Hemorrhage risk score and peripartum quantified blood loss.

Proc (Bayl Univ Med Cent)

November 2024

Department of Obstetrics and Gynecology, NYC/H+H - South Brooklyn Health, Brooklyn, New York, USA.

Background: Risk assessment tools are used by clinicians to predict which patients might have excessive bleeding. We studied the association between a peripartum hemorrhage risk assessment score using peripartum quantified blood loss (QBL) among those with vaginal deliveries who are often at lower risk for peripartum hemorrhage.

Methods: In this retrospective analysis of 1221 patients with term vaginal deliveries at a public New York City hospital, the Association of Women's Health, Obstetric and Neonatal Nurses (AWOHNN) risk assessment tool was used to categorize patients as low risk, medium risk, or high risk for postpartum hemorrhage.

We sought to conduct a systematic review of the literature and a meta-analysis of post-operative outcomes following TKA with an all-polyethylene tibial (APT) or a metal-backed tibial component (MBT). A systematic review of the literature was conducted to identify all studies comparing APT and MBT for TKA based upon rates of complications, revisions, reoperation, and patient-reported outcomes. Data was pooled and relative risk of each outcome measure at short (< 5 year), mid (5-10 year), and long (> 10 year) follow-up was calculated.

A community-based analysis of security practices regarding over-the-counter skincare products in nationwide retail drug stores.

Arch Dermatol Res

December 2024

Dermatology Service, Veterans Affairs New York Harbor Healthcare System Brooklyn Campus, Brooklyn, NY, USA.

Access to over-the-counter (OTC) skincare products is critical for managing a wide range of dermatologic conditions, yet recent trends in urban retail stores may jeopardize this accessibility and exacerbate healthcare disparities. However, the emerging trend of securing skincare products behind security glass in urban retail environments poses significant risks to healthcare equity and accessibility. Herein, we aim to quantify the prevalence of skincare products behind security glass in pharmacies within a 25-mile radius of Times Square, New York City.

Article Synopsis
  • - The study investigates the optimal timing for starting direct oral anticoagulants (DOACs) for stroke prevention in patients with atrial fibrillation and acute ischaemic stroke, comparing early initiation (around 3.5 days) to later initiation (about 5.7 days) due to concerns about bleeding risks.
  • - An analysis of 11 studies, including 13,020 participants, found that early initiation of DOACs resulted in significantly lower rates of recurrent ischaemic stroke and intracranial hemorrhage compared to the late group.
  • - While key outcomes like major hemorrhage and all-cause mortality showed no significant differences between the groups, the variability in the study designs and limited number of RCT

Most physicians appreciate that practicing medicine is a commitment to continuous learning. However, "learning" can be mistakenly understood as simply the acquisition of facts and new knowledge. But learning also necessitates the constant re-examination and challenging of one's existing body of knowledge, as misinformation persists when one's beliefs are not challenged or questioned in the light of new information.

Background: Peripheral nerve stimulation (PNS) has been used for over 50 years to treat chronic pain by delivering electrical pulses through small electrodes placed near targeted peripheral nerves those outside the brain and spinal cord. Early PNS systems often required invasive neurosurgical procedures. However, since 2015, the Food and Drug Administration (FDA) approved percutaneously implanted PNS leads and neurostimulators  offering a much less invasive, non-opioid option for managing recalcitrant chronic pain.
