Article Synopsis
  • The study examines the effects of low vs. high vancomycin trough levels on clinical outcomes in adults with sepsis or gram-positive infections, moving away from the previously recommended dosage guidelines.
  • A systematic review and meta-analysis of 14 studies involving over 5,200 patients showed no significant links between vancomycin levels and various clinical outcomes like microbial clearance and ICU stay.
  • However, lower trough levels appeared to trend towards reduced treatment failure and were significantly associated with lower all-cause mortality rates.

Background And Hypothesis: Oxidative stress pathways may play a role in schizophrenia through direct neuropathic actions, microglial activation, inflammation, and by interfering with NMDA neurotransmission. -acetylcysteine (NAC) has been shown to improve negative symptoms of schizophrenia, however, results from trials of other compounds targeting NMDA neurotransmission have been mixed. This may reflect poor target engagement but also that risk mechanisms act in parallel.

Background: Catheter ablation and antiarrhythmic drug therapy are utilized for rhythm control in atrial fibrillation (AF), but their comparative effectiveness, especially with contemporary treatment modalities, remains undefined. We conducted a systematic review and meta-analysis contrasting current ablation techniques against antiarrhythmic medications for AF.

Methods: We searched PubMed, SCOPUS, Cochrane CENTRAL, and Web of Science until November 2023 for randomized trials comparing AF catheter ablation with antiarrhythmics, against antiarrhythmic drug therapy alone, reporting outcomes for > 6 months.

Background: Psychosis treatment guidelines recommend cognitive behaviour therapy (CBT) and family intervention (FI), for all patients with first episode psychosis (FEP), though guidance borrows heavily from literature in adults from high income countries. To our knowledge, there are few randomized controlled trials (RCTs) examining the comparative effect of these commonly endorsed psychosocial interventions in individuals with early psychosis from high-income countries and no such trials from low and middle-income countries (LMICs). The present study aims to confirm the clinical-efficacy and cost-effectiveness of delivering culturally adapted CBT (CaCBT) and culturally adapted FI (CulFI) to individuals with FEP in Pakistan.

The purpose fo this prospective study was to find the impact of primary tumor size (Ts), standardized uptake values (SUVmax) of primary tumor, and the most avid neck node on disease recurrence in patients with head and neck oropharyngeal squamous cell carcinoma (HNOP-SCC).  We included patients with HNOP-SCC (without distant metastasis-M0 disease) who had pre- and post-treatment F-18 fluorodeoxyglucose positron emission tomography/computed tomography ( FDG PET/CT) using strict standardized imaging protocol from 2017 to 2019. Based on follow-up ( FDG PET/CT) findings, patients were categorized as disease free (no or minimal FDG uptake ≤ background over surgical bed and no distant metastasis) and disease recurrence ( FDG uptake > background over surgical bed with or without nodal and/or distant metastasis).

  • Leadless pacemakers (LPM) aim to reduce complications commonly associated with conventional pacemakers (CPM), such as lead dislodgement and pocket infections.
  • A study analyzing 204 cases found that complications with CPM were significantly higher than those with LPM in areas like electrode dislodgement (56% vs 7%), pocket site infection (16% vs 3.4%), and lead fractures (8% vs 0%).
  • Although LPMs had a non-significant higher risk of pericardial effusion (8% vs 4%), they generally demonstrate a better safety profile compared to CPMs.
