Med J Armed Forces India

December 2022

Consultant (Ophthalmology) & Brig I/C Adm, Command Hospital (Eastern Command), Kolkata, India.

Background: To study the correlation of different variables of Vogt striae to other attributes of keratoconus and to suggest a new grading system for Vogt striae to exactly represent the configuration and extent in each individual keratoconic cornea.

Methods: The study was an observational and data review study. A total of 386 eyes of 207 keratoconus patients were screened from February 2016 to August 2018.

Systemic coronavirus disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus has had several ocular consequences. Many vaccines have been developed against the disease, with adverse events being reported as well. Various ocular adverse events secondary to coronavirus disease 2019 (COVID-19) vaccines have also featured in literature in recent times.

Purpose: To evaluate the impact of the inverted internal limiting membrane (ILM)-flap technique on the visual outcome and anatomical recovery for small (<250 μ), medium (<400 μ), and large (>400 μ) macular holes (MHs).

Methods: Retrospective study included consecutive idiopathic MH cases operated on using the inverted ILM-flap technique. Clinical data were retrieved from electronic medical records (EMRs), surgical videos, and optical coherence tomography (OCT) machines.

Guava Leaf Extract Suppresses Fructose Mediated Non-Alcoholic Fatty Liver Disease in Growing Rats.

Diabetes Metab Syndr Obes

September 2022

Department of Pharmacology, Delhi Institute of Pharmaceutical Science and Research, Delhi Pharmaceutical Sciences and Research University, New Delhi, 110017, India.

Purpose: Fructose is highly lipogenic, and its unhindered ingestion by children and adolescents is understood to induce hypertriglyceridemia and non-alcoholic fatty liver disease (ped-NAFLD) that is till date managed symptomatically or surgically. The aim of the present study was to investigate the potential of hydroethanolic extract of leaves of Guava (PG-HM) to suppress the alterations in the hepatic molecular signals due to unrestricted fructose (15%) drinking by growing rats.

Methods: Weaned rats (4 weeks old) in control groups had access to fructose drinking solution (15%) for four (4FDR) or eight (8FDR) weeks, ie, till puberty or early adulthood, respectively, while treatment groups (4PGR, 8PGR) additionally received PG-HM (500 mg/kg, po).

Purpose: To analyze the pattern of bacterial pathogens causing infective keratitis and their resistance to the recommended antibiotics over six years.

Methods: It was a retrospective study of 9,357 cases of bacterial keratitis from January 2015 to December 2020, at a tertiary care ophthalmic center. A total of 9,547 corneal specimens were obtained from the study subjects.

Morning glory disc (MGD) is known to develop secondary maculopathy and vision loss. We followed a 7-year-old girl with MGD in right eye from 2010 to 2021. Her best-corrected Snellen visual acuity (BCVA) was 20/20 in both eyes till 2017.

Article Synopsis
  • - Acute lymphoblastic leukaemia (ALL) is the most prevalent cancer in children, with a high cure rate of 70%-80% through modern chemotherapy, but 20% may relapse, typically in the bone marrow.
  • - Isolated ocular involvement as a first sign of relapse is quite rare, occurring in less than 2.2% of ALL cases, but can indicate a serious visual emergency linked to central nervous system relapse.
  • - The case emphasizes the necessity of regular eye screenings for ALL patients, even during maintenance treatment, and the need for quick action to treat isolated optic nerve issues.

Article Synopsis
  • The study aims to investigate immune checkpoint markers (PD-1, PD-L1, and CTLA-4) in the tumor microenvironment of primary and chemoreduced retinoblastoma, linking them to patient outcomes.
  • A total of 262 cases were analyzed, revealing differences in marker expression between primary and chemoreduced retinoblastoma; notably, PD-1 was higher in chemoreduced cases, while PD-L1 was more prevalent in primary cases.
  • The findings suggest that these immune markers have prognostic significance, indicating possible therapeutic targets for chemoresistant retinoblastoma, with specific markers correlating to the aggressiveness of the disease.

Purpose: To evaluate the effects of topical nepafenac in patients undergoing vitreoretinal surgery.

Methods: One hundred and twenty eyes of 120 patients undergoing vitreoretinal surgery were randomized to receive either topical nepafenac 0.1% (60 eyes) or placebo (60 eyes) in this investigator-masked, randomized, comparative case series.

Circumscribed choroidal hemangiomas are rare ophthalmic entities that cause diminution in vision due to accumulation of subretinal and/or intraretinal fluid in the macular area. Various treatment options ranging from conventional laser to photodynamic therapy have been employed to destroy the tumor and reduce the exudation; however, either the inability to penetrate through the exudative fluid or the collateral retinal damage induced by these treatment modalities make them unsuitable for lesions within the macula. We evaluated the role of intravitreal bevacizumab, a pan-vascular endothelial growth factor (VEGF) inhibitor, in reducing the sub- and intraretinal fluid in three patients with circumscribed choroidal hemangiomas.

Dye-assisted internal limiting membrane (ILM) peeling and gas tamponade is the surgery of choice for idiopathic macular holes. Indocyanine green and trypan blue have been extensively used to stain the ILM. However, the retinal toxicity of indocyanine green and non-uniform staining with trypan blue has necessitated development of newer vital dyes.

Background: Herpes Simplex Virus (HSV) keratitis is a leading cause of corneal blindness. Definitive laboratory diagnosis is essential for timely management. Collection of corneal scrapings in patients with advanced epithelial keratitis and corneal thinning poses perforation risks; tear fluid is a feasible and convenient alternative but has not been widely evaluated for HSV detection.

Typical ischemic retinal microvascular changes after bone marrow transplantation is a well-established entity and is seen in up to 7% of allogenic transplant recipients. Such cases are characterized by symmetric presence of cotton wool spots, retinal hemorrhages, and multiple microaneurysms and have a good visual prognosis. In this report we describe a case of atypical ischemic maculopathy with poor visual recovery after bone marrow transplantation.

Vitreous hemorrhage is an uncommon cause of decreased vision in children. We report a 3-year-old child with bilateral vitreous hemorrhage secondary to an isolated platelet factor 3 (PF-3) availability defect. Prophylactic infusion of platelet-rich plasma followed by pars plana vitrectomy in both eyes resulted in an excellent surgical and visual outcome.

Purpose: The purpose of this study is to correlate optic nerve head topography with fluorescein angiography of the optic nerve head in patients with primary open-angle glaucoma (POAG), chronic primary angle closure glaucoma (CPACG), and normal controls.

Methods: This was an institution-based, cross-sectional, case-control study of 30 consecutive patients each with POAG or CPACG, which were compared with 30 age- and sex-matched controls. The fluorescein angiograms undertaken in one eye of each of the 90 subjects were then analyzed both qualitatively and quantitatively.
