While tibiotalocalcaneal arthrodesis with internal fixation may be a suitable method of salvage for the treatment of a failed tibiotalar arthrodesis in selected patients, ring external fixation has been reported as an alternative. [Diagnosis and treatment of osteochondral lesions of the talus].

Delayed diagnosis of osteochondral lesions of the talus is common; acute osteochondral injuries to the talar dome frequently go unrecognized. While acute lesions may be repaired, typically the focus of operative treatment for osteochondral lesions is on: (a) débridement and drilling/microfracture (leading to filling of the defect with fibrocartilage) or (b) salvage procedures, including osteochondral autograft transfer system and autologous chondrocyte implantation (potentially leading to resurfacing of the defect with hyaline cartilage). In this article, we review current trends in preoperative assessment, defining indications and surgical management of osteochondral lesions of the talus.

Photodynamic therapy (PDT)--also called phototherapy, photoradiation therapy, or photochemotherapy--is a relatively new treatment option being used to treat certain types of cancer, including endobronchial and esophageal cancers. PDT is a two-step process that involves administration of a photosensitizing agent followed by exposure to non thermal laser light. Intensive patient education and support are required during PDT.
