We describe a case of intracranial hypotension syndrome due to overdrainage of cerebrospinal fluid presented with hearing loss after ventriculoperitoneal shunting procedure. A 69-year-old man suffering from subarachnoid hemorrhage presented with an angiogram showing two aneurysms, one of the right internal carotid and one of the middle cerebral artery. Neck clipping was performed.

The clinical response and QOL were examined with 16 outpatients treated with home hepatic arterial infusion chemotherapy for liver metastasis from colorectal cancer. Fluorouracil 1,000 mg/body/week was administered by 5-hour intra-arterial infusion with a portable disposable pump. Six out of 8 who replied to a questionnaire survey led a their usual life during the treatment, and the average hospitalization period was only 7 days (including mortal cases).

Terminal stage was informed to the incurable cancer patient, suffering severe pain and riding wheel chair in the hospital. Whether her therapy had been continued in the hospital or in her house has been discussed several times with patient her self or her family. We decided to continue her final therapy in her house, however we had to give up the therapy in her house because of so many problems.

Recently, in consideration of cost performance and improvement in quality of life (QOL) of the patient, the significance of home care and outpatient treatment has also been recognized in Japan. A terminal cancer patient who had local recurrence in the pelvic region after surgery for rectum carcinoma, was treated for cancer pain by us with a continuous epidural infusion of morphine by means of a prolonged operating continuous infusion device. The treatment made home therapy possible and allowed the patient to return to normal social activities.
