The rising prevalence of kratom use in the United States has led to increased encounters with individuals who consume kratom and those who develop Kratom Use Disorder (KUD) among healthcare professionals. This case series highlights the need for diverse treatment approaches tailored to the individual motivations for kratom consumption. Three cases are presented: one involving the progression from kratom use for energy to opioid-driven use with subsequent challenges in detoxification and transition to naltrexone necessitating buprenorphine low-dose induction for maintenance; another detailing successful inpatient detoxification using methadone and symptom-driven clonidine, followed by successful maintenance treatment with naltrexone; and a final case involving kratom consumption for pain and anxiety self-management by an individual without a history of addictive disorders, without meeting KUD criteria.

Objective: Gastric antral vascular ectasia (GAVE) is characterized by vascular ectasias accounting for 4% of nonvariceal upper gastrointestinal bleeds, which can range from occult bleeds to severe acute upper gastrointestinal bleeding. In turn, GAVE can lead to severe morbidity and recurrent hospitalization. Current endoscopic treatments for GAVE include argon plasma coagulation (APC), endoscopic band ligation (EBL), and radiofrequency ablation.

Background: Locally advanced pancreatic cancer (LAPC) comprises 40% of pancreatic cancer diagnoses and has a relatively poor prognosis. Trans-arterial micro perfusion (TAMP)-mediated chemotherapy delivery to the primary tumor is a novel approach worthy of investigation. The RR1 (dose escalation) and RR2 (observational) studies examined the safety and preliminary efficacy of TAMP-delivered gemcitabine for LAPC.

Objectives: Delays in acute stroke treatment lead to poor outcomes. Women can present with atypical stroke symptoms, are older at the time of stroke, and tend to be living alone, causing delays in pre-hospital diagnosis and seeking care. It is unclear if gender disparities in ED arrival and stroke assessment are compounded by gender differences after ED arrival.

Background: The obesity paradox in patients with coronary artery disease is well established, but the role of the metabolic syndrome associated with obesity is not well studied. Our study aims to evaluate the in-hospital outcomes of percutaneous coronary intervention (PCI) in metabolically healthy individuals with obesity (MHO) and metabolically unhealthy (MUHO) individuals with obesity over 65 years of age with acute coronary syndrome (ACS) between 2016 and 2020.

Methods: This was a retrospective and observational study.

Reports of parental dissatisfaction from incomplete or inconsistent information led to a quality improvement (QI) project to establish planned family conferences at 10 days and 1 month of life, for 50% of the medically complex neonates admitted to a neonatal intensive care unit within 1 year. A QI team instituted a system in which social workers scheduled family conferences and a neonatologist conducted the conferences. Team members tracked measures using statistical process control charts over 21 months.

Article Synopsis
  • This study examined early outcomes of a new medication, LCPT, compared to the traditional IR TAC in African American kidney transplant patients.
  • The study found that while LCPT led to higher tacrolimus levels shortly after transplant, the overall kidney function and key clinical outcomes after one year were similar to those of IR TAC.
  • The results suggest that LCPT can be safely used as a first-line treatment post-transplant without increasing complications like delayed graft function.

Background And Aims: Screening colonoscopy has significantly contributed to the reduction of the incidence of colorectal cancer (CRC) and its associated mortality, with adenoma detection rate (ADR) as the quality marker. To increase the ADR, various solutions have been proposed including the utilization of Artificial Intelligence (AI) and employing second observers during colonoscopies. In the interest of AI improving ADR independently, without a second observer, and the operational similarity between AI and second observer, this network meta-analysis aims at evaluating the effectiveness of AI, second observer, and a single observer in improving ADR.

Background: Neonates affected by isoimmune hemolytic disease (HDN) are at risk of developing severe hyperbilirubinemia.
