Article Synopsis
  • The study investigates the impact of single arterial cannulation (SAC) versus double arterial cannulation (DAC) during repairs for acute type A aortic dissection, focusing on patient outcomes such as hospital stay and complications.
  • A systematic review of 7 studies involving 3,534 patients revealed that DAC led to longer stays in intensive care but did not significantly affect overall length of hospital stay or rates of stroke, paraplegia, or acute kidney injury compared to SAC.
  • The findings suggest that while DAC could be associated with higher risks of complications like need for dialysis or reintubation, overall clinical outcomes remain comparable, underscoring the importance of individualized cannulation strategies for patients.*

Article Synopsis
  • Pulsed-field ablation (PFA) is a new technique for treating atrial fibrillation (AF) that targets heart tissue more selectively than traditional thermal methods like radiofrequency and cryoablation.
  • A systematic review of 17 studies with 2,255 patients found that PFA significantly lowers AF recurrence, procedure time, heart rate changes, and risks of certain complications like phrenic nerve palsy and esophageal lesions.
  • However, PFA is also linked to a higher risk of pericardial tamponade, indicating it has both benefits and potential risks compared to thermal ablation.

Background: People who are addicted to amphetamines have a much greater chance of developing psychosis compared to those who are not. It is essential to study the behavioral and psychological effects of amphetamines. Therefore, this research aimed to examine conditions such as depression, anxiety, mood, cognitive abilities at the workplace, and social responsibilities by using sociodemographic factors as useful tools in determining effective strategies for preventing, managing, and treating amphetamine addiction.

This study retrospectively investigated the impact of interleukin-1 receptor-associated kinase-3 (IRAK-3/IRAK-M) silencing by methylation on the likelihood of multiple sclerosis (MS) activity. This cross-sectional study included 90 patients with MS: 45 with active disease (Group 1), 45 in remission (Group 2), and 45 healthy controls. The study included quantitation of IRAK-3 methylation index (MI%), IRAK-3 mRNA, and myeloid differentiation factor88 (MyD88) and assessment of NF-κB activity.

Background And Aims: Hypertension (HTN) is a leading cause of morbidity and mortality affecting about 30%-40% of the adult population in developed countries. Fewer data were published about the prevalence, sociodemographics, and clinical characteristics of the resistant hypertensive population in Egypt. Hence, our aim is to focus the attention on these determinants especially in the delta region of Egypt.

Article Synopsis
  • The study investigates the causes of in-stent restenosis (ISR) in the right coronary artery (RCA), particularly focusing on mechanical and biological mechanisms.
  • Using intravascular ultrasound (IVUS), researchers classified ISR lesions and found that 50% were due to mechanical issues like stent fractures or deformations, while 53% were related to biological factors like neointimal hyperplasia.
  • The one-year target lesion failure rate was 11.5%, with significantly higher event rates (41.4%) for mechanically caused ISRs that were managed without new stents, highlighting the need for better treatment strategies.

Freestyle perforator flap in earlobe reconstruction.

BMC Surg

November 2022

Department of Plastic and Reconstructive Surgery, Faculty of Medicine - Tanta University, Tanta, Egypt.

Background: In earlobe reconstruction, it is a challenging task to find a simple one-stage technique that is applicable to the variable configurations of defects and that results in a longstanding naturally appearing earlobe.

Methods: In this article, we present a perforator-based flap design for earlobe reconstruction with preoperative perforator detection by pencil doppler to ensure the maximum vascularity of the used flap. Thirty cases of earlobe reconstruction were performed in the Department of Plastic and Reconstructive Surgery at Tanta University in the period from July 2015 to July 2019 using the technique described in this article.

Article Synopsis
  • The study aims to summarize the prevalence, diagnosis, and management of osteitis pubis (OP) from a urological perspective, while proposing a management algorithm based on current evidence.
  • Literature searches found that OP is a chronic, painful condition with unclear management strategies and multifactorial causes, highlighting the need for a diagnostic index and clinical experience for accurate diagnosis.
  • Although conservative management is preferred, surgical options may be necessary in some cases, and a multi-disciplinary approach is essential for effective treatment; prognosis is generally good, but some patients may experience debilitating long-term issues.

Background: Patent ductus arteriosus (PDA) stenting is an alternative to modified Blalock-Taussig shunt (MBTS) as first-stage palliation of duct-dependent lesions. The superiority of one approach over the other is still controversial. Our objective was to compare PDA  stent versusMBTS for palliation in regard to safety, efficacy, and efficiency.

Background And Aims: Ulcerative colitis (UC) and Crohn's disease (CD) are the most common types of Inflammatory bowel disease (IBD), with variable responses to traditional therapies and unpredicted prognosis. In Egypt and most developing countries, the lack of recent epidemiological and prognostic data adversely affects management strategies. We collected and analyzed data of patients with IBD from multiple centers across Egypt to evaluate patients' clinical and epidemiological characteristics.

Association between genetic variations of mitochondrial isocitrate dehydrogenase (IDH2) and acute myocardial infarction.

Gene

June 2022

Medical Biochemistry Department, Faculty of Medicine. Tanta University, Tanta, Egypt; Biochemistry Department, College of Medicine, Ha'il University, Ha'il, Saudi Arabia.

Purpose: Mitochondrial isocitrate dehydrogenase (IDH2) is a major contributor to cellular redox control. The aim of this study was to preliminary link IDH2 genetic variations to redox imbalance, atherogenesis and risk of acute myocardial infarction (AMI).

Methods: This case-control study included 120 AMI patients and 120 healthy controls.

Cyclosporine (CsA) is a widely used immunosuppressive agent that incurs marked nephrotoxicity in the clinical setting. Thus, there is a need for finding safe/effective agents that can attenuate CsA-induced kidney injury. Meanwhile, the underlying mechanisms for CsA-associated nephrotoxicity are inadequately investigated, in particular, the AKT/eNOS/NO pathway.

The pandemic of COVID-19 requires rapid and easy access to reliable imaging modalities for diagnosis and follow up. Considering the cost-effectiveness of the imaging used, ultrasound is a non-ionizing, portable and bedside imaging modality with a high diagnostic impact in emergencies and intensive care units in pandemics, but it is operator dependent. In our article, we provide a comprehensive review of the role of point-of-care ultrasound in the diagnosis of COVID-19 infection and its impact on the lungs, cardiovascular system, eyes and abdominal organs.

Background And Aim: Esophagogastroduodenoscopy (EGD) is the gold standard tool in both screening/diagnosis and management of varices in cirrhotic patients; however, its invasive nature may be uncomfortable to some patients, and in addition, it may be unavailable in some centers that cannot afford it. Therefore, to decrease the economic and physical burden on patients, multiple noninvasive clinical, laboratory, and radiological parameters are evaluated as triage screening predictors of varices before patients' referral to endoscopy. In this respect, we tried to evaluate the validity of portal vein velocity (PVV) as a noninvasive screening tool of esophageal varices (EV).
