Objectives: As previous studies on the use of a cervical pessary to prevent preterm birth (PTB) have produced conflicting results, we aimed to investigate the feasibility, acceptability and safety of a new technique for inserting a cervical pessary and compare it with the traditional technique in patients at high risk of PTB.

Methods: Women at high risk of PTB treated with a cervical pessary between January 2018 and January 2021 were retrospectively evaluated. After applying exclusion criteria, a total of 68 eligible patients were identified and retrospectively analyzed.

Use of the WHO partograph and Zhang's guideline for labor and delivery in China: implications for clinical practice.

BMC Pregnancy Childbirth

November 2024

Department of Obstetrics, Beijing Obstetrics and Gynecology Hospital, Capital Medical University, Beijing Maternal and Child Health Care Hospital, No. 251 Yaojiayuan Road, Beijing, 100026, China.

Article Synopsis
  • There is a lack of a comprehensive dataset for national birthing trends in China, prompting a study to evaluate the WHO partograph versus Zhang's guideline for labor and delivery.
  • The analysis involved low-risk first-time mothers from 2010 to 2022, comparing outcomes under both management protocols, which highlighted significant differences in rates of interventions and complications.
  • Results showed that while Zhang's guideline had lower rates of certain interventions and better outcomes regarding birth asphyxia, it was associated with higher rates of assisted reproduction and hypertension compared to the WHO partograph.

Article Synopsis
  • Forceps delivery is increasingly overlooked in favor of cesarean sections, despite cesarean complications, with concerns primarily focused on safety and long-term effects on children.
  • A study involving 49 women in Serbia assessed the neurological outcomes of children born via forceps at age 7, showing a 16.3% incidence of pathological neurological status among newborns.
  • Long-term issues were found in 6.4% of cases, highlighting that while forceps delivery is generally safe for neurological development, there are still mild disorders that can arise.

Article Synopsis
  • - Postpartum pubic symphysis diastasis is a rare condition often linked to complications during childbirth, such as difficult labor or larger babies.
  • - While most cases are managed conservatively, surgery may be necessary for severe diastasis (over 4 cm) to prevent long-term issues like chronic pain and functional disabilities.
  • - A study involving four women (ages 20-30) with over 7 cm of diastasis demonstrated successful outcomes with surgical intervention, allowing for early ambulation and full recovery within three months.

Article Synopsis
  • The RCOG promotes elective caesarean sections for patients with obstetric anal sphincter injury (OASI) and related dysfunction, but asymptomatic patients often lack access to necessary diagnostic tests in the UK.
  • A study of 69 women with 3a tears showed that 42% had defects seen on endoanal ultrasound (EA USS), with no significant difference in defect rates between symptomatic and asymptomatic individuals.
  • Results indicated that those delivered by forceps and of Asian ethnicity were more likely to be symptomatic, highlighting the need for universal EA USS for all OASI patients.

Article Synopsis
  • - The study examined the impact of resolving placenta previa in pregnant women during the second trimester, comparing outcomes for 403 women with resolved placenta previa to 403 women with normal placental positioning.
  • - Key findings revealed that women in the study group had higher rates of postpartum hemorrhage and related interventions than those in the control group, with statistically significant differences in rates and volumes of blood loss.
  • - Despite these differences in postpartum outcomes, neonatal metrics such as birth weight and incidences of complications showed no significant disparities between the two groups.

Exploring the Needle Tip Interaction Force with Retinal Tissue Deformation in Vitreoretinal Surgery.

IEEE Int Conf Robot Autom

May 2024

Aided Medical Procedures & Augmented Reality, Technical University of Munich, 85748 Munich, Germany; Whiting School of Engineering, Johns Hopkins University, Baltimore, MD, USA.

Article Synopsis
  • Recent advancements in treatments for age-related macular degeneration require precise, minimally invasive delivery methods to avoid trauma to vital eye structures.
  • Skilled surgeons face challenges due to hand tremors, prompting the development of robotic systems that utilize fiber Bragg grating (FBG) sensors to monitor forces during surgery.
  • This paper introduces a specialized dataset combining force feedback data with optical coherence tomography (OCT) images and presents a neural network model for estimating forces based on these images, highlighting the dataset's potential use.

PLoS One

October 2024

Department of Obstetrics and Gynecology, Faculty of Health Sciences, McMaster University, Hamilton, ON, Canada.

Article Synopsis
  • The study investigates racial disparities in maternal blood transfusions in the US from 2016-2021, analyzing data from nearly 18 million live births.
  • It finds that certain racial groups, particularly American Indian and Alaska Native, Black, Filipino, Korean, and Pacific Islander women, have higher odds of receiving blood transfusions compared to White women.
  • Additionally, the rates of transfusion and disparities vary significantly depending on the mode of delivery, with notable differences particularly in cesarean deliveries.

Article Synopsis
  • The study aims to compare the rates of vaginal uncomplicated deliveries (VUD) with cesarean deliveries (CD) to assess performance in obstetric care.
  • An analysis was conducted on 16,620 deliveries by 210 obstetricians, revealing a vaginal delivery rate of 39.6% and a higher adverse delivery (AD) rate of 9.9%.
  • The findings suggest that focusing on VUD rates rather than CD rates could better reflect the quality of maternal and infant outcomes in childbirth.

Article Synopsis
  • The study examines how the time of day affects the likelihood of operative vaginal births (OVBs) compared to spontaneous vaginal births (SVBs) using U.S. birth data from 2016 to 2021.
  • After analyzing over 15 million births, it was found that OVBs were more common during the late afternoon (4:30-7:30 p.m.) and less common in the early morning (3:00-6:00 a.m.).
  • The results suggest that different times of day influence the use of birth intervention methods, indicating a need for further research into the reasons behind these temporal trends.

Department of Obstetrics and Gynaecology, Monash University, Faculty of Medicine, Nursing and Health Sciences, Monash Medical Centre, 246 Clayton Rd, Clayton, Victoria 3168 Australia (Seo, Davies-Tuck, Warty, Smith, and Palmer); Monash Women's, Monash Health, 246 Clayton Rd, Clayton, Victoria Australia (Palmer).

Article Synopsis
  • The study examines the effectiveness, safety, and acceptability of two methods for cervical preparation prior to labor induction: digital insertion vs. speculum-based insertion of catheter balloons.
  • It utilized various medical databases to find randomized controlled trials that met specific criteria, focusing on viable singleton pregnancies and excluding studies that didn't involve cervical balloons.
  • The results indicated that while digital insertion was generally less painful and had comparable maternal satisfaction, overall outcomes showed no significant difference between the two methods, despite some concerns over study biases.
