Bone Health ECHO (Extension for Community Healthcare Outcomes) is a virtual community of practice, where healthcare professionals have met via videoconferencing weekly since 2015. This model of learning is focused on short didactics and the presentation of real but de-identified patient cases followed by highly interactive discussions. These are often clinical situations with diagnostic and therapeutic dilemmas that are not readily addressed by randomized placebo-controlled clinical trials and clinical practice guidelines.

Article Synopsis
  • * Data from 120 T2DM patients, aged 30-60, was analyzed over a year, showing that women taking 10 mg of Empagliflozin had a notable decrease in ALT levels when compared to men not on the medication.
  • * The findings suggest that starting SGLT2-I treatment can lower ALT levels in these patients, highlighting the potential benefit of early intervention along with lifestyle changes to improve liver health and reduce

  • The glucagon receptor antagonist volagidemab is a novel monoclonal antibody that helps manage type-1 diabetes (T1D) by lowering insulin needs and reducing risks like diabetic ketoacidosis.
  • A systematic review of three randomized controlled trials (98 patients) found that volagidemab significantly decreased the total daily dose of insulin and average blood glucose levels compared to a placebo.
  • Additionally, while it increased the time patients spent within the target blood glucose range, there was no significant change in the occurrence of adverse events or hypoglycemia when compared to the placebo group.

Atenolol is a widely prescribed β-cardioselective blocker. We studied atenolol effects on cardiac and renal development in day 18 (D18) chicken embryos. Embryos were dosed with atenolol (3 μg atenolol/g estimated embryo mass) for three days during one of the mesonephric kidney stage (D7-D9), mesonephric-metanephric stage (D11-D13), or metanephric stage (D15-D17), and then sampled on D18.

Article Synopsis
  • Type 2 diabetes (T2D) is a significant health issue in the U.S., and this study focuses on how racial and ethnic differences affect the prescription of effective diabetes medications.
  • The research analyzed data from 57,320 patients, revealing notable disparities in the prescription rates of GLP-1 medications like tirzepatide, semaglutide, and dulaglutide among different racial and ethnic groups compared to White patients.
  • The findings suggest that these disparities could lead to worse health outcomes for underrepresented groups, signaling a need for targeted interventions to ensure equitable access to diabetes care.

Diabetic autonomic neuropathy (DAN) and its associated cardiovascular autonomic neuropathy (CAN) can lead to potentially fatal complications. We analyzed two distinct cases of DAN/CAN based on comprehensive cardiovascular autonomic reflex tests (CARTs). Case 1 involves a 27-year-old patient with T1DM suffering from recurrent severe hypoglycemic unawareness due to DAN.

Objectives: This feasibility study evaluated the effectiveness of Support-Engage-Empower-Diabetes (SEE-Diabetes), a patient-centered educational tool designed to promote shared decision-making of diabetes management in older adults. We aimed to assess SEE-Diabetes's ability to facilitate patient engagement and collaborative goal setting, as measured by the Observational Patient Involvement (OPTION) scale and Shared Decision-Making Questionnaire (SDM-Q-Doc). We hypothesized that these instruments would effectively differentiate between healthcare providers who actively leveraged SEE-Diabetes to guide patient-centric conversations and set goals compared to those who did not.

October 2024

Department of Clinical and Experimental Medicine, Division of Gynecology and Obstetrics, University of Pisa, Via Roma 67, 56126 Pisa, PI, Italy. Electronic address:

Article Synopsis
  • The menopause transition significantly increases a woman's risk of mood disorders due to fluctuations in estrogen and progesterone levels.
  • Low estradiol levels after menopause impact the central nervous system, leading to an imbalance in neurotransmission that can result in symptoms like hot flashes, depression, and anxiety.
  • The review highlights the complex neuroendocrine changes occurring during this transition, focusing on the various signaling pathways and neurotransmitters involved in mood alterations.

Immediate and delayed placement of the intrauterine device after abortion: a systematic review and meta-analysis.

Sci Rep

May 2024

Reproductive Endocrinology Center, Hangzhou Women's Hospital, 369 Kunpeng Road, Shangcheng District, Hangzhou, 310000, Zhejiang, China.

This article aims to report the comprehensive and up-to-date analysis and evidence of the insertion rate, expulsion rate, removal rate, and utilization rate of immediate placement of intrauterine devices (IUDs) versus delayed placement after artificial abortion. PubMed, Embase, Cochrane, Web of Science, CNKI, and Wanfang databases were comprehensively searched up to January 12, 2024 for studies that compared immediate versus delayed insertion of IUDs after abortion. The evaluation metrics included the number of IUD insertion after surgical or medical abortions, the frequency of expulsion and removal at 6 months or 1 year, the number of continued usage, pain intensity scores, the number of infections, the duration of bleeding, and instances of uterine perforation during or after IUD insertion.

At the beginning of the eighteenth century, most physicians recognized cancer as an aggressive process that gradually spreads, leading to cachexia and death. Thyroid malignancies had long been underestimated because the majority of the population of West Europe suffered from diffuse goiters that masked malignant processes in the neck. Moreover, the life expectancy at that time was very low (about 37-40 years), so the majority of people died of other causes before metastatic thyroid cancer could develop and manifest.

April 2024

Brazilian Group for the Study of Inherited and Acquired Lipodystrophies (BRAZLIPO), Fortaleza, Brazil.

Article Synopsis
  • Lipodystrophies are disorders that cause the selective loss of fat tissue, leading to increased risks of insulin resistance and related health issues, particularly in women with familial partial lipodystrophy type 2 (FPLD2), who also often face complications with pregnancy and gynecological conditions like PCOS.
  • The study conducted a retrospective analysis of 8 women diagnosed with FPLD2 who had pregnancies, examining their pregnancy outcomes and associated complications.
  • Results indicated significant fetal complications, including high rates of gestational diabetes, preeclampsia, miscarriages, and other neonatal issues, although no cases of infertility were reported.

Subtype Identification of Surgically Curable Primary Aldosteronism During Treatment With Mineralocorticoid Receptor Blockade.

Hypertension

June 2024

Internal Emergency Medicine Unit, Department of Medicine, Specialized Center for Blood Pressure Disorders-Regione Veneto (G.P., T.M.S., D.B., B.C., G.C., G.R., G.P.R.), University of Padova, Italy.

Background: Current guidelines and consensus documents recommend withdrawal of mineralocorticoid receptor antagonists (MRAs) before primary aldosteronism (PA) subtyping by adrenal vein sampling (AVS), but this practice can cause severe hypokalemia and uncontrolled high blood pressure. Our aim was to investigate if unilateral PA can be identified by AVS during MRA treatment.

Methods: We compared the rate of unilateral PA identification between patients with and without MRA treatment in large data sets of patients submitted to AVS while off renin-angiotensin system blockers and β-blockers.
