Article Synopsis
  • Fitz-Hugh-Curtis Syndrome (FHCS) is an inflammation linked to pelvic inflammatory disease caused by sexually transmitted infections, leading to symptoms like abdominal pain and possible liver capsule issues.
  • A case study of a 22-year-old woman revealed her abdominal pain was misdiagnosed as advanced ovarian cancer, but mNGS testing revealed an infection with Chlamydia trachomatis in her ascitic fluid.
  • The use of mNGS proved to be an effective method for diagnosing this rare condition, showing promise as a non-invasive technique for identifying pathogens with high accuracy.

Article Synopsis
  • - This case report details a rare advanced abdominal pregnancy in a 36-year-old woman, complicated by chronic pelvic inflammatory disease and Fitz-Hugh-Curtis syndrome, presenting with severe symptoms including abdominal pain and fever.
  • - The diagnosis was difficult due to the pregnancy's advanced stage and its location in the Douglas pouch, identified via ultrasound, classifying it as an intraperitoneal pregnancy.
  • - Successful management through laparotomy showcases the need for skilled clinical evaluation and suggests that safe removal of the placenta is possible, highlighting the importance of recognizing rare ectopic pregnancies for better patient outcomes.

Fitz-Hugh-Curtis syndrome with disseminated intra-abdominal gonorrhoea.

Department of Gastroenterology and Hepatology, Monash Health, Clayton, Victoria 3168, Australia.

Neisseria gonorrhoea continues to be implicated in a large proportion of sexually transmitted infections worldwide. Prompt recognition of infection is required to prevent further complications which include pelvic inflammatory disease and less commonly, perihepatitis which is known eponymously as Fitz-Hugh-Curtis syndrome. Third generation cephalosporins such as ceftriaxone remain effective in the treatment of gonococcal infection, however failure in initiation of appropriate antibiotic therapy in a timely manner can result in further disseminated disease.

Article Synopsis
  • A 38-year-old man experienced severe right upper quadrant abdominal pain for seven years, undergoing multiple tests that found no issues with his bile or upper digestive system.
  • In March 2023, he was set to have robotic surgery for suspected biliary problems, but during surgery, doctors discovered perihepatic adhesions linked to Fitz-Hugh-Curtis syndrome.
  • After removing the adhesions and performing a liver biopsy, the patient felt completely better two weeks post-surgery, emphasizing the need to consider FHCS in persistent abdominal pain cases with negative tests to avoid lengthy diagnostics.

A 25-year-old man presented with a fever and right upper quadrant abdominal pain. Computed tomography (CT) of the abdomen revealed diffuse perihepatic capsular enhancement, suggesting perihepatitis. Although the patient was a man, Fitz-Hugh-Curtis syndrome was suspected based on the CT findings.

Perihepatitis (Fitz-Hugh-Curtis syndrome) is a rare complication of sexually transmitted infections, mostly seen in women. Only 12 male cases have been reported to date, of which was confirmed in 2. We report a case of chlamydial perihepatitis in a male patient, occurring 1 month after Mpox and associated with the unusual LGV ST23 strain.

Article Synopsis
  • Perihepatitis, particularly Fitz-Hugh-Curtis syndrome, is a rare complication of pelvic inflammatory disease that generally affects premenopausal women, leading to right upper quadrant pain from inflammation.
  • The study proposes a new physical examination sign, called the "liver capsule irritation sign," which involves increased tenderness in the right upper abdomen when patients are positioned on their left side, aiding early diagnosis.
  • The findings suggest that this sign could help identify perihepatitis not only related to Fitz-Hugh-Curtis syndrome but possibly from other causes as well.

We describe a case of multiple missed opportunities to diagnose Fitz-Hugh-Curtis syndrome in a sexually active 26-year-old woman in the emergency department (ED). Repeat ultrasound scans showed a hemorrhagic ovarian cyst. Multiple ED providers relied exclusively on these ultrasound findings as the presumed cause of her pelvic pain, to the detriment of their physical examination.
