Purpose: We constructed a clinical clue-based algorithm to identify the microbiology-positive post-cataract surgery endophthalmitis.

Methods: The Endophthalmitis Infectivity Measurement Algorithm (EIMA) was constructed using presenting Snellen vision (Letter score [LS]) and Inflammation Score (IS, from the cornea, anterior chamber, iris, and vitreous). Article Synopsis
  • The study examines current practices of immediate sequential bilateral cataract surgery (ISBCS) among Italian ophthalmologists and their future attitudes toward it.
  • A multicenter survey was conducted with 246 of 2300 invited participants, revealing that 63.4% perform ISBCS, mostly on an occasional basis, with over half interested in making it routine.
  • The main advantages for ISBCS were identified as procedure efficiency and patient benefits, while concerns over risks like bilateral endophthalmitis and legal issues hinder broader adoption.

Purpose: Report the clinical findings, risk factors, treatment, and visual outcomes associated with  endophthalmitis in comparison to culture-positive endophthalmitis associated with non- species.

Methods: A retrospective chart review of adults between 18 and 89 years of age diagnosed with exogenous culture-positive endophthalmitis between January 1, 2009, and January 1, 2018, at the Duke Eye Center (Durham, North Carolina) with at least six months of follow-up from time of initial diagnosis was conducted. Clinical data including patient demographics, ocular history, baseline corrected visual acuity (VA) prior to presentation, time to presentation, presenting exam findings, VA at presentation, presumed etiology of endophthalmitis, medical and surgical management, and VA at the six-month follow-up was extracted and statistically analyzed.

Cryptococcus neoformans is a global invasive mycosis that is known to cause significant morbidity and mortality. It is commonly observed that individuals with compromised immune systems are more prone to developing cryptococcal meningitis. Although ocular involvement is rare, previous studies have indicated that ocular lesions precede symptomatic meningitis in only 27 % of patients with central nervous system involvement.

Article Synopsis
  • - The study highlights the importance of early diagnosis and treatment of viral keratitis (VK) to improve patient outcomes, focusing on the role of antiviral medications and supportive care.
  • - Researchers analyzed samples from 162 patients suspected of ocular infections and found that 29.6% tested positive for various viruses, with the highest prevalence being varicella-zoster virus (VZV) and herpes simplex virus type 1 (HSV-1).
  • - Multiplex real-time PCR was used for efficient viral diagnosis, revealing that aqueous humor samples were particularly effective for detecting VK, potentially changing how diagnoses are made and reducing unnecessary antibiotic use.

Purpose: The aim of this study was to investigate the predictive factors for evisceration or enucleation surgery after open globe injury (OGI) and to investigate the effectiveness of the ocular trauma score (OTS) in predicting the outcome.

Method: Data from 380 patients who underwent OGI repair were retrospectively analyzed. The eyes of 31 patients who underwent evisceration or enucleation after OGI repair and who were followed up for at least 6 months were included in the study.

In-vitro safety assessment of meropenem on human retinal pigment epithelium (RPE).

Heliyon

July 2024

Poostchi Ophthalmology Research Center, Department of Ophthalmology, School of Medicine, Shiraz University of Medical Sciences, Shiraz, Iran.

Purpose: Endophthalmitis is a severe infection accompanied by inflammation that affects the anterior and posterior parts of the eye. It is typically treated with a combination of antibiotics that cover various microorganisms. However, retinal pigment epithelium (RPE) cells are highly susceptible to damage from intravitreal injection therapy.

Endophthalmitis Rates and Types of Treatments After Intraocular Procedures.

JAMA Ophthalmol

September 2024

Vickie and Jack Farber Vision Research Center at Wills Eye, Wills Eye Hospital, Philadelphia, Pennsylvania.

Importance: Long-term trend analyses of overall endophthalmitis rates and treatment patterns are scarce. It is also unknown if the deviation from the recommendations of the Endophthalmitis Vitrectomy Study toward decreased utilization of vitrectomy is associated with different vision outcomes.

Objective: To determine whether the rate of endophthalmitis after intraocular procedures or the primary treatment (prompt vitrectomy vs tap and inject) for endophthalmitis has changed over the past 20 years.

Background: Endophthalmitis is a clinical diagnosis but identification of the disease-causing agent or agents allows for a more tailored treatment. This is routinely done through intraocular fluid cultures and staining. However, culture-negative endophthalmitis is a relatively common occurrence, and a causative organism cannot be identified.

Management of open globe injury: a narrative review.

Eye (Lond)

November 2024

Northern Ireland Medical and Dental Training Agency, Belfast, UK.

Open globe injuries are a significant global cause of visual loss, including unilateral and bilateral blindness. Prognosis is predicted by injury severity, with lower presenting visual acuity and more posterior injuries associated with poor visual outcomes, although even the most severely injured eyes with no perception of light vision may regain some visual function. In addition to severity of the primary injury, the secondary injuries and complications causing poor outcomes include proliferative vitreoretinopathy (PVR) and endophthalmitis.

Article Synopsis
  • Diabetic macular edema (DME) is a serious eye condition linked to diabetes that can lead to vision loss, with treatments like pars plana vitrectomy (PPV) and internal limiting membrane (ILM) peeling being used to improve outcomes.
  • A study involving 81 patients compared the effects of PPV alone versus PPV combined with ILM peeling, finding that the combination treatment led to significantly better improvements in central macular thickness, best-corrected visual acuity, and cystoid macular edema volume after surgery.
  • Both groups experienced similar rates of complications post-surgery, indicating that while the combined approach may enhance visual outcomes, it does not increase the risk of adverse effects.

Introduction Endogenous endophthalmitis is characterized by severe intraocular inflammation caused by the invasion of microorganisms into the anterior and posterior chambers of the eye. It results from hematogenous spread from distant foci of infection. This, in turn, leads to potential vision loss and blindness due to reduced anatomical and functional outcomes.
