Background And Objectives: Covert brain infarcts (CBIs) in patients with first-ever ischemic stroke (IS) and atrial fibrillation (AF) are associated with an increased risk of stroke recurrence. We aimed to assess whether CBIs modify the treatment effect of early vs late initiation of direct oral anticoagulants (DOACs) in patients with IS and AF.
Methods: We conducted a post hoc analysis of the international, multicenter, randomized-controlled ELAN trial, which compared early (<48 hours after ischemic stroke for minor and moderate stroke, 6-7 days for major stroke) vs late (>48 hours for minor, 3-4 days for moderate, 12-14 days for major stroke) initiation of DOACs in patients with IS and AF.

We report here on a patient with concomitant indolent lymphoma who showed a rapid progressive deterioration of his general condition and emerging neurological symptoms. The combination of severe B symptoms with hypermetabolic involvement of the adrenal glands and multiple central nervous system (CNS) lesions initially suggested a malignant disease. However, when the patient presented to us with biopsy results from one of the CNS lesions, the biopsy revealed granulomatous inflammation but no evidence of malignancy.

Methods: A total of n = 52 healthy volunteers and n = 52 patients with histologically confirmed glioma underwent two consecutive 3-T MRI scans separated by a 1-min break.

Perihematomal hypoperfusion may lead to ischemic damage during intraparenchymal cerebral hemorrhage (ICH), resulting in worse prognosis. We aimed to (1) investigate the relationship between serum biomarkers related to oxidative stress and vasoactive substances and the occurrence of hypoperfusion and ischemic perihematomal lesions in ICH and (2) evaluate their correlation with the volumetric evolution of the hematoma and perihematomal edema. Emergency neuroradiology provides rapid diagnostic decision-making and guidance for management for a wide range of acute conditions involving the brain, head and neck, and spine. This narrative review aims at providing an up-to-date discussion about the state of the art of applications of artificial intelligence in emergency neuroradiology, which have substantially expanded in depth and scope in the past few years. A detailed analysis of machine learning and deep learning algorithms in several tasks related to acute ischemic stroke involving various imaging modalities, including a description of existing commercial products, is provided.

Resting-state functional MRI (rs-fMRI), a promising method for interrogating different brain functional networks from a single MRI acquisition, is increasingly utilized in clinical presurgical and other pretherapeutic brain mapping. However, challenges in standardization of acquisition, preprocessing, and analysis methods across centers, and variability in results interpretation, complicate its clinical use. Additionally, inherent problems regarding reliability of language lateralization, interpatient variability of cognitive network representation, dynamic aspects of intranetwork and internetwork connectivity, and effects of neurovascular uncoupling on network detection still must be overcome.

MCAP is caused by somatic mosaicism of PIK3CA, a crucial gene in regulation of cell growth and survival, and is one of the disorders in the PIK3CA-related overgrowth spectrum.

Purpose: The aim of our study is to verify the reliability of Diffusion Kurtosis Imaging (DKI) parameters through correlation with perfusion metrics obtained by Dynamic Contrast Enhanced (DCE)- and Dynamic Susceptibility Contrast (DSC)-MRI techniques in histologic-proven primary central nervous system lymphoma (PCNSL).
Methods: A total of 15 lesions were analyzed in patient with neo-diagnosis of Epstein-Barr Virus negative PCNSL. DKI was acquired using 5b values from 0 to 2500 s/mm.
