Survival with Trastuzumab Emtansine in Residual HER2-Positive Breast Cancer.

Background: Patients with human epidermal growth factor receptor 2 (HER2)-positive early breast cancer with residual invasive disease after neoadjuvant systemic therapy have a high risk of recurrence and death. The primary analysis of KATHERINE, a phase 3, open-label trial, showed that the risk of invasive breast cancer or death was 50% lower with adjuvant trastuzumab emtansine (T-DM1) than with trastuzumab alone.

Methods: We randomly assigned patients with HER2-positive early breast cancer with residual invasive disease in the breast or axilla after neoadjuvant systemic treatment with taxane-based chemotherapy and trastuzumab to receive T-DM1 or trastuzumab for 14 cycles.

In Immune-mediated Thrombotic Thrombocytopenic Purpura (iTTP), patients develop antibodies against ADAMTS13. The majority of patients exhibit inhibitory anti-spacer antibodies. Non-inhibitory antibodies binding to the carboxy-terminal CUB domains have been suggested to enhance the clearance of ADAMTS13 in iTTP.

Testing the effectiveness of a new French assisted resilience program among long-term unemployed people: a randomized control trial.

Long-term unemployment can affect life satisfaction, reduce well-being and increase mental health problems. Promoting psychological resilience to support the ability to cope with life challenges such as unemployment, may be a useful strategy. A specific program named AD-Ré was designed for Long-Term Unemployed (LTU) people in order to improve their mental health by fostering psychological resilience.

Wave ripples formed in ancient, ice-free lakes in Gale crater, Mars.

Symmetrical wave ripples identified with NASA's Curiosity rover in ancient lake deposits at Gale crater provide a key paleoclimate constraint for early Mars: At the time of ripple formation, climate conditions must have supported ice-free liquid water on the surface of Mars. These features are the most definitive examples of wave ripples on another planet. The ripples occur in two stratigraphic intervals within the orbitally defined Layered Sulfate Unit: a thin but laterally extensive unit at the base of the Amapari member of the Mirador formation, and a sandstone lens within the Contigo member of the Mirador formation.

Insights on the evolution and adaptation toward high-altitude and cold environments in the snow leopard lineage.

How snow leopard gradually adapted to the extreme environments in Tibet remains unexplored due to the scanty fossil record in Tibet. Here, we recognize five valid outside-Tibet records of the snow leopard lineage. Our results suggest that the snow leopard dispersed out of the Tibetan Plateau multiple times during the Quaternary.

Tracking bottom-fishing activities in protected vulnerable marine ecosystem areas and below 800-m depth in European Union waters.

Numerous studies have highlighted bottom-contact fishing gears as the primary threat to vulnerable marine ecosystems (VMEs). In November 2022, the European Commission closed 87 VME protection polygons to bottom fishing in European waters. Using public automatic identification system (AIS) data, we found an 81% decrease in bottom-contact fishing effort within these areas in the year following the closures.

The Roles of Empathy and Theory of Mind in Sexual Violence Perpetration: An Integrated Social Information Processing Framework.

Empathy is an aspect of social cognition which has gained much attention to explain sexual violence perpetration, with research resulting in mixed and contradictory findings. Another aspect of social cognition, which has been linked to, and even used synonymously with, cognitive empathy is Theory of Mind (ToM). However, research is severely lacking on the role which ToM may play in sexual violence.

Impact of seasonal malaria chemoprevention timing on clinical malaria incidence dynamics in the Kedougou region, Senegal.

Seasonal malaria chemoprevention (SMC) with sulfadoxine-pyrimethamine and amodiaquine is recommended by the World Health Organization since 2012 for clinical malaria prevention in children in the Sahelian region of Africa. In Senegal, SMC implementation began in 2013 and is given to children under 10 years old. This study aimed to describe clinical malaria incidence in the general population during routine SMC implementation and to analyse how SMC timing impacted clinical malaria dynamics in eligible children.

Outcomes and challenges in adolescents living with HIV in France: a nationwide cohort study over 35 years (1985-2020).

Objective: Most data published on adolescents living with HIV (ALH) have been collected before the large diffusion of second-generation integrase strand transfer inhibitors (INSTI) among the pediatric population. We analyzed the nationwide ANRS-MIE CO10 Pediatric cohort to assess the changes over time in health and social outcomes of French ALH.

Design: The cohort enrolled children born in France since 1985 and, from 2005, children diagnosed with HIV at ≤13 years, including those born abroad if antiretroviral-naive at first medical care in France.

Binaural speech intelligibility for combinations of noise, reverberation, and hearing-aid signal processing.

Binaural speech intelligibility in rooms is a complex process that is affected by many factors including room acoustics, hearing loss, and hearing aid (HA) signal processing. Intelligibility is evaluated in this paper for a simulated room combined with a simulated hearing aid. The test conditions comprise three spatial configurations of the speech and noise sources, simulated anechoic and concert hall acoustics, three amounts of multitalker babble interference, the hearing status of the listeners, and three degrees of simulated HA processing provided to compensate for the noise and/or hearing loss.

Tuning the electronic structure and SMSI by integrating trimetallic sites with defective ceria for the CO reduction reaction.

Heterogeneous catalysts have emerged as a potential key for closing the carbon cycle by converting carbon dioxide (CO) into value-added chemicals. In this work, we report a highly active and stable ceria (CeO)-based electronically tuned trimetallic catalyst for CO to CO conversion. A unique distribution of electron density between the defective ceria support and the trimetallic nanoparticles (of Ni, Cu, Zn) was established by creating the strong metal support interaction (SMSI) between them.

Daily life for humans and other animals requires switching between periods of threat- and reward-oriented behavior. We investigated neural activity associated with spontaneous switching, in a naturalistic task, between foraging for rewards and seeking information about potential threats with 7T fMRI in healthy humans. Switching was driven by estimates of likelihood of threat and reward.

Vestibular stimulation and space-time interaction affect the perception of time during whole-body rotations.

Among the factors, such as emotions, that distort time perception, vestibular stimulation causes a contraction in subjective time. Unlike emotions, the intensity of vestibular stimulation can be easily and precisely modified, making it possible to study the quantitative relationship between stimulation and its effect on time perception. We hypothesized that the contraction of subjective time would increase with the vestibular stimulation magnitude.

Effect of fatigability on sprint time performance and force-velocity profile according to maturity status in young rugby players.

Little is known about the influence of fatigue in repeated overground sprinting on force-velocity properties in children and adolescents, while this ability to repeat sprints is important for future progress in rugby union. Sprint time decline is commonly used to assess fatigability. However, it does not provide data on biomechanical aspects of sprint performance such as maximal power, force, and velocity production.

Curing Alzheimer's disease remains hampered by an incomplete understanding of its pathophysiology and progression. Exploring dysfunction in medial temporal lobe networks, particularly the anterior-temporal (AT) and posterior-medial (PM) systems, may provide key insights, as these networks exhibit functional connectivity alterations along the entire Alzheimer's continuum, potentially influencing disease propagation. However, the specific changes in each network and their clinical relevance across stages are not yet fully understood.

Public knowledge, attitudes, and perceptions toward genetically modified foods in Lebanon.

Genetically modified foods (GMFs) have garnered significant attention due to their implications for health, agriculture, and food security. Understanding knowledge and perceptions of GMFs is essential, as these factors influence acceptance and attitudes. This study, the first of its kind in Lebanon, aimed to assess the knowledge, attitudes, and perceptions regarding GMFs and to explore their association with socio-demographic characteristics.

Impact of the Implementation of a Short Stay Observation Unit Associated With Admitter-Rounder Model Onto Other Internal Medicine Units in a French University Hospital.

Introduction: Few data on the impact of specific interventions against Emergency Rooms 'or Hospitals overcrowding are available in France.

Methods: In the present report, we retrospectively investigated the impact of the implementation of a short-stay observation unit associated with the admitter-rounder model, especially onto the other in-patient internal medicine units in a French University Hospital.

Results: During the first 100 days, 242 patients were admitted into the short-stay observation unit.

Importance: Surgeon stress can influence technical and nontechnical skills, but the consequences for patient outcomes remain unknown.

Objective: To investigate whether surgeon physiological stress, as assessed by sympathovagal balance, is associated with postoperative complications.

Design, Setting, And Participants: This multicenter prospective cohort study included 14 surgical departments involving 7 specialties within 4 university hospitals in Lyon, France.

Clinical and radiological characteristics of calcified cerebral embolism: a large case series including 242 calcified cerebral embolism.

Introduction: Radiological calcified cerebral embolism (CCE) characteristics have been reported in small case series. Our aim was to describe clinical and radiological CCE characteristics in a large number of CCE and to compare characteristics between different patient groups.

Methods: Characteristics of 79 stroke patients with CCE were analyzed retrospectively.

Time course of hospitalizations in patients with heart failure and chronic obstructive pulmonary disease around sleep-disordered-breathing diagnosis.

Purpose: In heart failure (HF) and chronic obstructive pulmonary disease (COPD) populations, sleep-disordered breathing (SDB) is associated with impaired health outcomes. We evaluated whether in patients with HF, concomitant HF and COPD or COPD, the number of hospitalizations would be reduced in the year after testing for SDB with and without treatment initiation compared to the year before.

Methods: We performed a multicentre retrospective study of 390 consecutive sleep-clinic patients who had a primary diagnosis of chronic HF, HF and COPD or COPD and a secondary diagnosis of SDB.

Background: Longitudinal qualitative data on what matters to people with Parkinson's disease are lacking and needed to guide patient-centered clinical care and development of outcome measures.

Objective: To evaluate change over time in symptoms, impacts, and relevance of digital measures to monitor disease progression in early Parkinson's.

Methods: In-depth, online symptom mapping interviews were conducted with 33 people with early Parkinson's at baseline and 1 year later to evaluate (A) symptoms, (B) impacts, and (C) relevance of digital measures to monitor personally relevant symptoms.

Impulse control disorders in Parkinson's disease: What's new?

Impulse Control Disorders (ICDs) are increasingly recognized as a significant non-motor complication in Parkinson's disease (PD), impacting patients and their caregivers. ICDs in PD are primarily associated with dopaminergic treatments, particularly dopamine agonists, though not all patients develop these disorders, indicating a role for genetic and other clinical factors. Studies over the past few years suggest that the mesocorticolimbic reward system, a core neural substrate for impulsivity, is a key contributor to ICDs in PD.
