Cardiology A Department, Research Laboratory LR12 SP 16 Fattouma Bourguiba University Hospital, University of Monastir, Monastir University, Rue du 1er juin 1955, 5000, Monastir, Tunisia.

Unlabelled: BACKGROUND AND OBJECTIVE: Left atrial strain (LAS) has prognostic value in patients with atrial fibrillation (AF). Consequently, therapies that improve LAS may help reduce AF-related adverse cardiac events. We aimed to compare how digoxin and bisoprolol modulate LAS in patients with AF being treated with rate control.

The Horoscope trial aimed to assess the efficacy of home blood pressure (BP) telemonitoring (TLM) in controlling BP reduction in hypertensive patients compared with usual care. This is a multi-center, prospective randomized, parallel-group trial comparing TLM with usual care during a period of 6 months in patients with hypertension. We included 525 patients randomly assigned in a 1-1 ratio to telemonitoring (TLM group; n = 260) or usual care (control group; n = 265).

Radiol Case Rep

February 2025

Orthopedic and Trauma Department, Fattouma Bourguiba University Hospital, Monastir, Tunisia.

Adipose tumors are dominated by lipomas, but other rarer entities may be encountered such as hibernoma which is a benign tumor that develops from brown fat. The thigh is the preferred location for hibernomas, but the tumor can appear in other areas. The upper limb is a rare localization and there is only 1 case of hibernoma on the hand already reported in the English literature.

Background And Aims: Neurofibromatosis-Noonan syndrome (NFNS) is an extremely rare genetic entity combining the clinical phenotype of two conditions: neurofibromatosis type 1 syndrome (NF1) and Noonan syndrome (NS). Nevertheless, many inconsistencies reside in our understanding of this condition, mainly its clinical features and genetic background. Through this systematic review, we aim to shed light on the epidemiological features, the broad clinical spectrum, the underlying genetic defects and the associated comorbidities of NFNS.

Article Synopsis
  • Hepatic disorders often progress silently and are usually detected only when liver function is significantly impaired, making early diagnosis challenging.
  • Oral lichen planus (OLP) is frequently associated with Hepatitis C and can serve as an early warning sign for the disease, particularly in patients with chronic HCV infection.
  • This case report highlights the importance of dentists in diagnosing and managing OLP, emphasizing their role in treatment and the potential for early hepatitis C screening, as demonstrated by a female patient who achieved full remission through topical corticosteroids and oral sanitation.

Parenting a Child With Disorder of Sexual Development in Traditional Society: Psychological Impact.

J Pediatr Surg

February 2025

Department of Pediatric Surgery, Fattouma Bourguiba University Hospital, Monastir, Tunisia; Research Laboratory LR12SP13, Faculty of Medicine of Monastir, Tunisia.

Background: Disorders of Sexual Development can cause significant psychological distress for parents, particularly in traditional societies where such conditions are stigmatized.

Aim: This study aims to investigate the psychoaffective impact of sexual development disorders caused by congenital adrenal hyperplasia in children with 46, XX karyotype, in order to determine the predictive factors for the onset of anxiety and depression among parents and the coping strategies employed by parents to adapt to their child's condition.

Materials And Methods: This is a retrospective, cross-sectional, descriptive, and analytical study involving parents of children treated for sexual development disorders related to congenital adrenal hyperplasia, with a 46, XX karyotype, who underwent feminizing genitoplasty during the period from January 1, 2005 and December 31, 2021.

Article Synopsis
  • The study aimed to analyze microvascular changes around the optic nerve in patients with optic neuropathies using optical coherence tomography angiography (OCTA).
  • During the research, 63 eyes from patients with various optic neuropathy causes were compared to 36 healthy eyes, revealing more vascular changes in acute non-arteritic ischemic optic neuropathy than in optic neuritis.
  • Results indicated that a lower peripapillary vascular density (PVD) was linked to optic atrophy and hypertension, with some correlations between visual acuity and PVD, highlighting significant changes in microvasculature related to the condition and systemic health factors.

Triple-negative breast cancer (TNBC) is known to have the worst outcome compared to the other forms of breast cancer. Moreover, molecular markers identified basal-like breast cancer (BLBC) phenotypes to be also related to a worse prognosis. In this study, we evaluated by immunohistochemistry (IHC) the prognostic significance of combining Cytokeratin-19 (CK19), E-cadherin, and Ki-67 tissue expression in triple-negative breast cancer (TNBC) cases presenting a basal-like (BLBC) or a non-basal-like (n-BLBC) phenotype to improve the selection and the monitoring of BC patients with a more aggressive outcome.

  • Homozygous familial hypercholesterolaemia (HoFH) is a rare genetic disorder that causes extremely high LDL cholesterol levels, leading to heart disease at an early age; lomitapide is a medication designed to lower these cholesterol levels in affected adults and is being tested for safety and efficacy in children.
  • The APH-19 study involved 43 pediatric patients aged 5-17 years on existing cholesterol treatments; they received varying doses of lomitapide over a 24-week period to measure its effect on LDL cholesterol levels and other lipid parameters.
  • Results indicated a significant decrease in LDL cholesterol by 53.5% after 24 weeks of treatment, suggesting lomitapide may be effective for managing cholesterol

  • A 73-year-old man with GCT in his knee had successful treatment via extensive curettage and bone grafting, leading to good long-term outcomes.
  • In older patients, differential diagnosis with other conditions is crucial, and while joint-replacing surgeries are common, joint-preserving options can also be effective if osteoarthritis is not present.

Cardiovascular clinical trials continue to under-represent children, older adults, females and people from ethnic minority groups relative to population disease distribution. Here we describe strategies to foster trial representativeness, with proposed actions at the levels of trial funding, design, conduct and dissemination. In particular, trial representativeness may be increased through broad recruitment strategies and site selection criteria that reflect the diversity of patients in the catchment area, as well as limiting unjustified exclusion criteria and using pragmatic designs that minimize research burden on patients (including embedded and decentralized trials).
