Centre d'Investigations Cliniques INSERM 1432 (CIC1432), Dijon Bourgogne University Hospital, Dijon, France.

Introduction: Proton pump inhibitors (PPIs) block the gastric H/K-ATPase, therefore inhibiting acid gastric secretion, leading to an increased pH (>4). They account for an extremely high number of prescriptions worldwide. Numerous drug-drug interactions have been described with PPIs, but all the described interactions do not have clinical significance.

Dynamic full-field optical coherence tomography as complementary tool in fungal diagnostics.

J Mycol Med

November 2022

Department of Parasitology/Mycology, Dijon Bourgogne University Hospital, 21000 Dijon, France; UMR PAM A 02.102 Procédés Alimentaires et Microbiologiques, Univ. Bourgogne Franche-Comté, AgroSup Dijon, Dijon, France.

Histopathology and microscopic examination of infected tissue are the gold standards to prove the diagnosis of invasive fungal infection (IFI). Yet, they suffer from essential limitations that hamper rapid diagnosis and require the future development of new imaging tools dedicated to fungal diagnostics. To this end, the present work introduces the first use of dynamic full-field optical coherence tomography (D-FF-OCT) for the visualization of microscopic filamentous fungi.

Article Synopsis
  • Stage IIB Hodgkin lymphoma (HL) patients with certain risk factors have a poor prognosis and are treated either as limited or advanced stage, with differing clinical trials comparing these treatment approaches.
  • A study involving 148 patients showed that baseline total metabolic tumor volume (TMTV) and responses after two cycles of chemotherapy significantly influenced progression-free survival rates, with a median follow-up of 4.1 years showing PFS rates around 88%.
  • The findings suggest that both upfront ABVD plus radiation therapy and upfront escBEACOPP without radiotherapy yield similar outcomes in these high-risk patients, while TMTV can effectively stratify their risk at baseline.

  • Lymphopenia, a condition where lymphocyte (a type of white blood cell) levels are low, is often found in sepsis patients and is linked to higher mortality rates.
  • The study examined if lymphopenia could be a reliable indicator of outcomes in patients with bacteremia, involving 574 individuals from two hospitals.
  • Results showed that initial lymphocyte counts and changes over the first four days did not predict 12-week mortality, suggesting the need for a better understanding of each patient’s immune profile before pursuing immune therapies.

  • Intellectual disability (ID) is often linked to genetic defects, with this study identifying 14 SEMA6B gene variants in patients diagnosed with ID but without epilepsy.
  • The researchers conducted experiments showing that these SEMA6B variants affect protein localization and lead to reduced spine density in both cell cultures and chicken embryos, indicating a significant role in neuron development.
  • The findings broaden the understanding of SEMA6B-related conditions, suggesting it is also a contributor to ID, expanding its clinical significance beyond previously known associations with epilepsy.

Early-onset forms of Alzheimer disease (AD) have been associated with pathogenic variants in the APP , PSEN1 , and PSEN2 genes. Mutations in presenilin-1 ( PSEN1 ) account for the majority of cases of autosomal dominant AD. Numerous phenotypes have been associated with PSEN1 -pathogenic variants, including cerebellar ataxia and spastic paraplegia.

Background: High-output heart failure is a rare condition that occurs when the heart is unable to respond to a sustained increase in blood demand. On echocardiography, a cardiac index of > 4 L/min/m (or 6 L/min) is a clear indicator of this disorder. The causes of high-output heart failure vary, but they all involve peripheral vasodilation or arteriovenous shunting.

Service de Réanimation Médico-Chirurgicale, Hôpital Avicenne, APHP, UFR SMBH, Université Sorbonne Paris Nord, Bobigny, French National Institute of Health and Medical Research (INSERM), Common and Rare kidney Diseases (CORAKID), Hôpital Tenon, Paris, France.

Background: Trials comparing early and delayed strategies of renal replacement therapy in patients with severe acute kidney injury may have missed differences in survival as a result of mixing together patients at heterogeneous levels of risks. Our aim was to evaluate the heterogeneity of treatment effect on 60-day mortality from an early vs a delayed strategy across levels of risk for renal replacement therapy initiation under a delayed strategy.

Methods: We used data from the AKIKI, and IDEAL-ICU randomized controlled trials to develop a multivariable logistic regression model for renal replacement therapy initiation within 48 h after allocation to a delayed strategy.
