Diagnostic Dilemma: Unusual Post-replacement Hip Pain Following Trauma Leading to Metallosis - A Case Report.

J Orthop Case Rep

December 2024

Department of Orthopaedics, AIIMS Raebareli, Raebareli, Uttar Pradesh, India.

Introduction: Metallosis, characterized by the collection of metallic debris in periarticular tissues, is primarily associated with metal-on-metal bearings in hip arthroplasty. This report presents a rare case of metallosis after metal-on-polyethylene (MoP) total hip arthroplasty (THA), highlighting diagnostic challenges and management strategies. Metallosis following MoP arthroplasty is extremely rare, with only a limited number of documented cases in the literature.

Background: Occupational lung diseases (OLDs) contribute a significant proportion to the global burden of pulmonary morbidities but are grossly misdiagnosed due to the relative lack of attribution given to occupational exposures. Obstructive lung diseases are known to be associated with long-lasting disability and loss of earning capacity (LOEC) among workers in industrial setups, thus reducing nationwide productivity.

Objective: In this context, the study aimed to find out the pattern of OLD and factors associated with the severity of it among patients in a tertiary care hospital.

Monaldi Arch Chest Dis

July 2024

Department of Pulmonary, Critical Care and Sleep Medicine, ESI-PGIMSR, Basaidarapur, New Delhi.

Article Synopsis
 • * The study involved primarily male patients (78.26%), with a significant portion being current or former smokers (75%), and a majority suffering from non-small cell lung cancer (86.95%).
 • * Findings revealed that the overall mean survival was 12.7 months, with better survival rates in stage IV due to targeted therapy and immunotherapy, highlighting the importance of molecular analysis for optimizing treatment in eligible patients.

Article Synopsis
 • This study evaluated therapeutic drug monitoring of key anti-tubercular medications, rifampicin and isoniazid, among 75 patients with drug-sensitive tuberculosis who were not responding to standard treatment.
 • An intervention was applied to patients with low drug levels, either by increasing dosages or adding second-line drugs to their regimen.
 • Results showed that 85.5% of patients had low drug levels, and those who received additional treatment had a favorable outcome in 59.6% of cases, suggesting that addressing low drug levels can improve treatment efficacy.

Introduction: Necrotizing fasciitis (NF) is an expeditiously escalating inflammatory infection of the fascia, with secondary necrosis of the subcutaneous tissues. To aid diagnosis, the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) scoring system was devised to stratify the likelihood of infection in patients on presentation.

Aim: The study aimed to investigate and quantify the relationships between the LRINEC scoring system and the outcomes such as the need for amputation, length of hospital stay, and mortality in patients with necrotizing fasciitis.

Purpose: To analyze whether the arthroscopic Bankart repair using a knotless suture anchor has a better functional outcome than the conventional knot-tying Bankart repair.

Methods: A comprehensive literature search was done in the PubMed, Scopus, Embase, and Cochrane databases in May 2023. Studies comparing the clinical outcome of Bankart repair using knotless and knot-tying techniques were included in the study.

International Society for the Study of Vascular Anomalies classification defines Congenital Limb Overgrowth Vascular Syndromes (CLOS) as a subset of vascular syndromes with other abnormalities that present with unilateral limb overgrowth. It includes Klippel-Trenaunay Syndrome, Parkes-Weber Syndrome, CLOVES (Congenital Lipomatous Overgrowth, Vascular Malformations, Epidermal Nevi, Spinal/Skeletal Anomalies/Scoliosis) Syndrome, Proteus Syndrome, PTEN Hamartomatous Syndrome, and Fibroadipose Vascular Anomaly. Due to their rare and complex nature, a multidisciplinary approach to diagnosis and treatment is required.

Article Synopsis
 • Plantar fasciitis, a common and painful foot condition, has various treatments, and this study aimed to compare the effectiveness of autologous platelet-rich plasma (PRP) injections versus corticosteroid injections.
 • The study involved 70 patients with chronic plantar fasciitis, who were randomly assigned to receive either PRP or corticosteroid injections, with pain and function assessed before and at multiple follow-ups after the injections.
 • Results indicated that the PRP group had significantly better pain reduction and functional improvements compared to the corticosteroid group after one, three, and six months, suggesting that PRP is a more effective long-term treatment for this condition.

Certain clinical features, like changes in presentation, the presence of neurological signs, and a poor response to treatment, mandate an organic evaluation of psychiatric illnesses. In this case series, four cases with established mental illnesses who presented with psychiatric symptoms, on evaluation by neuroimaging, were found to have neurological disorders like neurocysticercosis, sub-arachnoid hemorrhage, sub-dural hemorrhage, and lacunar infarction. They were managed with a combination of medical and surgical treatment and psychotropics, and no relapse of symptoms was noted at follow-up.

Background: Surgical site infection (SSI) is the third most commonly reported nosocomial infection, accounting for 10%-40% of all nosocomial infections and is a major cause of postoperative morbidity. Knowledge of factors related to SSI can help in reducing its incidence and related morbidity, which in many studies is shown to account for 38% of all infections in surgical patients. Lack of extending nosocomial infection surveillance programme and prevention measures in countries like India is viewed as a major challenge for the future.

Breast cancer stem cells generate immune-suppressive T regulatory cells by secreting TGFβ to evade immune-elimination.

Discov Oncol

December 2023

Division of Molecular Medicine, Bose Institute, P-1/12, Calcutta Improvement Trust Scheme VII M, Kolkata, 700054, India.

Article Synopsis
 • Cancer stem cells (CSCs) are crucial for tumor development and can evade the immune system by converting effector T cells into immunosuppressive T-regulatory cells (Tregs).
 • The study found that even a small number of CSCs could generate Tregs from CD4 T cells without direct contact, suppressing the antitumor activity of immune cells.
 • Additionally, CSCs are resistant to chemotherapy and produce high levels of the cytokine TGFβ, which promotes Treg formation, contributing to ongoing tumor growth and potential relapse after treatment.

Expression of stathmin in oral squamous cell carcinoma and its correlation with tumour proliferation.

J Oral Maxillofac Pathol

March 2023

Department of Pathology, ESI-PGIMSR Manicktala, Kolkata, West Bengal, India.

Background: Stathmin is a member of microtubule-associated protein. Inhibition of Stathmin expression can interfere with tumour progression and also alter the sensitivity of tumour cells to microtubule-targeting agents. Thus, it could be a potential therapeutic target for planning new treatment strategies.
