Background: Lower gastrointestinal perforation (LGP) is an acute abdominal condition associated with a high mortality rate. Timely and accurate diagnosis is crucial. Nevertheless, a diagnostic delay has been estimated to occur in approximately one-third of the cases, and the factors contributing to this delay are yet to be clearly understood.

Purpose: This study aimed to explore the perspectives of residency program directors in Japan regarding overtime duty hours and the balance between clinical training and self-improvement activities. This study explores the impact of work-hour regulations on resident well-being and training quality, contributing to global discourse on medical education reform.

Participants And Methods: A cross-sectional survey was distributed to 701 residency training hospitals across Japan to investigate their readiness for new duty-hour limits under the Medical Care Act, which categorizes working hours into Level A (960 hours/year), Level B (1440 hours/year), and Level C-1 (1920 hours/year).

Background: The general medicine in-training examination (GM-ITE) assesses physicians' clinical knowledge. This study expanded on findings from a previous pilot study to assess the relationship between general medicine in-training examination (GM-ITE) scores and the diagnostic skills of resident physicians in Japan by employing an innovative clinical simulation video (CSV-IE).

Methods: This multicenter cross-sectional study included 4,677 resident physicians who took the GMITE between January 17 and 30, 2023.

Multi-lineage differentiating stress-enduring (Muse) cells, identified as pluripotent surface marker SSEA-3(+) cells, are stress tolerant endogenous pluripotent-like stem cells, and are involved in tissue repair. However, the significance of Muse cells in acute myocarditis has not been evaluated. In the present study, we counted Muse cells/area in biopsied myocardial tissue samples from 17 patients with fulminant myocarditis, and 6 with non-inflammatory myocardial disease as controls.

Article Synopsis
  • The study aimed to analyze clinical features of patients with interstitial cystitis (IC) and bladder pain syndrome (BPS) by using a patient registry in Japan, classifying patients based on cystoscopic findings.
  • A total of 529 patients from 14 university hospitals were included, showing that 66.5% had IC, with these patients being older and predominantly female, and with more comorbid autoimmune diseases.
  • Results indicated that patients with IC experienced worse symptoms, quality of life, and bladder function compared to those with BPS, with specific bladder measurements significantly correlating with pain levels in IC patients.

Introduction: Diagnostic errors in emergency departments (ED) are a significant concern and exacerbated by cognitive biases during patient handoffs. The timing and accuracy of disclosing working diagnoses during these handoffs potentially influence diagnostic decisions, yet empirical evidence remains limited.

Materials And Methods: This parallel, quasi-experimental study involved 40 interns from Japanese teaching hospitals, randomly assigned to control or intervention groups.

Introduction: Benzodiazepine (BZD) dependence and withdrawal are significant public health concerns, affecting 15-30% of regular users within 4-6 weeks of administration. Rapid BZD tapering protocols in acute care settings can mitigate withdrawal symptoms and facilitate rehabilitation, yet are challenging due to variability in patient responses.

Case Presentation: We report a case of a 67-year-old male with a history of high-dose BZD use for panic disorder and depression and recent mild ischemic stroke, presenting with worsening left lower limb paralysis.

  • Resident physicians in Japan were expected to care for COVID-19 patients, but many faced burnout, with limited studies on their experiences and competency gains in this context.
  • A national study involving over 6,000 resident physicians assessed their training and COVID-19 care experience using the General Medicine In-Training Examination to measure competency.
  • Results indicated that while many residents had no COVID-19 care experience, those who did performed better on COVID-19-related questions, suggesting that this experience positively impacted their competency without hindering their fundamental clinical skills.

  • Skeletal muscle atrophy, often caused by muscle disuse, significantly affects quality of life, particularly in older adults and those with chronic illnesses, highlighting the need for understanding its molecular mechanisms for better treatment options.
  • The study investigated the roles of vascular endothelial growth factor (VEGF) and specific microRNAs (miRNAs) in muscle atrophy using a denervation model in mice, analyzing gene expression changes and regulatory networks.
  • Findings showed that denervation led to reduced muscle weight and altered gene expression, with VEGF and certain miRNAs being key regulatory factors, suggesting they may be promising targets for therapies aimed at combating muscle atrophy.

  • The study investigates treatment options and costs for malignant brain tumors (GBM and PCNSL) in Japan through a survey of 47 institutions.
  • For GBM, the most common treatment is 'Surgery + radiotherapy with temozolomide', costing approximately 7.50 million JPY, with additional treatments raising costs significantly.
  • PCNSL treatments, especially R-MPV therapy, also incur high costs, with some regimens exceeding 1 million JPY per month, highlighting the need for better cost-effectiveness assessments in future health studies.

  • Lifebuoy cataract is an uncommon congenital condition that leads to lens thinning, and this study aims to address the limited knowledge and treatment options available through detailed case reports and surgical techniques.
  • A 14-year-old boy with a history of untreated congenital cataract experienced worsening vision, leading to a diagnosis confirmed by various examinations, including slit-lamp microscopy and optical coherence tomography.
  • The surgical intervention involved a careful cataract surgery, highlighting techniques to handle complications like central calcification and residual opacification, although posterior capsulotomy was avoided due to risk of vitreous prolapse.

Purpose: The concept of non-mass abnormalities of the breast has been employed in Japan for approximately 20 years. Although B-mode findings are classified as non-mass abnormalities, the usefulness of adding color Doppler ultrasonography (US) and strain elastography to B-mode US is unclear. Therefore, we conducted a multicenter study (JABTS BC-07) to establish the diagnostic criteria for breast US, including color Doppler and elastography, for non-mass abnormalities of the breast and verify their diagnostic usefulness.

Article Synopsis
  • - Insulin autoimmune syndrome (IAS) causes high levels of insulin in the body, leading to low blood sugar (hypoglycemia) without external insulin use.
  • - The presence of insulin autoantibodies and low C-peptide levels can create confusion, but C-peptide is crucial for identifying the cause of hypoglycemia.
  • - A 47-year-old male nurse's case highlights the need to consider IAS in non-diabetics experiencing hypoglycemia, even when C-peptide levels are low.

Ceftriaxone and lansoprazole are commonly used in clinical settings, but recent analyses indicate a potential risk for QTc prolongation and cardiac events when used together. This case series examines three patients from a cohort of sudden death cases at a single institution over a decade, who received both medications within 24 hours before death. Three cases were identified, each with contributing factors for cardiac arrhythmias.
