Case: A 44-year-old male was admitted complaining of recurrent swelling and pain in the left parotid gland with recent history of penetrating parotid trauma and foreign body removal.

Foreign bodies in the upper aerodigestive tract represent one of the most common ENT emergencies, but patients with foreign bodies in the floor of the mouth rarely present to the otolaryngologist. Only a very small number of these foreign bodies are embedded under normal mucosa and can result in lethal complications. Anatomic relationships of the structures exposed in type B and C infratemporal fossa approaches were studied in 20 temporal bones. The intrapetrous carotid artery (ICA), cochlea (CH), eustachian tube (ET), foramen spinosum (FS), foramen ovale (FO) and anterior foramen lacerum (AFL) were exposed by drilling of the glenoid fossa and base of middle cranial fossa. Previous studies have described a primary afferent vestibular projection to second-order cochlear neurons originating from the saccular maculae. As could be shown in the guinea pig by means of anterograde transport of horseradish peroxidase and immuno-cytochemistry, these saccular afferents terminated at cells immunoreactive to glutamate and aspartate but not to gamma-aminobutyric acid (GABA). These were intermingled among the fibers of the acoustic striae and situated between the dorsal cochlear nucleus and the octopus cell area.

The amplitude ratio between the paretic and the normal side as determined by electroneuronography is used as a basis for estimating the prognosis of facial palsy. Several methods have been described of placing the surface electrodes and of finding the supramaximal impulse strength.
