The length of the biliopancreatic limb in one anastomosis gastric bypass.

Front Surg

August 2024

Bariatric and Metabolic Surgery Unit, Department of General and Abdominal Surgery, AZ Sint Elisabeth Hospital, Zottegem, Belgium.

Introduction: The one-anastomosis gastric bypass (OAGB), first published by Dr Rutledge in 1997 is now a well-established procedure in the bariatric-metabolic armamentarium. This procedure based on a (single) loop gastro-jejunal anastomosis (the biliopancreatic limb or BPL) with a long narrow gastric pouch combines restriction with hypo-absorption. The biliopancreatic limb and in particular its length is held responsible for the degree of the hypo-absorptive effect but the most appropriate or "optimal" length of the BPL remains debatable.

Article Synopsis
  • Preoperative evaluation of axillary lymph nodes is important for determining treatment options in early breast cancer, and this study explored the effectiveness of shear wave elastography (SWE) for staging these lymph nodes.
  • The research involved 100 patients, measuring the stiffness of axillary lymph nodes with SWE, and established a cutoff velocity of 2.66 m/s to differentiate between malignant and benign nodes with notable sensitivity and specificity.
  • The findings suggest that SWE can enhance the assessment of suspicious axillary lymph nodes, providing a valuable tool for improving biopsy decisions and treatment planning in breast cancer patients.

Article Synopsis
  • Emergence delirium is a common complication in young children after anesthesia, especially in those undergoing ear, nose, and throat surgeries, and is assessed using the Pediatric Assessment of Emergence Delirium (PAED) scale.
  • In a study of 105 children aged 0-6, only 4.7% displayed severe emergence delirium, while 35% had some degree of it, with higher pain levels and lower blood glucose being significant risk factors.
  • The results suggest that maintaining higher blood glucose and managing pain could reduce the risk of emergence delirium in pediatric patients.

JCO .Although dose-dense adjuvant chemotherapy administered once every 2 weeks leads to superior outcomes compared with standard regimens once every 3 weeks, the observed improvement is largely limited to studies using the suboptimal paclitaxel schedule once every 3 weeks as control. PANTHER is an international phase III trial which compared sequential epirubicin/cyclophosphamide and docetaxel administered either once every 2 or once every 3 weeks, with tailored dosing at the dose-dense schedule according to hematologic toxicity.
