Objective: To describe and discuss potential mechanisms for modulation of visual hallucinations by nystagmus.

Methods: We present 2 patients with coexistent Charles Bonnet syndrome and periodic alternating nystagmus in the context of acquired visual loss.

Results: The combination has given rise to a rare phenomenon: visual hallucinations that move in a manner governed by the nystagmus, specifically by the direction and velocity of the slow phase.

Unlabelled: We describe a 40-year-old woman with von Hippel-Lindau syndrome, secondary chronic exudative retinal detachment, and white cataract with consequent leukocoria. Because the cataract was contracted and calcified, a black Morcher 85F poly(methyl methacrylate) IOL was implanted in the ciliary sulcus anterior to the lens to prevent posterior capsule rupture and potential destabilization of the retinal detachment with a risk for phthisis. Excellent centration of the black IOL was achieved with good cosmetic outcome and without postoperative uveitis or glaucoma.

Unlabelled: We describe a technique that uses flexible iris hooks to dilate and stabilize the capsular bag in a patient with a very deep set and small eye, narrow palpebral fissure, shallow anterior chamber, and very small pupil, who was scheduled for routine phacoemulsification for a white cataract. At the time of iris hook placement, the capsular bag was noted to be markedly unstable. The patient was managed successfully with phacoemulsification of the lens using 4 iris hooks to dilate the pupil and 4 iris hooks to stabilize the capsular bag during surgery.

We describe 2 cases of contact lens-related microbial keratitis caused by infection with Pseudomonas aeruginosa in which perineural infiltrates were observed at presentation. In both cases, examination by confocal microscopy was negative for Acanthamoeba cysts but bacterial cultures and microscopy of corneal scrapings were positive for P aeruginosa. Both cases responded rapidly to treatment with topical levofloxacin with no significant long-term sequelae.
