Article Synopsis
  • The study explored the phenomenon of opioid-induced somatic activation (OISA), which is the stimulation effect of opioids often overlooked compared to their known sedative effects.
  • Nearly half of the 189 patients reviewed experienced somatic activation from opioids, with oxycodone showing a higher likelihood of this effect compared to morphine sulfate.
  • The findings highlight the need for future research on OISA to better understand its potential risks and benefits, especially given the current concerns surrounding opioid use.

Background/aims: Esophageal baseline impedance (BI) can be extracted from pH-impedance tracings as mean nocturnal baseline impedance (MNBI), and from high-resolution impedance manometry (HRIM), but it is unknown if values are similar between acquisition methods across HRIM manufacturers. We aim to assess correlations between MNBI and BI from HRIM (BI-HRIM) from 2 HRIM manufacturers in the setting of physiologic acid exposure time (AET).

Methods: HRIM and pH-impedance monitoring demonstrating physiologic AET (< 4%) off proton pump inhibitors were required.

Purpose: The Enhancing Quality of Prescribing Practices for Older Veterans Discharged From the Emergency Department (EQUIPPED) program is a quality improvement initiative that combines education, clinical decision support (ie, geriatric pharmacy order sets), and in-person academic detailing coupled with audit and feedback in an effort to improve appropriate prescribing to older veterans discharged from the emergency department. Although the EQUIPPED program is effective at reducing the prescribing of potentially inappropriate medications, the reliance on in-person academic detailing may be a limitation for broader dissemination. The EQUIPPED dashboard is a passive yet continuous audit and feedback mechanism developed to potentially replace the in-person academic detailing of the traditional EQUIPPED program.

Article Synopsis
  • Anorexia nervosa significantly increases the risk of sudden cardiac death, but the underlying causes, particularly concerning QT interval prolongation, remain unclear.
  • Researchers analyzed ECG data from over 1,000 patients with various eating disorders to assess the population-mean heart-rate-corrected QT interval (QTc).
  • The study found that overall QTc values were within the normal range, with notable prolongation only occurring in patients with additional factors like hypokalemia and specific medications, indicating that QTc prolongation isn't a fundamental characteristic of eating disorders.
