Posture-induced compressive peroneal neuropathy during harvesting season: A case series.

Turk J Phys Med Rehabil

September 2024

Department of Physical Medicine and Rehabilitation, Çukurova University Faculty of Medicine, Adana, Türkiye.

Posture-induced compressive peroneal neuropathy usually occurs after maintaining certain positions, such as prolonged squatting or habitual leg crossing. Peroneal neuropathy mainly presents with unilateral foot drop and variable sensory deficit. In this article, a case series of unilateral/bilateral peroneal nerve palsy secondary to prolonged squatting during peanut harvesting was reported.

Linezolid induced acute toxic leukoencephalopathy and severe thrombocytopenia presenting as stroke mimic-a case report.

Oxf Med Case Reports

December 2024

Haematology, Colchester Hospital, East Suffolk and North Essex NHS Foundation Trust, Turner Road, Colchester, United Kingdom.

Article Synopsis
  • A 67-year-old woman was admitted for right-sided weakness, shaking, lethargy, and loss of appetite after completing treatment for a diabetic foot infection.
  • MRI results revealed changes in the deep white matter of her brain, potentially indicating an ischemic stroke, alongside symptoms of verbal dysfluency and visual disturbances.
  • After a decline in her condition and a drop in blood counts, she was diagnosed with Linezolid-induced Acute Toxic Leukoencephalopathy and thrombocytopenia based on her clinical presentation and imaging.

A syrinx involves cystic dilation of the central canal of the spinal cord due to the accumulation of cerebrospinal fluid and often results in a neurological deficit. While treatment options vary, surgical management is often utilized and requires an open durotomy. A 70-year-old female presented with one year of progressive low back pain with associated leg numbness, urinary incontinence, bilateral foot drop, and imbalance resulting in multiple falls.

Foot position during landing directly influences knee mechanics. Thus, the purpose of this study was to determine success in practicing, repeating, and transferring a desired foot progression angle (FPA) during landing, as well as changes in knee mechanics. Twenty females were randomly assigned to a control or verbal instruction group.

Background: According to previous literature reports, PTPN11 gene variants account for approximately 50% of Noonan syndrome (NS) cases and 85% of Leopard syndrome (LS) cases. Several patients who were diagnosed with NS or LS complicated with Chiari I malformation (CIM) and/or syringomyelia have been reported to have a PTPN11 variant. However, it is not always clear whether the association between CIM and/or syringomyelia and PTPN11 variants is real or random.

Background: Percutaneous intradiscal hydrogel injection has been used to treat low-back pain (LBP) due to degenerative disc disease with or without mild radicular pain. Complications from these procedures are underreported. In this case lesson, the authors present a rare case of a patient with herniated intradiscal hydrogel following a minor trauma leading to neurological injury.

Presentation and management outcome of foot drop with tibialis posterior tendon transfer.

World J Orthop

November 2024

Department of Plastic Surgery, Consultant Plastic Surgeon and Head of Plastic Surgery Department, National Institute of Rehabilitation Medicine, Islamabad 44000, Pakistan.

Article Synopsis
  • Foot drop leads to significant mobility issues, including weak ankle movement and an awkward walking style, which increases energy use and risk of injury.
  • This study aimed to assess foot drop patients and the effectiveness of tibialis posterior tendon transfer to improve ankle dorsiflexion over 7 years at a rehabilitation institute in Pakistan.
  • Of the 37 patients studied, most were young males with road traffic accidents as the leading cause; post-surgery outcomes showed varying degrees of improvement, with complications occurring in a small number of cases.

Background: Recent research has found that strengthening hip joint stability can considerably affect foot mechanics. The purpose of this study was to determine the effect of short foot exercises (SFEs), combined exercises (CEs), and SFEs with isometric hip abduction (IHA) on navicular drop (ND), static parameters (SP), and postural sway in women with flat foot (FF).

Methods: This study recruited 45 women with flexible FF.

Article Synopsis
  • Congenital foot drop linked to congenital talipes equinovarus (clubfoot) is uncommon, and its symptoms can often be mistaken for those of clubfoot itself.
  • A case study is presented involving a child with peroneal nerve injury who experienced clubfoot symptoms later in life.
  • The child received tibialis posterior transfer surgery to enhance movement and overall function.

Background: Compressive neuropathy of the common fibular nerve (CFN) is increasingly recognized as an etiology for foot drop and falls. Electrodiagnostic (EDX) studies are widely used to evaluate this condition, but such tests are invasive and costly. As with carpal and cubital tunnel syndromes, there may be patients with characteristic symptoms of CFN compressive neuropathy but normal EDX studies in which ultrasound may aid in decision-making.

Lateral knee cysts are relatively common synovial swelling. They are commonly benign swelling due to synovial fluid herniation from the joint. They are usually asymptomatic and in most cases are so small that only magnetic resonance imaging (MRI) can confirm the diagnosis.

Article Synopsis
  • The author shares a 40-year journey as a clinical neurologist specializing in treating amyotrophic lateral sclerosis (ALS), highlighting five patient cases from their early career at Chiba University Hospital.
  • A notable case involves a patient who battled ALS for 31 years, ultimately facing total locked-in syndrome, illustrating the long-term challenges of the disease.
  • The author also examines the profiles and treatment responses of 24 patients, discussing diagnostic delays and the complexities of caregiving and medical management for ALS patients.

Objective: To investigate the clinical effect of anterolateral femoral free flap combined with Masquelet technique in the treatment of tibial osteomyelitis with wound surface.

Methods: A retrospective study was conducted to analyze the clinical data of 25 patients with tibial osteomyelitis combined with defected wounds in Xijing Hospital of Fourth Military Medical University from Jun 2017 to Oct 2021. After one-stage treatment with free flap combined with Masquelet technique, the infection index, lower extremity function score (LEFS), and Mazur ankle function score were compared before bone defect repair and at the last follow-up.

A newly identified subtype of hereditary axonal motor neuropathy, characterized by early proximal limb involvement, has been discovered in a cohort of 34 individuals with biallelic variants in von Willebrand factor A domain-containing 1 (). This study further delineates the disease characteristics in a cohort of 20 individuals diagnosed through genome or exome sequencing, incorporating neurophysiological, laboratory and imaging data, along with data from previously reported cases across three different studies. Newly reported clinical features include hypermobility/hyperlaxity, axial weakness, dysmorphic signs, asymmetric presentation, dystonic features and, notably, upper motor neuron signs.

A man in his 30s with a history of refractory classical Hodgkin's lymphoma presented with subacute onset asymmetric sensory disturbance, and severe weakness affecting his right more than left hand. Symptom onset was 18 weeks after commencing brentuximab vedotin (BV) treatment, an anti-CD30 antibody drug-conjugate for relapsed/refractory classical Hodgkin's lymphoma. Symptoms progressed to his lower limbs with distal sensory loss, gait imbalance, bilateral foot drop and mild proximal leg weakness.

Conventional low-pass filtering of 3D motion capture signals prior to estimating knee joint moments and ACL injury risk has known limitations. This study aimed to evaluate the fractional Fourier filter (FrFF), which employs a time-varying cut-off frequency, for assessing peak knee moments during common ACL injury risk screening tasks. Ground reaction force and motion data were collected from 23 team sport athletes performing 45° unanticipated sidesteps and drop jumps.
