Article Synopsis
  • Hospital-acquired bloodstream infections (HA-BSI) in ICU patients can be life-threatening, and this study aimed to see how early adequate antibiotic treatment affects 28-day mortality rates for patients who survive at least one day after infection onset.
  • Using data from a multicenter study with 2,418 patients, researchers found that those who received adequate treatment within 24 hours had a lower 28-day mortality rate (32.8%) compared to those who were inadequately treated (40%).
  • The study concluded that inadequate antibiotic therapy within 24 hours contributes significantly to 28-day mortality, indicating that quicker treatment could greatly improve patient outcomes in cases of HA-BSI.

  • This study compares two surgical methods, Hartmann's procedure (HP) and resection with primary anastomosis (RPA), for treating acute left-sided colonic emergencies among 1215 patients from 204 centers globally.
  • Results showed that while HP was the more common treatment (57.3%), RPA was favored for younger patients with fewer health issues and those needing surgery sooner.
  • The study concluded that although HP is still widely used, RPA might be the better option, emphasizing the importance of patient characteristics and surgeon experience in determining treatment choice.

  • - Cardiovascular disease significantly impacts patients with type 2 diabetes, yet the effects of different glucose-lowering medications on heart health in patients with low cardiovascular risk remain uncertain.
  • - In a study involving over 5,000 participants with type 2 diabetes, various medications (insulin glargine, glimepiride, liraglutide, sitagliptin) were evaluated to determine their effects on cardiovascular events over a median follow-up of 5 years.
  • - The results showed no major differences in heart-related events among the medication groups, but those treated with liraglutide had a notably lower risk of more severe cardiovascular complications compared to the other drugs combined.

  • Rivaroxaban 2.5 mg twice daily combined with aspirin was more effective than aspirin alone for preventing serious cardiovascular events in patients with stable coronary artery disease (CAD) or peripheral artery disease (PAD).
  • A cost-analysis over a 23-month follow-up showed that while the total costs for the combination treatment were higher ($7,426), it provided an extra 1.17 quality-adjusted life years (QALYs), resulting in a favorable incremental cost-effectiveness ratio (ICER) of $23,295/QALY.
  • The study concludes that using rivaroxaban with aspirin is a cost-effective strategy in the U.S., making it a valuable option for managing cardiovascular health in these patients.

Department of Hepatopancreatobiliary and Transplant Surgery, Singapore General Hospital and National Cancer Centre Singapore, Singapore; Surgery Academic Clinical Programme, Duke National University of Singapore Medical School, Singapore. Electronic address:

Article Synopsis
  • This study assessed how liver cirrhosis (LC) influences the challenges of minimally invasive liver resection (MILR), specifically for minor surgeries involving primary liver tumors in anterolateral segments.
  • Conducted from 2004 to 2021 across 60 centers, the research involved 3,675 patients, with varying degrees of cirrhosis classified as Child A and Child B.
  • Results indicated that patients with Child A cirrhosis faced higher risks of complications, such as increased blood loss and rates of open conversion, while those with Child B cirrhosis had longer hospital stays and more significant morbidity; overall, the severity of LC complicates the surgical process, highlighting the need for better difficulty

Background: Procoagulant platelets are a subpopulation of highly activated platelets that promote coagulation through surface-exposed, negatively charged phospholipids, especially phosphatidylserine. Procoagulant platelets are important for clot stabilization during hemostasis, and an increased number of these platelets is associated with thrombotic risk. There is a need for harmonization in this area since many of the markers and methods used to assess procoagulant platelets are not specific when used in isolation but are also associated with platelet apoptosis.
