Fetal head position significantly influences birth outcomes, with higher rates of complications observed when the fetal head is in the Occiput Posterior (OP) position compared to Occiput Transverse (OT) or Occiput Anterior (OA) positions. There is no consensus in the current literature on the precise rotational point at which the fetal occiput shifts from posterior to transverse, reducing clarity in both scientific and clinical communication. Different studies employ varying definitions of these positions, which affects management decisions.

Department of Obstetrics and Gynecology, Faculty of Medicine, The University of Tokyo, Tokyo, Japan (Yano, Sayama, Iriyama, Ariyoshi, Akiba, Ichinose, Toshimitsu, Seyama, Kumasawa, Nagamatsu, and Osuga).

Article Synopsis
  • - The study aimed to create a new method to predict spontaneous vaginal delivery by analyzing the difference in angle of progression (delta angle) during the prolonged second stage of labor in first-time mothers with epidural anesthesia.
  • - Researchers reviewed the deliveries of 129 eligible women and found that larger delta angles during both the prolonged and normal stages correlated with successful spontaneous vaginal deliveries compared to those needing operative interventions like forceps or cesarean sections.
  • - The findings indicate that measuring the delta angle of progression could provide a valuable tool for healthcare providers to identify when a vaginal delivery is likely, potentially reducing unnecessary interventions and associated complications.

Objective: Obesity and maternal age are increasing among pregnant patients. The understood effect of body mass index (BMI), advanced maternal age (AMA), and second stage of labor on obstetric anal sphincter injury (OASIS) at delivery is varied. The objective of this study was to assess whether incorporating BMI, second stage of labor length, and AMA into a model for predicting OASIS among forceps-assisted vaginal deliveries (FAVD) had a higher predictivity value compared to models without these additions.

Article Synopsis
  • There has been a notable decline in the use of vacuum and forceps in obstetric deliveries, primarily due to complications for both mothers and infants, as well as challenges with operators' skills.
  • A study of medical records from teaching hospitals in West Java revealed that the percentage of vaginal operative deliveries (using vacuums and forceps) is persistently low, with vacuum deliveries dropping from 0.75% in 2018 to 0.68% in 2022, and forceps deliveries from 2.77% to 0.98%.
  • The findings suggest that training opportunities for obstetrics and gynecology residents are insufficient to reach the minimum required cases using these tools, indicating a need for further research
