Optic disk melanocytoma (ODM) is a rare ophthalmic tumor that can present with local compressive effects such as retinal vascular occlusion (RVO) that results in neovascularization of the disk (NVD) and is reportedly challenging to treat. We report the case of a 37-year-old Black African male with a two-year history of painless nonprogressive blur in his right eye vision. Findings on ocular examination include best corrected visual acuity right eye 6/18 and left eye 6/6, right eye relative afferent pupillary defect, and a large dark brown pigmented mass covering the optic disc measuring 4.

Prcis: Patients' acceptance of glaucoma therapy was high overall in this study, but lower for surgery than for laser or medical therapy. Fear and cost were the leading reasons why patients declined recommended therapy and they were treatment-specific. Cost was the primary reason for declining medical and laser therapy, while fear was the most common reason for declining surgical therapy.

Article Synopsis
  • Stargardt disease (SD) is a genetic eye condition characterized by significant variation in its symptoms and genetic causes, leading to central vision loss over time.
  • Early signs include yellow-white flecks in the retina and a dark appearance of the choroid on imaging tests, with some cases developing additional complications like choroidal neovascular membranes.
  • A study of four Nigerian patients revealed diverse presentations of SD, with one having good vision while the others experienced varying levels of vision loss, all showing signs of macular atrophy and flecks.

To find the adherence rate to periodic dilated eye examinations (DEEs) and its determinants among patients with diagnosed diabetes. In this cross-sectional study of 165 participants with diagnosed diabetes (Type 1/2) attending a general hospital with a diabetes clinic, we explored perceptions of barriers and facilitators of DEE at the individual level using a framework adapted from the health belief model (HBM). Patients were compared using t tests for continuous data and chi-square tests for categorical data.

Aim: To quantify the impact of coronavirus disease 2019 (COVID-19) on the rate of intravitreal antivascular endothelial growth factor (VEGF) injections (IVI) in eye hospitals in Nigeria.

Materials And Methods: A retrospective, observational, comparative study. The IVIs given 12 months before (pre-COVID) and 12 months after the first announcement of the COVID-19 lockdown (COVID) in Nigeria in four hospitals were used as the sample for this study.

Aim: To determine the causes of tractional retinal detachment (TRD) in Nigerians.

Materials And Methods: A prospective, multicentre study evaluating eyes diagnosed to have TRD. History, clinical examination (including visual acuity, intraocular pressure measurement, anterior segment examination and dilated fundoscopy) and systemic evaluation (including previous diagnosis of diabetes, hypertension, sickle-cell disease and others) were performed in TRD eyes out of a cohort of retinal detachment eyes.

Background: Exudative retinal detachment (ERD) is a rare type of retinal detachment (RD), and information on its causes and presentation in Nigerians and Black Africans is scarce.

Aim: To report the prevalence, vision at presentation, and causes of ERD in a cohort of RD patients.

Materials And Methods: A prospective, multicentre, hospital-based study.

Purpose: The purpose of the study is to evaluate choroidal thickness (CT) and the effect of age and gender in healthy Africans using enhanced depth imaging spectral-domain optical coherence tomography.

Materials And Methods: In 172 eyes of 88 volunteers, the CT in the central subfoveal (CSF) area and at 2.5 mm superior, inferior, nasal, and temporal quadrants was measured.

Background: Optical coherence tomography (OCT) is a noninvasive, frequently used imaging technology that enables detailed viewing of retina anatomy. It is used to monitor disease progression in retinitis pigmentosa (RP) eyes, including detecting changes in retinal thickness.

Purpose: The purpose of the study is to determine the clinical presentation and macular morphology in RP eyes using OCT imaging.

Background: The fellow eye of a retinal detachment is at risk of developing a retinal detachment and other visually debilitating disease.

Aim: To investigate the rate of bilaterality of retinal detachment (RD), the presenting visual acuity (VA), and the presence of ocular morbidity in the fellow eye of patients with RD.

Patients And Methods: A multicenter, prospective, cross-sectional study examining the fellow eye of consecutive patients who were diagnosed with different types of RD.

Article Synopsis
  • The study looked at how to treat a serious eye problem in people with sickle cell disease called proliferative sickle cell retinopathy (PSCR).
  • They reviewed 108 eyes from 88 patients to see what treatments were used and how well they worked.
  • The results showed that most patients improved or kept the same vision after treatment, but more tests are needed to find the best ways to treat different stages of PSCR.

Background: Optical coherence tomography (OCT) is widely used as the standard of care in evaluating macular and retinovascular diseases. However, the degree of OCT utilization is yet to be researched in a resource-limited country where wide gaps exist in access to healthcare.

Aim: To determine the rate of utilization of the OCT in diagnosis, pre-treatment, and post-treatment evaluation of macular and retinovascular diseases treated with intravitreal anti-vascular endothelial growth factor injection (IVI).

Glaucoma Treatment Patterns in Sub-Saharan Africa.

J Glaucoma

October 2023

Department of Ophthalmology and Visual Sciences, West Virginia University, Morgantown, WV.

Prcis: Physicians were most likely to recommend primary medical therapy upon diagnosis of glaucoma. Laser therapy was underutilized where they were available. Physicians were more likely to recommend surgery in severe glaucoma, laser therapy in mild glaucoma, while recommendation of medical therapy did not depend on glaucoma severity.

We present the case of a middle-aged female Nigerian diagnosed to have right eye peripapillary polypoidal choroidal vasculopathy (PCV). At presentation, her right eye Snellen visual acuity was 6/24+ (unaided) and 6/12 (aided) and the left eye was 6/9 (unaided) and 6/6 (aided). Fundus fluorescein angiography showed a hyperfluorescent peripapillary subretinal lesion associated with subretinal fluid demonstrated on spectral-domain optical coherence tomography.

Background: Retinal vascular occlusions are a common cause of visual impairment. Studies on retinal vascular occlusions in sub-Saharan Africa (SSA) have primarily been retrospective and on retinal vein occlusion (RVO) only. The aim of this study, therefore, was to determine the prevalence and pattern of retinal vascular occlusions and their systemic associations in SSA.

Nonarteritic central retinal artery occlusion (NA-CRAO) is a variant of acute ischemic stroke (AIS) and is a cause of sudden severe loss of vision. There are guidelines by the American Heart Association and the American Stroke Association for the care of CRAO patients. This review explores the basis of retinal neuroprotection for CRAO and its potential for improving the outcome of NA-CRAO.

Background: To objectively measure visual function amongst Primary Open Angle Glaucoma (POAG) patients and compare these with age and sex-matched controls by describing the characteristics of visual function in relation to the severity of POAG.

Methodology: A case-control study was carried out among 106 POAG patients and an equal number of age-sex matched controls attending Asokoro District Hospital, Abuja, and Eye Foundation Hospital Abuja from Nov 2012 to April 2013. The objective measures of visual function assessed include visual acuity (VA), contrast sensitivity (CS), colour vision (CV), and visual fields (MD) in the better eye (BE)].

Background: Optical coherence tomography provides high resolution in vivo images of the retina which are essential for diagnosis and follow up of patients with retina disorders like macula edema and exudative age-related macular degeneration. Establishing the normal range of central fovea values in our population provides vital baseline data for comparison.

Aim: To report the range of normal central fovea thickness measurements in eyes of healthy hospital patients in sub-Saharan Africa using a commercially available Fourier domain optical coherence tomography (OCT) scan.
