures for COVID-19 may reduce the incidence of Kawasaki disease.

Introduction: Several studies have examined the incidence of childhood T1DM in Japan from the 1970s onwards, but none have been long-term studies using registration data. We estimate the incidence of childhood type 1 diabetes mellitus (T1DM) from 1986 to 2018 in Yamanashi Prefecture, Japan.

Methods: We began a population-based, long-term study of childhood T1DM in 1986 involving every hospital paediatrics department in Yamanashi Prefecture.

Aim: The landscape of cancer-related genetic aberrations in hepatocellular carcinoma (HCC) has gradually become clear through recent next-generation sequencing studies. However, it remains unclear how genetic aberrations correlate with imaging and histological findings.

Methods: Using 117 formalin-fixed paraffin-embedded specimens of primary liver tumors, we undertook targeted next-generation sequencing of 50 cancer-related genes and digital polymerase chain reaction of hTERT.

Article Synopsis
  • * A systematic review and the modified Delphi method were used to create 29 QIs, and we found that measuring adherence to 6 of these QIs was feasible based on data from over 396,000 stroke patients.
  • * The study revealed significant variation in adherence rates among hospitals, indicating that the DPC database can efficiently collect data for evaluating stroke care quality at a lower cost.

This study assessed abdominal organ motion induced by gastroduodenal motilities in volunteers during fasting and postprandial states, using cine magnetic resonance imaging (cine-MRI). Thirty-five volunteers underwent cine-MRI while holding their breath in the fasting and postprandial states. Gastric motility was quantified by the amplitude and velocity of antral peristaltic waves.

GH-secreting pituitary adenomas (GHomas) are rare in the pediatric population. Guanine nucleotide-binding protein, alpha stimulating (GNAS) somatic mutations are often found in patients with GHoma. Here, we report an 8-year-old girl with GH-secreting pituitary adenoma successfully treated by operative tumor resection and postoperative treatment with octreotide long-acting release (LAR).

: Selegiline enhances the patient's endogenous dopamine by inhibiting dopamine metabolism. The efficacy of selegiline monotherapy for drug-naïve Parkinson's disease (PD) patients may depend on the degree of dopaminergic neuronal degeneration. I-Ioflupane single photon emission computed tomography (SPECT) and I-meta-iodobenzylguanidine (MIBG) myocardial scintigraphy are diagnostic methods to assess the pharmacological and pathological changes in PD.

The Effects of Percutaneous Endoscopic Gastrostomy on Quality of Life in Patients With Dementia.

Gastroenterology Res

February 2012

Department of Neurology, The Jikei University School of Medicine, Aoto Hospital, 6-41-2 Aoto, Katsushika-ku, Tokyo, 125-8506, Japan.

Background: To examine the effects of percutaneous endoscopic gastrostomy (PEG) on quality of life (QOL) in patients with dementia.

Methods: We retrospectively included 53 Japanese community and tertiary hospitals to investigate the relationship between the newly developed PEG and consecutive dementia patients with swallowing difficulty between Jan 1st 2006 and Dec 31st 2008. We set improvements in 1) the level of independent living, 2) pneumonia, 3) peroral intake as outcome measures of QOL and explored the factors associated with these improvements.
