Increasing evidence indicates that circadian and diurnal rhythms robustly influence stroke onset, mechanism, progression, recovery, and response to therapy in human patients. Pioneering initial investigations yielded important insights but were often single-center series, used basic imaging approaches, and used conflicting definitions of key data elements, including what constitutes daytime versus nighttime. Contemporary methodologic advances in human neurovascular investigation have the potential to substantially increase understanding, including the use of large multicenter and national data registries, detailed clinical trial data sets, analysis guided by individual patient chronotype, and multimodal computed tomographic and magnetic resonance imaging.

Introduction: The standard treatment for intrahepatic cholangiocarcinoma (iCCA) and mixed hepatocellular-cholangiocarcinoma (HCC-CC) is surgical resection, nevertheless, recent studies show adequate survival rates in selected patients with iCCA or HCC-CC undergoing liver transplantation (LT).

Methods: A retrospective cohort study was design including all patients undergoing LT at our center between January, 2006 and December, 2019 with incidentally diagnosed iCCA or HCC-CC after pathological examination of the explanted liver (n = 13).

Results: There were no iCCA or HCC-CC recurrences during the follow-up, and hence, there were no tumor related deaths.

Pyogenic liver abscesses in liver transplant recipients versus non-transplant population. Outcome and risk factors of patient survival.

Clin Transplant

June 2023

Unit of HPB Surgery and Abdominal Organ Transplantation, "Doce de Octubre" University Hospital, Department of Surgery, Faculty of Medicine, Complutense University, Madrid, Spain.

Pyogenic liver abscess (PLA) is a life-threatening infection in both liver transplant (LT) and non-LT patients. Several risk factors, such as benign and malignant hepatopancreatobiliary diseases and colorectal tumors have been associated with PLA in the non-LT population, and hepatic artery stricture/thrombosis, biliary stricture, and hepaticojejunostomy in the LT patients. The objective of this study is to compare the outcomes of patients with PLA in LT and non-LT patients and to determine the risk factors associated with patient survival.

Pregnancy and cancer share CTLA-4 and PD-1/PD-L1 as some of the immunomodulatory pathways that reshape the immune system from a destructive response to a state of tolerance to the fetus and the tumor, respectively. Ipilimumab (anti-CTLA-4 inhibitor) and nivolumab (anti-PD-1 inhibitor) are used in combination for the treatment of metastatic renal cell carcinoma, and their use could theoretically result in an immune response against the fetus. Furthermore, these immune checkpoint inhibitors are immunoglobulin G antibodies that transfer from the mother to the fetus and may cause a direct toxicity.

Germline mutations leading to aneuploidy are rare, and their tumor-promoting properties are mostly unknown at the molecular level. We report here novel germline biallelic mutations in , encoding the spindle assembly checkpoint (SAC) protein MAD1, in a 36-year-old female with a dozen of neoplasias. Functional studies demonstrated lack of full-length protein and deficient SAC response, resulting in ~30 to 40% of aneuploid blood cells.

Modifications in Abdominal Wall Graft Retrieval: When the Donor Closure Is Not Guaranteed.

Transplant Proc

November 2022

Unit of HPB Surgery and Abdominal Organ Transplantation, Department of General Surgery, "Doce de Octubre" University Hospital, Instituto de Investigación (Imas12), Faculty of Medicine, Complutense University, Madrid, Spain.

Background: Twenty percent of intestinal transplant recipients will require a surgical alternative to conventional primary abdominal wall closure. Abdominal wall transplant is a developing technique that is increasingly performed for this purpose in isolated intestinal or multivisceral recipients; however, adequate closure of the donor is paramount while simultaneously obtaining a large enough graft. The aim of this study is to describe alternative surgical techniques for closure of the donor in cases in which abdominal wall graft extraction hinders subsequent donor abdominal closure.

Immunotherapy including immune checkpoint inhibitors (ICIs) has become the backbone of treatment for most lung cancers with advanced or metastatic disease. In addition, they have increasingly been used for early stage tumors in neoadjuvant and adjuvant settings. Unfortunately, however, only a subset of patients experiences meaningful response to ICIs.

Article Synopsis
  • This study investigated the incidence and predictors of not recovering left ventricular function in patients experiencing cardiogenic shock due to myocardial infarction related to unprotected left main coronary artery (ULMCA).
  • Conducted at two tertiary care centers from 2012 to 2020, the study included 70 patients, finding that 58.6% faced death or required heart transplantation (HT) or left ventricular assist devices (LVAD) during admission, especially among those with severe shock and occluded LMCA.
  • Successful percutaneous coronary intervention (PCI) lowered overall risk but didn't significantly impact outcomes in the most complex cases; a predictive model identified key factors like left ventricular ejection fraction and severity of shock with a strong ability to forecast

Purpose: The aim of this work is to study a new analytical model which describes the dose-response curve in megavoltage photon beams of the radiochromic EBT3 film measured with two commercially available flatbed scanners. This model takes into account the different increase of the number of two types of absorbents in the film with absorbed dose and it allows to identify parameters that depend on the flatbed scanner and the film model, and parameters that exclusively depend on the production lot. In addition, the new model is also compared with other models commonly used in the literature in terms of its performance in reducing systematic calibration uncertainties.

Relation between EEG resting-state power and modulation of P300 task-related activity in theta band in schizophrenia.

Prog Neuropsychopharmacol Biol Psychiatry

June 2022

Psychiatry Department, School of Medicine, University of Valladolid, Valladolid, Spain; Psychiatry Service, University Hospital of Valladolid, Valladolid, Spain; Neuroscience Institute of Castilla y León (INCYL), University of Salamanca, Salamanca, Spain. Electronic address:

There is some consistency in previous EEG findings that patients with schizophrenia have increased resting-state cortical activity. Furthermore, in previous work, we have provided evidence that there is a deficit in the modulation of bioelectrical activity during the performance of a P300 task in schizophrenia. Our hypothesis here is that a basal hyperactivation would be related with altered ability to change or modulate cortical activity during a cognitive task.
