Hypertriglyceridemic pancreatitis (HTGP) is well-known but it is extremely rare, especially in younger patients. The main treatment modalities for HTGP are apheresis and intravenous insulin. However, apheresis in severe HTGP is not well established and the efficacy of the treatment is lacking.

Objective: To identify impacts of patient and family engagement in child health research on the research process, research teams, and patient and family partners.

Study Design: A scoping review was conducted using the MEDLINE, Embase, Cumulative Index to Nursing and Allied Health Literature, PsycINFO, and Web of Science databases. English-language studies were included if they described ≥1 impact of patient and family engagement on child health research (age <18 years), researchers, or patient and family partners.

Understanding Barriers and Facilitators to Maternal Immunization: A Systematic Narrative Synthesis of the Published Literature.

Matern Child Health J

November 2022

Division of Infectious Diseases, Children's Hospital of Philadelphia, 3401 Civic Center Blvd., Philadelphia, PA, 19104, USA.

Introduction: Vaccines are being developed against Group B Streptococcus and respiratory syncytial virus. These vaccines are designed to be given to pregnant women to protect infants; thus, their success depends on uptake in this population. Maternal immunization programs have struggled to achieve target coverage rates.

Article Synopsis
  • Most evidence for treating newborns in delivery rooms is considered low quality due to insufficient clinical trials.
  • Trials often suffer from issues like small participant numbers and lack of blinding, which affects the reliability of results.
  • The authors suggest that improving the design of future trials could help gather better data and advance clinical practices in newborn care.

DEPDC5 (DEP Domain-Containing Protein 5) encodes an inhibitory component of the mammalian target of rapamycin (mTOR) pathway and is commonly implicated in sporadic and familial focal epilepsies, both non-lesional and in association with focal cortical dysplasia. Germline pathogenic variants are typically heterozygous and inactivating. We describe a novel phenotype caused by germline biallelic missense variants in DEPDC5.

Chronic obstructive pulmonary disease (COPD) patients frequently suffer from multiple comorbidities, resulting in poor outcomes for these patients. Diabetes is observed at a higher frequency in COPD patients than in the general population. Both type 1 and 2 diabetes mellitus are associated with pulmonary complications, and similar therapeutic strategies are proposed to treat these conditions.

The neuro-ocular effects of long-duration spaceflight have been termed Spaceflight Associated Neuro-Ocular Syndrome (SANS) and are a potential challenge for future, human space exploration. The underlying pathogenesis of SANS remains ill-defined, but several emerging translational applications of terrestrial head-mounted, visual assessment technology and machine learning frameworks are being studied for potential use in SANS. To develop such technology requires close consideration of the spaceflight environment which is limited in medical resources and imaging modalities.

Article Synopsis
  • This study investigates how antiretroviral therapy (ART) affects bone mineral density (BMD) in older individuals living with HIV, focusing on men over 50 and postmenopausal women.
  • Researchers examined a total of 247 participants and found that exposure to specific ART medications, particularly tenofovir disoproxil fumarate (TDF) and protease inhibitors (PIs), significantly increased the risk of low BMD, especially in the femoral neck and lumbar spine.
  • The findings suggest that the relationship between ART use and low BMD remains strong, regardless of adjustments for bone turnover and renal function markers.

Biopsychosocial approach to tendinopathy.

BMJ Open Sport Exerc Med

August 2022

Institute of Infection, Immunity and Inflammation College of Medicine, Veterinary and Life Sciences, University of Glasgow, Glasgow, UK.

Tendinopathy describes a spectrum of changes that occur in damaged tendons, leading to pain and reduced function that remains extremely challenging for all clinicians. There is an increasing awareness of the influence that psychological and psychosocial components, such as self-efficacy and fear-avoidance, have on rehabilitation outcomes in musculoskeletal medicine. Although it is widely accepted that psychological/psychosocial factors exist in tendinopathy, there is currently a distinct lack of trials measuring how these factors affect clinical outcomes.

Unlabelled: Inborn errors of metabolism are an individually rare but collectively significant cause of mortality and morbidity in the neonatal period. They are identified by either newborn screening programmes or clinician-initiated targeted biochemical screening. This study examines the relative contribution of these two methods to the identification of inborn errors of metabolism and describes the incidence of these conditions in a large, tertiary, neonatal unit.

Article Synopsis
  • * A subgroup analysis of data from a phase 3 study involving 134 adults revealed no significant differences in treatment benefits among 14 clinically relevant subgroups, including variations in sex, pain levels, and physical function.
  • * Overall, burosumab consistently showed better outcomes compared to placebo, suggesting it is beneficial for all symptomatic adults with active XLH, although some treatment effects were small and not statistically significant across subgroups. *

Background: With a significant increase in utilisation of computed tomography (CT), inappropriate imaging is a significant concern. Manual justification audits of radiology referrals are time-consuming and require financial resources. We aimed to retrospectively audit justification of brain CT referrals by applying natural language processing and traditional machine learning (ML) techniques to predict their justification based on the audit outcomes.

Introduction: Secure forensic mental health services are low volume, high cost services. They offer care and treatment to mentally disordered offenders who pose a high risk of serious violence to others. It is therefore incumbent on these services to systematically evaluate the outcomes of the care and treatment they deliver to ensure patient benefit in multiple domains.

Background: The ongoing epidemic of prescription opiate abuse is one of the most pressing health issues in the United States today. Consequently, analgesic adjuncts, such as multimodal drug regimens and regional anesthetic blocks (like transversus abdominis plane (TAP) block), have been introduced to the perioperative period in hopes of decreasing postoperative opiate use. However, the effect of these interventions on intraoperative opiate use has not been examined.

With the advent of bronchoscopic lung-volume reduction (BLVR), this minimally invasive technique represents a new and effective way of managing the debilitating symptoms associated with severe centrilobular emphysema. Despite its vast potential in the management of this disease, there are still several potential risk factors associated with the procedure that may predispose the patient to increased morbidity. Our patient received four endobronchial valves in the right-upper lobe (RUL) and right-middle lobe (RML).

Article Synopsis
  • - Women's involvement in field collision sports is on the rise, prompting a scoping review of existing research to better support coaches, policymakers, and academics in understanding this developing field.
  • - A total of 43 studies were analyzed from 2,318 records, focusing mainly on physical demands (24 studies), then technical (18), tactical (8), and preparatory strategies (1), revealing a holistic approach needed for improving performance and reducing injuries in female athletes.
  • - The review highlights a significant gender data gap, indicating that existing male-focused training guidelines cannot be effectively applied to female athletes; thus, there's an urgent need to enhance research visibility and data on women in these sports.
