The use of ultrasound by emergency physicians is now well established. It can be integrated as an extension of the clinical examination, providing diagnostic support during consultation in the emergency department. However, its use in osteoarticular pathologies remains less frequent, despite a growing body of literature demonstrating its value in a variety of pathologies.

Introduction: Fewer than 200 cases of Perforated Duodenal Diverticulum ("PDD") have been reported in the literature. The percentage of caused by trauma is generally very low and similar to rates recorded for duodenal injuries caused by trauma in the absence of diverticula (3 %-5 %). As a rare cause of abdominal pain after trauma, perforated duodenal diverticula are seldom diagnosed preoperatively.

Physical activity is undeniably associated with numerous health benefits. However, performance of high intensity and/or high-volume exercise poses a significant physiological challenge to the cardiovascular and respiratory systems, which must undergo several adaptations to meet the increased metabolic demands of the organism. Repeated and prolonged exposure to training leads to long-term cardiac remodeling aimed at optimizing the efficiency of the work performed by the heart during exertion.

An increasing number of people are surviving cancer in Switzerland : 163,450 people were still alive in 2006 after a cancer diagnosis less than ten years prior, compared to 210,350 in 2016. However, most have to cope with debilitating emotional and physical aftereffects. A new 12-week rehabilitation program aims to restore patients' abilities.

With the increasing prevalence of neoplasia and kidney disease, patients undergoing oncological treatment are at greater risk of acute renal failure. Joint oncological and nephrological management requires a thorough understanding of the patient's clinical history, as well as the potential nephrotoxic effects of chemotherapy and immunotherapy. This article proposes a comprehensive approach to the assessment of acute renal failure in patients undergoing treatment for neoplasia.

Lack of awareness, access to insulin and diabetes care can result in high levels of morbidity and mortality for children with type 1 diabetes (T1DM) in sub-Saharan Africa (SSA). Improvements in access to insulin and diabetes management have improved outcomes in some settings. However, many people still present in diabetic ketoacidosis (DKA) in parallel to misdiagnosis of children with T1DM in contexts with high rates of communicable diseases.

Epiploic appendagitis is an etiology only coming at last of differential diagnoses in the face of acute abdominal pain. The typical patient is often a man between 20 and 50 years old, presenting with non-radiating abdominal pain with nausea, vomiting and inappetence with or without a mild febrile state. Biological assessment most of the time does not show any inflammatory syndrome, although slight elevations in sedimentation rate and CRP are possible.

On the face of challenges, particularly related to population aging, increasing complexity of medicine, demographic changes, and uneven geographical distribution of general practitioners (GPs), the healthcare system needs to be reimagined to ensure and maintain efficient care and address the chronic burden on emergency services. A concept of integrated community-based continuity of care, aimed at training future GPs, is one of the solutions proposed by the Northern Vaud Hospital Institutions. This project contributes to strengthening the interface and transition between the hospital and community medicine.

We report on the case of a 73-year-old man, who was referred to our emergency department for confusion and dyspnea. He had been under a treatment of flucloxacillin for six weeks because of a possible methicillin-susceptible Staphylococcus aureus (MSSA) endocarditis that was complicated by an acute late infection of a knee prosthesis. The laboratory work-up revealed a metabolic acidosis with a high anion gap.

[Methicillin-resistant : 15 years of molecular epidemiology in Western Switzerland].

Rev Med Suisse

April 2022

Unité hygiène, prévention et contrôle de l'infection, Service des maladies infectieuses, Département de médecine, Centre hospitalier universitaire vaudois, 1011 Lausanne.

Since the introduction of antibiotics, successive waves of Staphylococcus aureus clones occurred, each one having characteristic susceptibility pattern to antibiotics and virulence factors. We report here the results of a molecular epidemiological surveillance of methicillin-resistant S. aureus (MRSA) in French-speaking Switzerland between 2006 and 2020 showing the emergence and disappearance of clones known for their international dissemination, and the sporadic appearance of other international clones.

Background: Dexamethasone is widely used for the prevention of postoperative nausea and vomiting (PONV) but little is known about its efficacy for the treatment of established PONV.

Objective: To test the antiemetic efficacy of intravenous dexamethasone for the treatment of established PONV in adults undergoing surgery under general anaesthesia and to determine whether there is dose-responsiveness.

Design: The DexPonv trial is a multicentre, placebo-controlled, randomised, double-blind, dose-finding study.

Primary tumors of the pelvis are considered difficult to treat due to the complex anatomy and the proximity of important neurovascular structures. The surgical armamentarium for the treatment of these tumors has evolved with the help of cutting-edge technology from debilitating hemipelvectomies to solutions such as precise resections guided by patient-specific instruments or computer navigation and reconstruction by modular prostheses, 3D-printed custom-made implants, or orthotopic autograft reimplantation after extracorporeal irradiation. Different combinations of these techniques have been described in the literature with various rates of success.

Background: Surgical wound infection contributes to prolonged recovery time after pilonidal sinus excision. As a standard procedure after surgery, we recommend our patients to perform water irrigations in the intergluteal cleft 4 to 6 times a day during the post-operative period. Our hypothesis is that this should reduce healing time and complication rates.

A combination of targeted molecular methods and phenotypic drug-susceptibility testing is the most widely used approach to detect drug resistance in Mycobacterium tuberculosis isolates. We report the delay in the introduction of an efficient anti-tuberculous drug regimen because of a M. tuberculosis strain displaying a high level of resistance to isoniazid, in the absence of the common mutations associated with isoniazid-resistance, including katG mutations and inhA promoter mutations.

Article Synopsis
  • The development of tyrosine-kinase inhibitors (TKIs) has significantly improved survival rates for chronic myeloid leukemia (CML) patients, but resistance to TKIs can occur, necessitating hematopoietic stem cell transplant (HSCT), which is harmful to fertility.
  • Fertility preservation strategies, like ovarian stimulation, are complicated because TKIs can negatively affect ovarian response, prompting a case where interferon-α was used instead of TKIs during the fertility procedure.
  • The patient successfully underwent ovarian stimulation, resulting in the cryopreservation of nine zygotes, and three years post-HSCT, she maintains a substantial molecular response to treatment.
