Patient-reported outcomes following esophagectomy.

J Thorac Dis

October 2024

Department of Surgery, Section of General Thoracic Surgery, Emory University School of Medicine, Atlanta, GA, USA.

The impact on quality of life (QoL) following esophagectomy is well reported in many studies that have analyzed patient-reported outcomes (PROs). The morbidity surrounding esophagectomy mainly lies within the significant lifestyle changes that can be permanent in some patients. Research efforts in cancer treatments primarily focus on measurements of survival and major morbidity to establish the best therapy that provides the longest survival, but they do not consider the global well-being of the patient.

Article Synopsis
  • * A total of 140 patients were evaluated, with 74% reporting symptom relief shortly after surgery and 84% maintaining that relief after an average of 2 years.
  • * Factors such as groove pancreatitis and shorter hospital stays were associated with better outcomes, indicating these procedures can significantly improve quality of life for patients.

This study evaluates the accuracy of a newly developed intraocular lens (IOL) power calculation method that applies four different IOL power calculation formulas according to 768 biometric subgroups based on keratometry, anterior chamber depth, and axial length. This retrospective cross-sectional study was conducted in at Korea University Ansan Hospital. A total of 1600 eyes from 1600 patients who underwent phacoemulsification and a ZCB00 IOL in-the-bag implantation were divided into two datasets: a reference dataset (1200 eyes) and a validation dataset (400 eyes).

Purpose: Opioid pain medication is most commonly prescribed after distal radius fracture fixation, and there is high variability in the quantity and duration prescribed. Comorbidities, including substance use and depression, have been associated with higher consumption habits, and increased sizes of postoperative opioid prescriptions have been previously linked to an increasing risk of chronic opioid use and opioid use disorder. The purpose of this study was to investigate opioid prescription patterns after distal radius fracture fixation and identify patient-specific risk factors associated with increased opioid prescription refills.

Objectives: Interprofessional (IP) collaboration and effective teamwork remain variable in healthcare organisations. IP bias, assumptions and conflicts limit the capacity of healthcare teams to leverage the expertise of their members to meet growing complexities of patient needs and optimise healthcare outcomes. We aimed to understand how a longitudinal faculty development programme, designed to optimise IP learning, influenced its participants in their IP roles.

Purpose: KLHL20 is part of a CUL3-RING E3 ubiquitin ligase involved in protein ubiquitination. KLHL20 functions as the substrate adaptor that recognizes substrates and mediates the transfer of ubiquitin to the substrates. Although KLHL20 regulates neurite outgrowth and synaptic development in animal models, a role in human neurodevelopment has not yet been described.

Small bowel obstructions (SBO) are a commonly encountered diagnosis within emergency departments. Typically, these patients have evident risk factors including, but not limited to, prior abdominal surgery, personal or family history of gastrointestinal disorders, femoral and inguinal hernias, or neoplasm. In this case, we describe an SBO in a female, professional athlete whose swift, severe symptom onset, rapid resolution with conservative treatment, lack of identifiable risk factors, and prompt return to high level competition without recurrence are certainly unique.

Background: The aim of this study was to assess the reader variability in quantitatively assessing pre- and post-treatment 2-deoxy-2-[F]fluoro-D-glucose positron emission tomography/computed tomography ([F]FDG PET/CT) scans in a defined set of images of cancer patients using the same semi-automated analytical software (Auto-PERCIST™), which identifies tumor peak standard uptake value corrected for lean body mass (SUL) to determine [F]FDG PET quantitative parameters.

Methods: Paired pre- and post-treatment [F]FDG PET/CT images from 30 oncologic patients and Auto-PERCIST™ semi-automated software were distributed to 13 readers across US and international sites. One reader was aware of the relevant medical history of the patients (read), whereas the 12 other readers were blinded to history but had access to the correlative images.

Article Synopsis
  • A phase 2 study investigated the effectiveness and safety of combining gemcitabine with pazopanib in treating advanced soft-tissue sarcoma (STS), as options for such patients are limited.
  • The trial involved 90 patients previously treated with anthracyclines, comparing two treatment regimens: gemcitabine plus pazopanib (G+P) vs gemcitabine plus docetaxel (G+T), with similar outcomes in terms of median progression-free survival (PFS) and response rates.
  • Both treatments resulted in high rates of grade ≥3 adverse events, although the specific types and frequencies varied, indicating that while both regimens were comparably tolerable, they

Outside sleep laboratory settings, peripheral arterial tonometry (PAT, eg, WatchPat) represents a validated modality for diagnosing obstructive sleep apnea (OSA). We have shown before that the accuracy of home sleep apnea testing by WatchPat 200 devices in diagnosing OSA is suboptimal (50%-70%). In order to improve its diagnostic performance, we built several models that predict the main functional parameter of polysomnography (PSG), Apnea Hypopnea Index (AHI).

Background: A narrow-profile powered vascular stapler (PVS) was developed to provide superior access and precise staple placement in thoracic procedures. The objective of this study was to determine if the PVS would yield an equivalent rate of hemostatic interventions compared with standard of care (SOC) staplers in video-assisted thoracoscopic surgery lobectomy.

Materials And Methods: A randomized, controlled, multicenter study was conducted comparing PVS with SOC staplers in lobectomies performed for non-small cell lung cancer.

Statin-Induced Necrotizing Autoimmune Myopathy.

JACC Case Rep

March 2020

Divison of General Medicine and Geriatrics, Grady Memorial Hospital, Atlanta, Georgia.

Statin-induced necrotizing autoimmune myopathy (SINAM) is an exceptionally rare yet devastating complication of statin therapy that can occur at any time after initiation. It should be considered in patients who develop proximal muscle weakness and marked elevated creatine phosphokinase while taking statin therapy. ().

Importance: Prior evidence suggests that racial differences exist in tau biomarkers in mild cognitive impairment (MCI) and Alzheimer disease (AD). Whether this reported disparity is associated with a differential level of neurodegeneration and disease stage or with underlying mechanisms separate from amyloid or tau is unclear.

Objectives: To compare cerebrospinal fluid (CSF) biomarkers in African American and white individuals with normal cognition and MCI, to estimate race-based cutoffs for these biomarkers that maximize diagnostic discrimination between normal cognition and MCI, and to study the association of demographic characteristics, cognitive performance, and common vascular risk factors with these differences.
