GPT-4: a new era of artificial intelligence in medicine.

Ir J Med Sci

December 2023

Human-Machine Perception Laboratory, Department of Computer Science and Engineering, University of Nevada, Reno, NV, USA.

GPT-4 is the latest version of ChatGPT which is reported by OpenAI to have greater problem-solving abilities and an even broader knowledge base. We examined GPT-4's ability to inform us about the latest literature in a given area, and to write a discharge summary for a patient following an uncomplicated surgery and its latest image analysis feature which was reported to be able to identify objects in photos. All things considered, GPT-4 has the potential to help drive medical innovation, from aiding with patient discharge notes, summarizing recent clinical trials, providing information on ethical guidelines, and much more.

Wilson's disease is an autosomal recessive disorder arising from pathogenic variants in the tp7b gene on chromosome 13. The defective translated ATPase copper (Cu) transport protein produced leads to Cu accumulation, initially affecting the liver but eventually affecting other cells. It is just over 20 years since the last Best Practice on this topic in this journal.

Introduction: The gut microbiota develops from birth and matures significantly during the first 24 months of life, playing a major role in infant health and development. The composition of the gut microbiota is influenced by several factors including mode of delivery, gestational age, feed type and treatment with antibiotics. Alterations in the pattern of gut microbiota development and composition can be associated with illness and compromised health outcomes.

Division of Pulmonary, Allergy, and Critical Care Medicine, Department of Medicine, University of Pittsburgh, NW 628 MUH, 3459 Fifth Avenue, Pittsburgh, PA, 15213, USA. Electronic address:

There is no consensus on the best model of care for individuals with CF to manage the non-pulmonary complications that persist after lung transplant. The CF Foundation virtually convened a group of international experts in CF and lung-transplant care. The committee reviewed literature and shared the post-lung transplant model of care practiced by their programs.

Centile reference chart for resting metabolic rate through the life course.

Arch Dis Child

July 2023

Stable Isotopes Laboratory, Nutritional Biomarker Laboratory, MRC Epidemiology Unit and Wellcome-MRC Institute of Metabolic Science, Metabolic Research Laboratories, University of Cambridge, Cambridge, UK.

Objective: Reference centile charts are widely used for the assessment of growth and have progressed from describing height and weight to include body composition variables such as fat and lean mass. Here, we present centile charts for an index of resting energy expenditure (REE) or metabolic rate, adjusted for lean mass versus age, including both children and adults across the life course.

Design, Participants And Intervention: Measurements of REE by indirect calorimetry and body composition using dual-energy X-ray absorptiometry were made in 411 healthy children and adults (age range 6-64 years) and serially in a patient with resistance to thyroid hormone α (RTHα) between age 15 and 21 years during thyroxine therapy.

Benefit of burosumab in adults with X-linked hypophosphataemia (XLH) is maintained with long-term treatment.

RMD Open

February 2023

Nuffield Department of Orthopaedics, Rheumatology and Musculoskeletal Sciences, National Institute for Health Research Oxford Biomedical Research Centre, University of Oxford, Oxford, UK.

Article Synopsis
  • The study examines the effects of continuing burosumab treatment on adults with X-linked hypophosphataemia after a 96-week phase 3 study, focusing on lab tests, patient-reported outcomes, and walking ability.
  • Results showed sustained improvements in serum phosphate levels, vitamin D levels, and overall physical health during the 48-week extension.
  • An analysis indicated that stopping burosumab led to a loss of benefits, highlighting that ongoing treatment is essential for maintaining clinical improvements.

Objective: This study evaluated whether adding sodium-glucose cotransporter-2 inhibitors (SGLT2i) and/or glucagon-like peptide-1 receptor agonists (GLP1-RA) to insulin reduced weight and glycemia in people with type 1 diabetes.

Methods: This retrospective analysis of electronic health records evaluated 296 people with type 1 diabetes over 12 months after medications were first prescribed. Four groups were defined: control n = 80, SGLT2i n = 94, GLP1-RA n = 82, and combination of drugs (Combo) n = 40.

Parturients with vascular Ehlers-Danlos syndrome are at particular risk of hemorrhage, and there is little evidence to guide prevention or management of hemorrhage in these patients. We present the case of a patient with vascular Ehlers-Danlos syndrome who had a cesarean delivery complicated by an intraoperative hemorrhage. Administration of desmopressin and tranexamic acid appeared to be of marked benefit in achieving hemostasis.

Article Synopsis
  • Hidradenitis suppurativa (HS) is a complex skin condition that's tough to treat, with adalimumab being the only approved therapy and only helping about 50% of patients.
  • The study investigated differences in gene expression between affected (lesional) and unaffected (nonlesional) skin in HS patients before and after 12 weeks of adalimumab treatment, aiming to uncover potential biomarkers for treatment response.
  • Findings suggest significant immune system involvement in HS, with certain genes indicating treatment response and disease severity, pointing towards the need for personalized treatment strategies targeting specific immune pathways.

Transfer learning as an AI-based solution to address limited datasets in space medicine.

Life Sci Space Res (Amst)

February 2023

Human-Machine Perception Laboratory, Department of Computer Science and Engineering, University of Nevada, Reno, Reno, Nevada, United States.

The advent of artificial intelligence (AI) has a promising role in the future long-duration spaceflight missions. Traditional AI algorithms rely on training and testing data from the same domain. However, astronaut medical data is naturally limited to a small sample size and often difficult to collect, leading to extremely limited datasets.
