We used machine learning to investigate the residual visual field (VF) deficits and macula retinal ganglion cell (RGC) thickness loss patterns in recovered optic neuritis (ON). We applied archetypal analysis (AA) to 377 same-day pairings of 10-2 VF and optical coherence tomography (OCT) macula images from 93 ON eyes and 70 normal fellow eyes ≥ 90 days after acute ON. We correlated archetype (AT) weights (total weight = 100%) of VFs and total retinal thickness (TRT), inner retinal thickness (IRT), and macular ganglion cell-inner plexiform layer (GCIPL) thickness.

Principles governing the effects of sensory loss on human abilities: An integrative review.

Neurosci Biobehav Rev

December 2024

Cambridge Hearing Group, Department of Psychology, University of Cambridge, Cambridge, United Kingdom; Vision and Eye Research Institute, School of Medicine, Anglia Ruskin University, Cambridge, United Kingdom. Electronic address:

Blindness or deafness can significantly influence sensory abilities in intact modalities, affecting communication, orientation and navigation. Explanations for why certain abilities are enhanced and others degraded include: crossmodal cortical reorganization enhances abilities by providing additional neural processing resources; and sensory processing is impaired for tasks where calibration from the normally intact sense is required for good performance. However, these explanations are often specific to tasks or modalities, not accounting for why task-dependent enhancement or degradation are observed.

Bacterial clustering biomaterials as anti-infective therapies.

Biomaterials

December 2024

Department of Pharmacy and Pharmaceutical Sciences, National University of Singapore, 18 Science Drive 4, Singapore, 117559, Singapore. Electronic address:

In Nature, bacterial clustering by host-released peptides or nucleic acids is an evolutionarily conserved immune defense strategy employed to prevent adhesion of pathogenic microbes, which is prerequisite for most infections. Synthetic anti-adhesion strategies present as non-lethal means of targeting bacteria and may potentially be used to avoid resistance against antimicrobial therapies. From bacteria-agglutinating biomolecules discovered in nature to synthetic designs involving peptides, cationic polymers and nanoparticles, the modes of actions appear broad and unconsolidated.

Inability to express the confidence level and detect unseen disease classes limits the clinical implementation of artificial intelligence in the real world. We develop a foundation model with uncertainty estimation (FMUE) to detect 16 retinal conditions on optical coherence tomography (OCT). In the internal test set, FMUE achieves a higher F1 score of 95.

Significance: A data-based calibration method with enhanced depolarization contrast in polarization-sensitive optical coherence tomography (PS-OCT) was developed and demonstrated effective for detecting melanin content in the eye.

Aim: We aim to mitigate the dependence between the measured depolarization metric and the intensity signal-to-noise ratio (SNR) for improved visualization of depolarizing tissues, especially in low SNR regions, and to demonstrate the enhanced depolarization contrast to evaluate melanin presence.

Approach: A function for calibrating the depolarization metric was experimentally derived from the young albino guinea pig, assuming depolarization free in the retina.

Evaluating glaucoma in myopic eyes: Challenges and opportunities.

Surv Ophthalmol

December 2024

Hamilton Glaucoma Center, Shiley Eye Institute, Viterbi Family Department of Ophthalmology, UC San Diego, La Jolla, CA, United States. Electronic address:

The increasing global prevalence of myopia presents a significant public health concern, and growing evidence has demonstrated that myopia is a major risk factor for the development of open-angle glaucoma. Therefore, timely detection and management of glaucoma in myopic patients are crucial; however, identifying the structural alterations of glaucoma in the optic nerve head (ONH) and retinal tissues of myopic eyes using standard diagnostic tools such as fundus photography, optical coherence tomography (OCT), and OCT angiography (OCTA) presents challenges. Additionally, myopia-related perimetric defects can be confounded with glaucoma-related defects.

Background: Traffic crashes are the leading cause of death globally for people aged 5-29 years, with 90% of mortality occurring in low- and middle-income countries (LMICs). The STABLE (Slashing Two-wheeled Accidents by Leveraging Eyecare) trial was designed to determine whether providing spectacles could reduce risk among young myopic motorcycle users in Vietnam.

Methods: This investigator-masked, stepped-wedge, cluster randomised naturalistic driving trial will recruit 625 students aged 18-23 years, driving ≥ 50 km/week, with ≥ 1-year driving experience and using motorcycles as their primary means of transport, in 25 clusters of 25 students in Ho Chi Minh City, Vietnam.

Intraocular pressure (IOP) elevation is the primary risk factor and currently the main treatable factor for progression of glaucomatous optic neuropathy. In addition to direct clinical and living animal in vivo studies, ex vivo perfusion of anterior segments and whole eyes is a key technique for studying conventional outflow function as it is responsible for IOP regulation. We present well-tested experimental details, protocols, considerations, advantages, and limitations of several ex vivo model systems for studying IOP regulation.

Glaucoma is a heterogenous group of optic neuropathies characterized by the degeneration of optic nerve axons and the progressive loss of retinal ganglion cells (RGCs), which could ultimately lead to vision loss. Elevated intraocular pressure (IOP) is a major risk factor in the development of glaucoma, and reducing IOP remains the main therapeutic strategy. Endothelin-1 (ET-1), a potent vasoactive peptide, has been shown to produce neurodegenerative effects in animal models of glaucoma.

Corneal endothelium cells (CECs) regulate corneal hydration between the leaky barrier of the corneal endothelium and the ionic pumps on the surface of CECs. As CECs do not regenerate, loss of CECs leads to poor vision and corneal blindness. Corneal transplant is the only treatment option; however, there is a severe shortage of donor corneas globally.

Department of Ophthalmology, Peking University Third Hospital, Beijing Key Laboratory of Restoration of Damaged Ocular Nerve, Beijing, China.

Human induced pluripotent stem cells (hiPSCs) represent a promising cell source for generating functional cells suitable for clinical therapeutic applications, particularly in the context of autologous cell therapies. However, the production of hiPSCs through genetic manipulation, especially involving oncogenes, may raise safety concerns. Furthermore, the complexity and high costs associated with hiPSCs generation have hindered their broad clinical use.

Health Services and Systems Research, Duke-NUS Medical School, National University of Singapore, Singapore, Singapore.

Article Synopsis
  • About 1 in 3 adults have multiple chronic diseases, and while digital health innovations are aimed at improving care for these conditions, their adoption is still low.
  • The scoping review seeks to evaluate how these digital health strategies for chronic disease management are implemented and assessed, identifying frameworks, methods, barriers, and recommendations.
  • The review analyzed 252 studies, focusing mainly on mobile health, eHealth, and telehealth, but only a small fraction utilized formal implementation science frameworks, indicating the need for better integration of these theories in practice.

Purpose: Disorders of the anterior optic nerve cause quantifiable patterns, or archetypes (AT), in visual fields (VFs) obtained using standardized automated perimetry using stimulus size III (size III). VFs with stimulus size V (size V) can reduce retest variability in eyes with moderate to severe loss. We postulated that VF testing using both stimuli would show similar ATs in eyes with glaucoma and nonarteritic anterior ischemic optic neuropathy (NAION).
