Background: Initial clinical studies of pulsed field ablation (PFA) to treat atrial fibrillation (AF) indicated a >90% durability rate of pulmonary vein isolation (PVI). However, these studies were largely conducted in single centers and involved a limited number of operators. The electrophysiological findings and outcomes in patients undergoing repeat ablation after an initial PF ablation for AF are incompletely understood.

Introduction: The medial patellofemoral ligament (MPFL) is the main patellar stabilizer in low knee flexion degrees (0-30°). Isolated MPFL reconstruction (MPFLr) is therefore considered the gold standard of surgical procedures for low flexion patellofemoral instabilities (PFIs). Despite excellent clinical results, little is known about the effect of MPFLr on kinematic parameters (KPs) of the patellofemoral joint in vivo.

Cyclin-dependent kinase 4 drives cystic kidney disease in the absence of mTORC1 signaling activity.

Kidney Int

November 2024

III. Department of Medicine, University Hospital Hamburg Eppendorf, Hamburg, Germany; Hamburg Center for Kidney Health (HCKH), University Medical Center Hamburg-Eppendorf, Hamburg, Germany.

Progression of cystic kidney disease has been linked to activation of the mTORC1 signaling pathway. Yet the utility of mTORC1 inhibitors to treat patients with polycystic kidney disease remains controversial despite promising preclinical data. To define the cell intrinsic role of mTORC1 for cyst development, the mTORC1 subunit gene Raptor was selectively inactivated in kidney tubular cells lacking cilia due to simultaneous deletion of the kinesin family member gene Kif3A.

Introduction: The use of Complementary and Integrative Medicine (CIM) is very popular among the general population in Germany. However, international studies show that nurses, physicians, and other health care professionals (HCPs) at hospitals often do not feel sufficiently informed about different CIM approaches. Moreover, they do not feel trained enough to counsel their patients appropriately.

Impact of Left Atrial Posterior Wall Ablation During Pulsed-Field Ablation for Persistent Atrial Fibrillation.

JACC Clin Electrophysiol

May 2024

Icahn School of Medicine at Mount Sinai, New York, New York, USA; Cardiology Department, Na Homolce Hospital, Homolka Hospital, Prague, Czech Republic. Electronic address:

Article Synopsis
  • Pulmonary vein isolation (PVI) alone is often inadequate for treating persistent atrial fibrillation, and adding left atrial posterior wall (LAPW) ablation has had mixed results due to concerns about safety and effectiveness.
  • This study compares the safety and effectiveness of PVI combined with LAPW ablation versus PVI alone using pulsed-field ablation (PFA) in patients with persistent atrial fibrillation.
  • Results show no significant difference in freedom from arrhythmias or major adverse events between the two groups, indicating that adding LAPW ablation may not improve outcomes in this patient population.

Nonpreferential but Detrimental Accumulation of Macrophages With Clonal Hematopoiesis-Driver Mutations in Cardiovascular Tissues-Brief Report.

Arterioscler Thromb Vasc Biol

March 2024

Department of Cardiology and Angiology, University Heart Center Freiburg-Bad Krozingen and Faculty of Medicine, University of Freiburg, Germany (T.-S.D., A.Y., H.H., T.A.V., C.v.z.M., T.H., D. Wolf, D. Westermann, I.H.).

Article Synopsis
  • Clonal hematopoiesis of indeterminate potential (CHIP) is linked to increased risks for leukemia and cardiovascular diseases due to the presence of mutated myeloid cells; however, their specific behavior in cardiovascular tissues is not fully understood.
  • The study involved patients undergoing cardiovascular surgeries, where researchers analyzed blood and tissue samples to identify CHIP mutation carriers and assess the characteristics of myeloid and lymphoid cells using advanced genetic techniques.
  • Results showed that although CHIP-mutated myeloid cells did not accumulate more in cardiovascular tissues compared to non-mutated cells, they exhibited a more proinflammatory and disease-prone gene profile.

Background: Perioperative echocardiography is of paramount importance during cardiac surgery. Nonetheless, in the experimental large-animal setting, it might be challenging obtaining optimal imaging when using conventional imaging acquisition techniques, such as transthoracic and transesophageal screenings. Open-chest surgery allows epicardial echocardiographic assessment with direct contact between probe and heart, thus providing superior quality.

Introduction: Cognitive behavioral therapy and dialectical behavior therapy (DBT) can be effective in treating adults with ADHD, and patients generally consider these interventions useful. While adherence, as measured by attendance at sessions, is mostly sufficient, adherence to therapy skills has not been assessed. Furthermore, the relationship between patient evaluation of therapy effectiveness, treatment adherence, and clinical outcomes is understudied.

Importance: Previous studies evaluating the association of patient sex with clinical outcomes using conventional thermal ablative modalities for atrial fibrillation (AF) such as radiofrequency or cryoablation are controversial due to mixed results. Pulsed field ablation (PFA) is a novel AF ablation energy modality that has demonstrated preferential myocardial tissue ablation with a unique safety profile.

Objective: To compare sex differences in patients undergoing PFA for AF in the Multinational Survey on the Methods, Efficacy, and Safety on the Postapproval Clinical Use of Pulsed Field Ablation (MANIFEST-PF) registry.

Background: In Germany and Western Europe, gastroesophageal junction cancer (AEG) and proximal gastric cancer are currently treated with (transhiatal-extended) total gastrectomy (TG) according to the latest treatment guidelines. TG leads to a severe and long-lasting impairment of postoperative health-related quality of life (HRQoL) of the treated patients. Recent studies have suggested that HRQoL of these patients could be improved by proximal gastrectomy with double-tract reconstruction (PG-DTR) without compromising oncologic safety.

Background: Pulsed field ablation is a novel nonthermal cardiac ablation modality using ultra-rapid electrical pulses to cause cell death by a mechanism of irreversible electroporation. Unlike the traditional ablation energy sources, pulsed field ablation has demonstrated significant preferentiality to myocardial tissue ablation, and thus avoids certain thermally mediated complications. However, its safety and effectiveness remain unknown in usual clinical care.
