This case presents a 12-year-old male patient diagnosed with preseptal cellulitis that progressed to a subperiosteal orbital abscess and eventually intracranial extension, despite outpatient antibiotic therapy. Initially treated with oral antibiotics for left eyelid swelling and pain, his condition worsened, prompting hospital admission and eventual surgical intervention. Imaging revealed multiple abscesses and a hematoma, causing mass effect on the globe and extraocular muscles.

Coronavirus disease 2019 (COVID-19) has been associated with numerous complications beyond the respiratory tract, including spinal and joint co-infections and secondary infections. However, we could not locate any reported cases of bilateral shoulder infections with additional spinal infections associated with mild (like our patient) COVID-19 or with more severe cases of COVID-19. We report the case of a healthy 62-year-old male who presented with mild COVID-19 and concurrent methicillin-sensitive (MSSA) bacteremia associated with multiple musculoskeletal sites of deep-seated infection.

Imaging is crucial in assessing bacterial and tubercular spondylodiscitis, as well as their associated complications. Magnetic resonance imaging in particular can clearly depict osteo-structural changes in the vertebral body and the associated disc, as well as any soft-tissue complications, such as paravertebral abscess and/or epidural abscess, improving disease characterization and helping to recognize the agent involved.

This is a case of a spinal epidural abscess (SEA) in a 41-year-old male with a history of chronic alcohol abuse, who initially presented with severe diabetic ketoacidosis (DKA). Following stabilization of DKA, the patient developed persistent gram-positive bacteremia, which progressed to an epidural abscess extending from the cervical to the thoracic spine with potential intracranial extension. Despite the complexity of the case, prompt diagnosis via magnetic resonance imaging (MRI) and timely neurosurgical intervention for drainage, combined with targeted antibiotic therapy, led to a favorable outcome.
