Moderate traumatic brain injury (TBI) is a diagnosis that describes diverse patients with heterogeneity of primary injuries. Defined by a Glasgow Coma Scale between 9 and 12, this category includes patients who may neurologically worsen and require increasing intensive care resources and/or emergency neurosurgery. Despite the unique characteristics of these patients, there have not been specific guidelines published before this effort to support decision-making in these patients.

In orthopaedic surgery, as well as other areas in medicine, it is common for a surgical technique to carry the original authors' name describing the procedure. The Judet family represents a unique history, since several orthopaedic procedures are known as "Judet's technique". The aim of this historic review is to outline the genealogy of the orthopaedic arm of the Judet family, while crediting each surgical procedure to the specific family member that described the technique.

Clinical Use of Trabecular Bone Score: The 2023 ISCD Official Positions.

J Clin Densitom

February 2024

Centre of Excellence in Skeletal Health Assessment, University of Toronto, Toronto, Ontario, Canada; Osteoporosis Program, University Health Network and Sinai Health System, Toronto, Ontario, Canada.

Article Synopsis
  • Osteoporosis is diagnosed using bone mineral density (BMD) measured by DXA, but other skeletal factors also impact bone strength and fracture risk.
  • Lumbar spine texture analysis (TBS) enhances fracture prediction beyond BMD and can be used to refine fracture risk assessments, particularly for patients aged 40 and older who are nearing treatment thresholds.
  • Recommendations include using TBS from L1-L4 vertebrae for adjusted FRAX calculations and noting that TBS changes may not be useful with current methods, although future updates to the algorithm may improve its effectiveness.

Background: Dysesthesia, electrical and burning sensations, in addition to allodynia are frequent symptoms of neuropathic pain. Despite the high frequency, scientific data on the development of neuropathic pain after surgery for fracture fixation are scarce. The goal of the present study was to determine the prevalence, risk factors, and evaluate potential associations among neuropathic pain, pain intensity, sociodemographic, and clinical variables after wrist, hip, and ankle fracture fixation.

Risk factors for major complications after surgical treatment of primary ileocecal Crohn's disease. A multicentric Latin American experience.

Cir Esp (Engl Ed)

December 2023

Colorectal Surgery Unit, Pontificia Universidade Católica do Paraná (PUCPR), Curitiba, Brazil.

Introduction: Complications after ileocecal resection for Crohn's disease (CD) are frequent. The aim of this study was to analyze risk factors for postoperative complications after these procedures.

Materials And Methods: We conducted a retrospective analysis of patients treated surgically for Crohn's disease limited to the ileocecal region during an 8-year period at 10 medical centers specialized in inflammatory bowel disease (IBD) in Latin America.

Fracture Prediction from Trabecular Bone Score is Unaffected by Anti-Resorptive Treatment: A Registry-Based Cohort Study.

J Clin Densitom

February 2023

School of Medicine, Centro Universitario de Belo Horizonte - UNI BH, Brazil; Endocrinology Clinic, Felicio Rocho Hospital, Belo Horizonte, Brazil; Endocrinology Unit, Santa Casa Hospital, Belo Horizonte, Brazil.

Article Synopsis
  • Trabecular bone score (TBS) is an effective predictor of osteoporotic fractures, independent of bone mineral density (BMD) and clinical risk factors.
  • The study analyzed two large cohorts in Manitoba, Canada, to determine how anti-resorptive treatment impacts fracture risk prediction from TBS, showing that treatment status did not change TBS's predictive capability.
  • Regardless of whether anti-resorptive medication was used before or after TBS measurement, TBS consistently provided strong fracture risk predictions, reinforcing its reliability in assessing fracture risk in various patient conditions.

Introduction: Robotic surgery has expanded on it's surgical application and it is also noted an increase in surgical procedures complexity. Occurrence of emergency situations that require conversion of the minimally invasive access route to open access routes is uncommon. The urgent undocking of the robotic platform is a complex and neglected process in robotic surgery training.

Background: Acute Generalised Exanthematous Pustulosis (AGEP) is a rash with multiple sterile intraepidermal or subcorneal non-follicular pustules on edematous papules, with a sudden development and rapid evolution, triggered by drugs, vaccination, insect bites, exposure to mercury, and allergens.

Objectives And Methods: We describe a female patient who developed extensive and abnormally prolonged AGEP following exposure to terbinafine and SARS-CoV vaccine. A detailed review of terbinafine-induced-AGEP cases was performed, with the aim of evaluating if the AGEP criteria would follow a different pattern when the disease is triggered by this drug.

Objective: Anterior cranial fossa dural arteriovenous fistulas (DAVFs) have been almost exclusively considered as surgical lesions. However, new advances in endovascular technology have made the endovascular treatment (EVT) of ethmoidal DAVFs feasible. The aim of this study was to report the clinical and angiographic outcomes of patients harboring DAVFs of the anterior cranial fossa who had undergone EVT as a first-line approach.

Comparing Intramedullary Nailing Versus Locked Plating in the Treatment of Native Distal Femur Fractures:

Indian J Orthop

June 2021

Division of Orthopaedic Trauma, Department of Orthopaedic Surgery, Jersey City Medical Center-RWJBarnabas Health, 377 Jersey Ave, Suite 280A, Jersey City, NJ 07302 USA.

Introduction: Distal femur fractures make up < 1% of all fractures and 3-6% of all femur fractures. In the literature, both intramedullary nailing (IMN) and locked plating (LP) have shown favorable results, but there is no consensus on a gold standard. The purpose of this systematic review is to compare outcomes of native distal femur fractures treated via IMN versus LP in an effort to determine if one is superior to the other.
