Purpose: To report a case of transient diplopia and upgaze paresis in the setting of acute dorsal midbrain infarcts from a cervical vertebral artery dissection in an otherwise healthy man.
Observations: A 33-year old man presented to the ophthalmology urgent clinic with a 1 h history of blurred and double vision, asthenopia, and a mild focal left posterior headache. Ocular motility examination revealed a profound upgaze palsy and convergence-retraction horizontal jerk nystagmus in attempted upgaze that gradually improved over the course of 1 h.

Background: Unhealthy alcohol use is a leading cause of preventable mortality and a risk factor for an array of social and health problems. The Intervention in Small primary care Practices to Implement Reduction in unhealthy alcohol use (INSPIRE) study is part of a nationwide campaign to improve the identification and treatment of patients engaging in unhealthy alcohol use.
Methods: We conducted a single arm, pragmatic study consisting of seventeen primary care practices in the Chicago metropolitan area, Wisconsin, and California across two waves with a 6-month latent period, a 12-month intervention period, followed by a 6-month sustainability period.

People living with HIV are at higher risk of heart failure and associated left atrial remodeling compared to people without HIV. Mechanisms are unclear but have been linked to inflammation and premature aging. Here we obtain plasma proteomics concurrently with cardiac magnetic resonance imaging in two independent study populations to identify parallels between HIV-related and aging-related immune dysfunction that could contribute to atrial remodeling and clinical heart failure.

Introduction: Prasinezumab was shown to potentially delay motor progression in individuals with early-stage Parkinson's disease (PD) who were either treatment-naïve or on monoamine oxidase type B inhibitor (MAO-Bi) therapy in the PASADENA study. We report the rationale, design, and baseline patient characteristics of the PADOVA study, designed to evaluate prasinezumab in an early-stage PD population receiving standard-of-care (SOC) symptomatic medications.
Methods: PADOVA (NCT04777331) is a Phase 2b, multicenter, randomized, double-blind, placebo-controlled, parallel-group study, in which individuals with early-stage PD on SOC stable symptomatic monotherapy (levodopa or MAO-Bi) receive intravenous prasinezumab 1500 mg every 4 weeks.

Background: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a recurrent inflammatory disease associated with several comorbidities and a significant disease burden for patients. Treatments include corticosteroids and sinonasal surgery, but these can be associated with the risk of adverse events and nasal polyp recurrence. The association between hypopituitarism and metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized, although data about therapies targeting recurrence post-transplant is limited. An 8-year-old with hypopituitarism-associated MASLD underwent a liver transplant due to rapid progression of metabolic dysfunction-associated steatohepatitis (MASH). Hepatosteatosis recurred within weeks.
