Retin Cases Brief Rep

September 2024

Department of Ophthalmology, Massachusetts Eye and Ear, Harvard Medical School, Boston, Massachusetts.

Purpose: Postoperative endophthalmitis is a relatively uncommon, but potentially visually devastating, complication associated with cataract surgery. Specific microbial causes of endophthalmitis are characteristically associated with particular disease time courses. Although Cutibacterium acnes is typically associated with an indolent course of inflammation, we report a case of C.

Pathobiology of Fungal Endophthalmitis: A Major Review.

ACS Infect Dis

September 2024

Jhaveri Microbiology Centre, Brien Holden Eye Research Centre, L. V. Prasad Eye Institute, Hyderabad, Telangana 500034, India.

Fungal endophthalmitis is a chronic inflammatory condition of the eye's posterior segment that can lead to irreversible vision loss. While relatively rare in western countries, its incidence is notably higher in Asia, particularly India. The condition's prevalence is exacerbated by factors such as intravenous drug use, antibiotics, and ocular surgeries.

: To compare the prevalence of intra-ocular inflammation (IOI) between brolucizumab and aflibercept in neovascular age-related macular degeneration (nAMD) after intra-vitreal injections (IVI) and to compare the IOI odds ratios (ORs) of both therapies with the prevalence of septic endophthalmitis after IVI that was previously reported in the literature. : A total of 468 IVI of brolucizumab (117 eyes) were compared with 2884 IVI of aflibercept (305 eyes) regarding IOI and occlusive retinal vasculitis (RV) from December 2021 to June 2023 in this retrospective study. The OR was calculated for both anti-VEGF agents and was compared with the relative risk of septic endophthalmitis after IVI.

Article Synopsis
  • The study examined endogenous endophthalmitis (EE) in Spain and Mexico, analyzing 705 cases and identifying 78 EE cases, highlighting differences in microbial infections, antibiotic resistance, and patient outcomes between the two countries.
  • In Spain, 60.25% of cases had bacterial infections (primarily Staphylococcus and Streptococcus), while in Mexico, 39.74% had fungal infections (mostly Candida), with diabetes mellitus being a significant risk factor, especially prevalent in Mexico.
  • Treatment included various antibiotics and procedures, revealing overall better success rates for bacterial infections (around 50%) than fungal infections (around 30%); the study stresses the need for region-specific healthcare strategies and continuous research on EE management

Department of ENT and Head & Neck Surgery, Kasturba Medical College, Mangalore, Manipal Academy of Higher Education, Manipal, India.

Background: Pantoea agglomerans is a gram negative, aerobic/facultative anaerobic, rod shaped bacilli commonly isolated from plants, soil, food and faeces.(1) It is a rare cause of opportunistic infections in humans acquired mainly via two major routes being, wound infection or hospital acquired.

Case Report: Here, we encountered a landmark, first of its kind, head and neck manifestation of a cervical soft tissue abscess with Pantoea agglomerans being the miscreant.

Ocular surface inflammatory disorders, such as dry eye, are becoming increasingly prevalent. Developing new treatment strategies targeting harmful bacteria could provide significant therapeutic benefits. The purpose of this study was to characterize the common ocular pathogen Staphylococcus aureus and the rarer endophthalmitis-associated species Enterococcus faecalis isolated from the ocular surface of dry eye disease patients in Norway.

Article Synopsis
  • The study aimed to compare the effectiveness of iodine compounds and levofloxacin in cleaning the conjunctival sac after cataract surgery.
  • It involved 128 patients who received either levofloxacin or diluted iodine drops for a week after surgery, with bacterial cultures taken before surgery and at one and two months post-op.
  • Results showed that both treatments reduced bacterial contamination initially, but levofloxacin was more effective in reducing bacterial DNA; however, it also led to the emergence of more resistant bacteria over time, unlike the iodine treatment.

Article Synopsis
  • The study analyzed the demographic and clinical profiles of 245 evisceration patients, focusing on factors like age, gender, causes of evisceration, and postoperative complications.
  • Results indicated that the majority of patients were young males, with penetrating globe injuries being the leading cause of evisceration and a mean follow-up of 52.5 months.
  • The Global Ocular Prosthesis Score (GOPS) demonstrated that while evisceration has negative psychosocial impacts, the use of ocular prostheses significantly enhances patient satisfaction.

endophthalmitis is a rare cause of endogenous endophthalmitis, with very few cases documented in the US. We present a male patient in his 60s with a history of latent tuberculosis who presented to the hospital with complaints of acute bilateral vision loss that began three days prior to admission. The workup revealed bacteremia, a large hepatic abscess, severe orbital swelling, and acute angle-closure glaucoma.

Article Synopsis
  • This study evaluates the rate of acute endophthalmitis following secondary intraocular lens (IOL) implantation in South Korea, utilizing data from the National Health Insurance Service over 20 years (2002-2021).
  • Out of 39,364 patients who had secondary IOL surgeries, only 62 developed acute postoperative endophthalmitis, resulting in an incidence rate of 0.16%, with most cases occurring within the first week post-surgery.
  • Higher risks for endophthalmitis were associated with patients having pre-existing glaucoma and those undergoing concurrent vitrectomy, and the study findings align with endophthalmitis rates observed in other countries.

Article Synopsis
  • This study aimed to evaluate the effectiveness of a new ophthalmic solution made from ozonated sunflower oil in liposomes combined with hypromellose against the standard povidone iodine 5% in reducing bacterial presence in patients before eye injections.
  • The research involved 195 patients, where one eye received the new treatment (Ozodrop and BlefOX) while the other was treated with povidone iodine, with swabs taken from both eyes to measure microbial levels before and after treatment.
  • Results showed that the new treatment significantly decreased bacterial loads more than povidone iodine alone, indicating its potential as a superior antiseptic for ocular procedures.

Background: Age-related macular degeneration (AMD) is the leading cause of vision loss in elderly people. Current pharmacological treatment in vascular AMD includes anti-VEGF agents, such as ranibizumab and aflibercept. Additionally, the off-label use of bevacizumab has been shown to be effective and has a lower cost, making it an interesting pharmacological approach; however, there is no consensus about its use.

The combination of multiplex polymerase chain reaction (mPCR) and microfluidic technologies demonstrates great significance in biomedical applications. However, current microfluidics-based molecular diagnostics face challenges in multi-target detection due to their limited fluorescence channels, complicated fabrication process, and high cost. In this research, we proposed a cost-effective sandblasting method for manufacturing silicon microchips and a chip-based microdevice for field mPCR detection.
