Background: The feasibility and outcomes of salvage endoscopic submucosal dissection (ESD) and ESD for metachronous esophageal squamous cell cancer (ESCC) inside the irradiated field are not clear.
Aims: To explore the viability of ESD for ESCC in areas previously exposed to radiation, identify elements hindering successful ESD, assess prognosis, and elucidate considerations for follow-up.
Methods: Patients with ESCC who underwent salvage ESD or ESD for metachronous ESCC within the previously irradiated field at Kanagawa Cancer Center between January 1, 2013, and December 31, 2023, were retrospectively investigated to evaluate patient and lesion characteristics and short- and long-term outcomes.

Alkali caustic exposures can occur in the workplace with industrial chemicals, or in the home with common household products. These exposures, whether accidental or intentional, create risk for death or acute injury, such as airway compromise and esophageal or gastric perforation, as well as long-term complications such as stricture formation. Squamous cell carcinoma (SCC) of the stomach is a rare entity with fewer than 100 cases of primary SCC reported in the literature, while esophageal SCC is prevalent and more common. However, a synchronous squamous cell carcinoma found in the esophagus and stomach remains very uncommon. We present the case of a 64-year-old with a history of dysphagia who had an endoscopy that showed an impassable stenosis of the middle esophagus, with histopathology in favor of an esophagus squamous cell carcinoma.

Magnetic compression anastomosis (MCA) for esophageal stenosis requires the insertion of magnets through two channels, transoral and transgastrostomy. Herein, we designed a Y-Z deformable self-assembled magnetic anastomosis ring (Y-Z DSAMAR) for esophageal stricture anastomosis through the transoral passage only. We introduce a Y-Z DSAMAR and verify its feasibility for single-access esophageal stenosis anastomosis in isolated organs.

Esophageal hiatal hernia is a condition in which the esophageal hiatus opens and the stomach escapes from the abdominal cavity into the mediastinum. The basic surgical procedures are 1) return of the prolapsed stomach into the abdominal cavity, 2) suture of the dilated esophageal hiatus, and 3) fundoplication and fixation of the stomach to prevent gastroesophageal reflux disease (GERD). The Japanese guidelines for the treatment of GERD recommend laparoscopic Toupet fundoplication as the standard procedure, which we also follow.
