  • The Trillat procedure, developed by Albert Trillat, is one of the earliest techniques for treating recurrent anterior shoulder instability, utilizing unique biomechanical strategies that enhance the shoulder's stability.
  • This procedure involves modifying the coracoid process and changing the subscapularis muscle's function, reinforcing its role in stabilizing the humeral head and preventing dislocation.
  • Though modern techniques like arthroscopic Bankart repair are more commonly used, the Trillat procedure is still relevant, especially for older patients with large cuff tears or younger ones with hyperlaxity, albeit with potential complications such as pain, reduced motion, and impingement.

An Update on Breakthrough Invasive Mold Infections.

Mycopathologia

June 2024

Division of Infectious Diseases, University Hospital of Geneva, Rue Gabrielle-Perret-Gentil 4, 1211, Geneva, Switzerland.

The incidence of breakthrough mold infections (bIMI) has been increasing, due to routine administration of broad-spectrum antifungal prophylaxis and an increasing pool of high-risk patient populations, with fungi more challenging to treat, resulting in a sustained high mortality, despite progress in diagnostic and therapeutic options. Pharmacokinetics of antifungal drugs, fungal, and host, including genetic, factors play a role in the emergence of bIMI. Suggested therapeutic approaches have included change of antifungal class treatment, with amphotericin-B products predominating as first-line empirical treatment and switching from one broad-spectrum azole to another remaining the most frequently used treatment modalities.

Article Synopsis
  • - The study investigates the genetic factors contributing to chronic post-surgical pain (CPSP) by analyzing data from 1,350 individuals who underwent various types of surgery, and highlights a significant genetic component, estimating a 39% heritability for CPSP through meta-analysis.
  • - Researchers identified 77 key genetic variations (SNPs) linked to CPSP and noted that most of these are associated with immune system genes, especially those related to B and T cells.
  • - Animal studies showed that mice without T and B cells experienced worsened pain after surgery, which could be mitigated by transferring B cells, suggesting that the adaptive immune system plays a crucial protective role against CPSP.

Article Synopsis
  • Angiogenesis plays a crucial role in heart repair following a heart attack (myocardial infarction), and the study focused on tracking its changes using a specific PET tracer, Ga-NODAGA-RGD, which targets a marker of angiogenesis (integrin αβ) in 20 STEMI patients.
  • By using PET imaging at multiple time points (1 week, 1 month, and 3 months post-STEMI), the study found that most patients (95%) showed increased uptake of the tracer in the affected heart tissue at one week, with levels stabilizing after one month but decreasing by three months.
  • Additionally, the study identified correlations between the uptake of the tracer and changes in blood flow parameters and heart
