Tonsil explants as a human model to study vaccine responses.

Front Immunol

October 2024

Translational Immunology, Department of Biomedicine, University of Basel, Basel, Switzerland.

Introduction: Vaccination is one of the most effective infection prevention strategies. Viruses with high mutation rates -such as influenza- escape vaccine-induced immunity and represent significant challenges to vaccine design. Influenza vaccine strain selection is based on circulating strains and immunogenicity testing in animal models with limited predictive outcomes for vaccine effectiveness in humans.

The anatomical variability of the thoracic duct and the right lymphatic duct predisposes them to inadvertent damage following head and neck surgery thereby leading to chyle leak which is an uncommon complication with potentially significant associated morbidity. Although chyle leak is predominately associated with left-sided neck surgery, it also occurs as a complication of the right-sided neck dissection. Variable figures concerning chyle leakage after right-sided neck dissections were reported, ranging from 0 per cent to higher prevalences such as 14%, 24%, 33% and 60% of total cases of chyle leakages associated with neck surgery.

Although advanced technologies and surgical procedures are used, cholesteatoma is a disease with the possibility of recurrence. The aim of this study was to determine the long-term effect of sodium 2-mercaptoethane sulfonate (MESNA) on cholesteatoma surgery. Patients who underwent cholesteatoma surgery between January 2009 and July 2014 by the same surgeon were divided into 2 groups: those where MESNA was used and those where it was not.

Article Synopsis
  • The study focused on the side effects of using the neural integrity monitor electromyogram endotracheal tubes (NIM-EMG-ETT) during thyroidectomy and parathyroidectomy surgeries, evaluating 239 patients.
  • Among patients, only 0.8% experienced transient recurrent laryngeal nerve paralysis, with other complications primarily related to tube issues and ventilation failure.
  • No significant differences were found between silicone and PVC tubes, except for irregular EMG responses, which increased with longer surgical times; adherence to manufacturer guidelines is crucial to minimize risks during intubation and extubation.

Article Synopsis
  • A global collaborative study reviewed the management and outcomes of malignant skull base tumors, collecting data from 28 institutions involving 3,061 patients.
  • The majority of surgeries utilized an open approach (55%), with endoscopic (36%) and combined techniques (9.6%) being less common, and the overall 5-year survival rates were 65% for overall survival (OS) and 71.7% for disease-specific survival (DSS).
  • Factors such as older age, comorbidities, and the extent of tumor involvement were identified as independent predictors of survival, while adjuvant radiation therapy (RT) emerged as a protective factor in outcomes.

Objective: Vascular disorders and viral infections are the presumed etiologies of idiopathic sudden sensorineural hearing loss (ISSNHL) and acute sensorineural hearing loss, with no identifiable cause. However, no clinical test for estimating the extent of vascular involvement in ISSNHL has been reported despite its potential impact on prognosis and treatment. We investigated the correlation between the cardio-ankle vascular index (CAVI), which reflects arterial stiffness and elasticity, and hearing improvement to ascertain its usefulness as an additional indicator of ISSNHL prognosis and etiology.

Background: The presumed etiology of vestibular neuritis (VN), a sudden onset of spontaneous vertigo without auditory or cranial nerve symptoms, includes viral infections and vascular disorders. However, no clinical test for estimating vascular disorders in VN has been reported. Moreover, estimating the etiology of VN is important to predict the prognosis and select appropriate treatment.

Background: Little is known whether sublingual immunotherapy using Japanese cedar pollen extract (cedar SLIT) is effective for not only Japanese cedar pollinosis but also Japanese cypress pollinosis. We investigated the prevalence rate of Japanese cypress pollinosis, efficacy of cedar SLIT on cypress pollinosis and patients' wish to receive cypress SLIT.

Methods: We investigated a multi-center (31 institutions), cross-sectional survey using a self-administrated questionnaire with four questions for patients received cedar SLIT aged from 5 to 69 years old.

Unveiling the etiology of peritonsillar abscess using next generation sequencing.

Ann Clin Microbiol Antimicrob

November 2023

Department of Microbiology, Institute of Biomedicine and Translational Medicine, Faculty of Medicine, University of Tartu, Ravila 19, Tartu, 50411, Estonia.

Article Synopsis
  • - The study focused on identifying bacteria causing peritonsillar abscesses (PTA) using next generation sequencing (NGS) on samples from 91 patients.
  • - Researchers discovered over 400 bacteria types, with the most common being Streptococcus pyogenes, Fusobacterium necrophorum, and Fusobacterium nucleatum; S. pyogenes was the main species in most cases.
  • - The findings suggest that pus samples from the abscess are more informative for diagnosis than tonsil tissue, helping improve treatment for PTA patients.

Sci Rep

August 2021

Department of Microbiology, Institute of Biomedicine and Translational Medicine, University of Tartu, Tartu, Estonia.

Polyols are effective against caries-causing streptococci but the effect on oropharynx-derived pyogenic streptococci is not well characterised. We aimed to study the effect of erythritol (ERY) and xylitol (XYL) against Streptococcus pyogenes isolated from peritonsillar abscesses (PTA). We used 31 clinical isolates and 5 throat culture collection strains.
