Background/aim: The combination of venetoclax (VEN) and azacitidine (AZA) (VEN+AZA) leads to higher complete remission rates and longer overall survival (OS) in patients with untreated acute myeloid leukemia (AML) who are ineligible for intensive combination chemotherapy. In practice, the doses of VEN and AZA are reduced at the attending physician's discretion to avoid adverse events; however, the impact of dose and duration reductions has not been fully clarified. We analyzed whether the efficacy was maintained with reduced VEN+AZA compared to AZA monotherapy in the real world.

  • - The study aimed to assess how hospitals' radiology departments manage power outages during disasters by surveying 600 hospitals nationwide, focusing on power availability and its effect on medical equipment.
  • - Out of the hospitals surveyed, 40.3% responded, revealing that 55.8%-68.2% could use their private generators for equipment like CT and digital radiography during outages, but up to 40.7% lacked access to medical information systems.
  • - The survey highlighted frequent equipment malfunctions (81.4%) due to outages, emphasizing the need for disaster drills to prepare for power loss scenarios.

Background: Neurointerventionalists must pay close attention to multiple devices on multiple screens simultaneously, which can lead to oversights and complications. Artificial intelligence (AI) has potential application in recognizing and monitoring these devices on fluoroscopic imaging.

Methods: We report out preliminary experience with a real time AI assistance software, Neuro-Vascular Assist (iMed technologies, Tokyo, Japan), in six patients who underwent carotid artery stenting.

  • Endoscopic mucosal dissection (ESD) is a common treatment for early gastric cancer, but it can lead to post-ESD ulcers in 4.4% of patients; this study examines whether PuraStat, a new hemostatic solution, can prevent related bleeding.
  • The research involved 101 patients who received PuraStat after ESD and compared them to a control group of 297 patients; results showed similar bleeding rates between both groups, with PuraStat not significantly reducing post-ESD bleeding.
  • Despite PuraStat not being linked to reduced bleeding, factors like larger resection sizes and anticoagulant use increased the risk of bleeding after ESD, indicating other underlying influences on the treatment

This study aimed to explore the relationship between gray matter volume changes and various clinical parameters in patients with migraine, focusing on symptom severity, quality of life, and states of depression and anxiety. Using a case-control design, we examined 33 patients with migraine, with or without aura, and 27 age-matched healthy subjects. We used magnetic resonance imaging to assess the volumes of 140 bilateral brain regions.

  • This study investigates the time from diagnosis to surgery for osteonecrosis of the femoral head (ONFH) and factors affecting its severity among 2074 cases in Japan from 1997 to 2018.
  • The median time to surgery was found to be 9 months, influenced by whether the patient used alcohol or corticosteroids, and the stage of joint destruction at diagnosis.
  • The study highlights that a large proportion of patients remain untreated for extended periods, but surgery was performed within 9 months for more than half of the patients, indicating the disease's serious impact.

  • Simple hepatic cysts (SHC) are usually harmless and discovered by chance during imaging; treatment is only necessary for symptomatic cases.
  • The study analyzed the safety and effectiveness of endoscopic ultrasonography (EUS)-guided drainage for treating symptomatic SHC, comparing it to traditional methods like percutaneous catheter drainage (PCD) with minocycline sclerotherapy.
  • Results showed that EUS-guided drainage was successful with a 100% reduction in cyst size and a shorter hospital stay (7 days), making it a promising alternative to PCD and surgery.

: Endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) is an alternative for failed endoscopic retrograde cholangiopancreatography (ERCP), with current success rates of 65-84% considered suboptimal. A novel ERCP catheter (SHOREN, Kaneka Corporation, Osaka, Japan) with a tapered 3.3-French tip may facilitate smoother insertion, potentially improving outcomes.

Sublingual immunotherapy is a widely used treatment, and serious adverse reactions such as anaphylaxis are rare. We report two cases of laryngeal edema as adverse reactions to sublingual immunotherapy, which could be continued due to a change in the administration method. Case 1 presents a 15-year-old male suspected to have had anaphylaxis due to the dust at the age of 6 years.

Video-assisted thoracic surgery esophagectomy (VATS-E) may increase the risk of postoperative nausea and vomiting (PONV) because it uses a high dosage of anesthesia through a long operative duration. However, no study has examined the risk factors for PONV after VATS-E. Therefore, we investigated the risk factors for PONV to support the appropriate risk management of PONV after VATS-E.

Jejunal vein tributary analysis and intraoperative clamp testing in avoiding congestion after pancreatoduodenectomy.

Langenbecks Arch Surg

March 2024

Department of General and Gastroenterological Surgery, Showa University Fujigaoka Hospital, 1-30 Fujigaoka, Aoba-ku, Yokohama, Kanagawa, 227-8501, Japan.

Purpose: Although venous drainage of the jejunal loop may be maintained after sacrifice of jejunal vein tributaries during pancreatoduodenectomy, risk of severe jejunal mesenteric congestion following division of these tributaries can be difficult to predict. This study considered how best to predict safety of jejunal vein tributary dissection.

Methods: Preoperative imaging findings and results of intraoperative clamp tests of jejunal vein tributaries during pancreatoduodenectomy were analyzed in 121 patients with hepatobiliary and pancreatic disease to determine whether this information adequately predicted safety of resecting superior mesenteric vein branches.
