Purpose: To investigate the clinical characteristics and systemic risk factors of related endogenous endophthalmitis (KPREE) in China and explore the possible pathophysiological mechanisms.

Methods: This was a retrospective comparative study. All enrolled KPREE patients were followed up for at least 1 month to observe their clinical characteristics, unfavorable prognosis, and risk factors, and were compared with intraocular surgery-related postoperative endophthalmitis (ISRPE).

We describe a unique case of sclerouveitis that progressed to endogenous endophthalmitis in a 69-year-old male with chronic lymphocytic leukemia (CLL). We highlight the risk of treating sclerouveitis with oral corticosteroids, which can exacerbate an infection and contribute to disease progression. This case demonstrates that probing and 10% povidone-iodine irrigation of the nasolacrimal duct prior to intravitreal bevacizumab injection can safely reduce the risk of endophthalmitis in infants with aggressive posterior retinopathy of prematurity and nasolacrimal duct obstruction.
