folliculitis (MF) is a fungal infection that often presents with pruritic follicular papules and pustules, primarily affecting the upper body. Due to its clinical similarity to bacterial folliculitis, misdiagnosis and delayed treatment are common. In this report, we present the case of a 16-year-old male who developed persistent pruritic papules on his upper back and chest, initially misdiagnosed as bacterial folliculitis.

  • - Majocchi's granuloma is a rare fungal skin condition caused mainly by the fungus Trichophyton rubrum and is often linked to the use of topical steroids and weakened immune systems.
  • - Symptoms typically include a rash with nodules, pustules, and scaling around hair follicles, which can be diagnosed through clinical exams and lab tests.
  • - Treatment involves using systemic terbinafine along with topical antifungal medications for effective management.

The hair follicle stem cell niche is an immune-privileged microenvironment, characterized by reduced antigen presentation, thus shielding against permanent immune-mediated tissue damage. In this study, we demonstrated the protective role of hair follicle-specific epidermal growth factor receptor (EGFR) against scarring hair follicle destruction. Mechanistically, disruption of EGFR signaling generated a cell-intrinsic hypersensitivity within the JAK-STAT1 pathway, which, synergistically with interferon gamma expressing CD8 T-cell and NK-cell-mediated inflammation, compromised the stem cell niche.

Hair follicle stem cells and the collapse of self-tolerance in alopecia: the interplay of barrier function, the microbiome, and immunity.

The hair follicle is thought to be a site of ‘immune privilege,’ or relative protection from autoimmune and inflammatory responses. In particular, the long-lived, quiescent hair follicle stem cells (HFSCs) are strikingly resistant to cytotoxic immune effectors such as T cells and natural killer (NK) cells, even in systems where these cells are artificially directed to attack HFSCs. Nonetheless, many forms of alopecia are associated with immune-mediated damage of the hair follicle epithelium, suggesting a failure of immune privilege.

Background: Binimetinib and selumetinib are two mitogen-activated protein kinase kinase (MEK) inhibitors used to treat low-grade gliomas and plexiform neurofibromas. Cutaneous toxicities are commonly associated with MEK inhibitors; however, limited studies have examined cutaneous effects in a pediatric population or whether toxicities vary between MEK inhibitors.

Methods: We conducted an IRB-approved, single-center, retrospective review of pediatric neuro-oncology patients on binimetinib or selumetinib who presented to NYU from April 2016 through July 2022.

Background: Folliculotropic mycosis fungoides (FMF) is a rare cutaneous malignancy that can be mistaken for inflammatory diseases, such as discoid lupus erythematosus (DLE), due to the variability of histopathological findings.

Methods: This study aims to provide dermatopathologists with evidence-based histopathologic criteria to distinguish DLE from FMF by reporting overlapping and distinguishing microscopic features. Forty-three biopsies from patients with a confirmed diagnosis of DLE or FMF were graded for the presence or absence of 18 histopathologic features.

Article Synopsis
  • * Patients using anti-TNF-α treatments, like adalimumab, often experience paradoxical skin reactions (PSRs), such as rashes and hair loss, necessitating a switch to alternative treatments.
  • * Guselkumab, an anti-IL-23 treatment, not only avoids PSRs but also effectively manages PPP/PAO, and it is suggested as a preferable option over adalimumab for patients dealing with these conditions.

Background: Conventional epilation methods commonly encounter challenges in achieving permanent removal of unwanted facial terminal hair (uFTH), particularly when it is interspersed among normal hairs.

Objective: To assess the safety and effectiveness of follicular unit excision (FUE) for permanent removal of uFTH.

Methods: The retrospective analysis included 231 patients who underwent FUE hair removal at three medical centers.

  • - Polytrichia is a common observation in folliculitis decalvans (FD), linked to compound follicular structures (CFS) that involve multiple hair follicles sharing a single opening and surrounded by fibrous tissue.
  • - A study analyzed 10 scalp biopsy samples from patients with early-stage FD, focusing on the types and quantities of follicles within these CFS.
  • - The findings revealed that of 205 follicles examined, 156 were terminal follicles, primarily in the anagen phase, providing insights into the pathology of FD.

  • Folliculitis decalvans (FD) is a chronic hair loss condition with an unclear cause, but recent research indicates a link to IL-17 secretion by mast cells.
  • A study found higher levels of IL-17 and mast cells in affected areas of the scalp compared to unaffected areas, suggesting an inflammatory response.
  • The results indicate that targeting IL-17 and mast cells may help manage the chronic inflammation associated with severe cases of FD.

  • Dermatitis cruris pustulosa et atrophicans (DCPA) is a chronic skin condition that can lead to hair loss if not treated and is characterized by certain dermoscopic features that distinguish it from similar conditions.
  • A study conducted on 30 patients in South India identified key dermoscopic findings such as follicular pustules, white structureless areas around follicles, and various vascular patterns, confirming the prevalence of DCPA in males and primarily affecting the lower limbs.
  • Limitations of the study included its retrospective nature, small sample size, and absence of a comparison group, highlighting the need for further research to better understand DCPA and its features.
