Misoprostol, a synthetic analogue of prostaglandin E1, is commonly used in gynecology, particularly in combination with mifepristone for elective abortion. Although its use is widespread, the potential teratogenic effects of misoprostol in cases of failed medical abortion have not been sufficiently explored in the medical literature. Severe congenital malformations, although rare, have been reported, raising concerns about the impact of this drug on fetal development.

Article Synopsis
  • Diagnosing Chronic Myeloid Leukemia (CML) during pregnancy poses significant challenges, especially in low- and middle-income countries, with limited management data available.
  • The study investigated outcomes for female patients diagnosed with CML-CP during pregnancy, analyzing medical records from 2002 to 2022, highlighting various pregnancy outcomes and treatment strategies.
  • Among the thirteen patients examined, outcomes included five elective terminations, five pre-term deliveries, and three full-term deliveries, with a median follow-up showing that most maintained major molecular remission, illustrating the need for coordinated care between haematologists and obstetricians.

Article Synopsis
  • The COVID-19 pandemic necessitated changes in how health care systems, like St. Paul's Hospital, handled surgical abortions, treating dilation and evacuation (D&E) procedures as emergency cases to reduce delays in care.
  • This retrospective study from April to July 2021 reviewed 19 second trimester D&E cases, focusing on their management under the new emergency COVID-19 pre-operative testing protocols.
  • The study found that patients were able to undergo D&E procedures on average within 21.83 hours of their initial evaluation, facilitating timely access to abortion services during a challenging period.

Article Synopsis
  • The study aimed to find a connection between parvovirus-B19 and Epstein-Barr virus (EBV) infections and levels of apoptosis (cell death) markers in placentas from spontaneous abortions compared to elective terminations.
  • Researchers analyzed 194 placental samples (152 from spontaneous abortions and 42 controls), using techniques like H&E staining and immunohistochemistry to assess viral presence and apoptotic markers.
  • Results showed significantly higher apoptosis levels in spontaneous abortion cases, suggesting viral infections could contribute to increased apoptosis and potentially trigger spontaneous abortions.

Glucagon-like peptide receptor agonists (GLP-1 RAs) are associated with delayed gastric emptying and may increase the risk of aspiration due to retained gastric contents. There are no guidelines on peri-endoscopic use of GLP-1 RAs, and real-world outcomes in an ambulatory setting remain unknown. This study reports real-world data from an ambulatory center associated with a large tertiary hospital.

The provision of abortion in Australia: service delivery as a bioethical concern.

Monash Bioeth Rev

December 2024

School of Medicine and Psychology, Australian National University, Canberra, Australia.

Despite significant progress in the legalization and decriminalization of abortion in Australia over the past decade or more recent research and government reports have made it clear that problems with the provision of services remain. This essay examines such issues and sets forth the view that such issues can and should be seen as (bio)ethical concerns. Whilst conscientious objection-the right to opt-out of provision on the basis of clear ethical reservations-is a legally and morally permissible stance that healthcare professionals can adopt, this does not mean those working in healthcare can simply elect not to be providers absent a clear ethical rationale.
